Özet

Çalışmamız Serebral Palsili (SP) bireylerde müzik terapinin üst ekstremite becerileri, spastisite, selektif motor kontrol, duyusal fonksiyonlar ve yaşam kalitesi üzerine etkisini araştırmak amacıyla randomize kontrollü olarak planlandı. Katılımcılar müzik terapi grubu (MT) (n=14) ve kontrol grubu (KG) (n=14) olmak üzere randomize olarak ikiye ayrıldı. Kontrol grubuna kaba motor fonksiyonlara yönelik fizyoterapi ve üst ekstremite fonksiyonlarına yönelik fizyoterapi; Müzik terapi grubuna kaba motor fonksiyonlara yönelik fizyoterapi ve üst ekstremite fonksiyonlarına yönelik Nörokreatif Müzik Terapi (NKMT) yaklaşımı 8 hafta boyunca haftada 2 seans uygulandı. Spastisite Modifiye Ashworth Skalası (MAS), üst ekstremite becerileri Çocukların El Kullanım Deneyimi Anketi (CHEQ) ve Abilhand Kids ile, üst ekstremite kaslarının selektif kontrolü Üst Ekstremite Selektif Kontrol Skalası (SCUES) ile, duyusal fonksiyonlar Semmes Weinstein Monofilament Testi (SWMT) ve İki Nokta Diskriminasyon Testi (TPDT) ile, yaşam kalitesi Çocuklar İçin Yaşam Kalitesi Ölçeği SP Modülü 3.0 (PedsQL) ile değerlendirildi. Tedavi sonrası MT grubunda, baş parmak adduktörleri, el bileği fleksörleri ve total spastisitede KG'ye göre daha fazla azalma görüldü (p<0,05). Üst ekstremite becerilerinde tedavi sonrası her iki grupta da anlamlı gelişme görüldü (p<0,05), MT grubundaki gelişmeler daha fazlaydı (p<0,05). Kontrol grubunda sadece total selektif kontrolde gelişme görülürken (p<0,05), MT grubunda önkol, el bileği, parmaklar ve total selektif kontrol KG'ye göre daha fazla gelişme gösterdi (p<0,05). Her iki grupta da SWMT sonucunda anlamlı gelişme görüldü (p<0,05), ek olarak az etkilenimli tarafın SWMT sonucu da MT'de KG'ye göre daha fazlaydı (p<0,05). TPDT sonucu MT ve KG'de bir gelişme görülmedi (p>0.05), iki grup arasında fark yoktu (p>0.05). MT ve KG'nin her ikisinde de tedavi sonrası yaşam kalitesinde gelişme görüldü (p<0,001), MT grubundaki gelişme KG'ye göre anlamlı ölçüde daha fazlaydı (p<0,05). Sonuç olarak NKMT yaklaşımının SP'li bireylerin üst ekstremite becerilerini, selektif kontrolünü, yaşam kalitesini geliştirmede ve spastisiteyi regüle etmede etkili olduğu görüldü. NKMT'nin SP'li bireylerin rehabilitasyonunda güvenle kullanılabilecek efektif bir müzik terapi yaklaşımı olduğunu düşünmekteyiz. Taktil duyu parametrelerindeki sonuçlardan elde edilen farklılık nedeniyle NKMT'nin duyusal fonksiyonlardaki etkisi üzerine daha fazla çalışmaya ihtiyaç olduğu görüşündeyiz.

Our study was planned as a randomized controlled trial to investigate the effects of music therapy on upper extremity skills, spasticity, selective motor control, sensory functions and quality of life in individuals with cerebral palsy (CP). Participants were randomly divided into two groups: the music therapy group (MT) (n=14) and the control group (CG) (n=14). Classical physiotherapy for gross motor functions and classical physiotherapy for upper extremity functions were applied to the CG group; classical physiotherapy for gross motor functions and Neurocreative Music Therapy (NCMT) approach for upper extremity functions were applied to the MT group twice a week for 8 weeks. Spasticity was assessed with the Modified Ashworth Scale (MAS), upper extremity skills with the Children's Hand-use Experience Questionnaire (CHEQ) and Abilhand Kids, selective control of upper extremity muscles with the Selective Control of the Upper Extremity Scale (SCUES), sensory functions with the Semmes Weinstein Monofilament Test (SWMT) and Two-Point Discrimination Test (TPDT), and quality of life with the Pediatric Quality of Life Inventory CP Module 3.0 (PedsQL). After treatment, a more significant decrease was observed in thumb adductors, wrist flexors and total spasticity in the MT group compared to the control group (p<0.05). Significant improvement in upper extremity skills was seen in both groups after treatment (p<0.05), with more improvements in the MT group (p<0.05). While improvement was observed only in total selective control in the CG (p<0.05), forearm, wrist, fingers and total selective control showed more improvement in the MT group compared to the CG (p<0.05). There was a significant improvement in the SWMT result both in groups (p<0.05). However, the SWMT result of the less affected side was higher in MT than in CG (p<0.05). There was no significant improvement in the TPDT result in the MT group (p>0.05), there was no difference in the improvement in the CG compared to MT (p>0.05). There was an improvement in the quality of life after treatment in the MT and CG (p<0.001), the improvement in the MT group was more significant than in the CG (p<0.05). As a result, it was observed that the NCMT approach was effective in improving upper extremity skills, selective control, quality of life and regulating spasticity in individuals with CP. We believe that NCMT is an effective approach that can be used safely in the rehabilitation of individuals with CP. We think that further studies are needed to determine the effect of NCMT on sensory functions due to the difference obtained from the results in tactile sensory parameters.
