Özet

ÖZET Amaç Pulmoner karsinoid tümör ve büyük hücreli nöroendokrin karsinomlar (BHNK) akciğerin nadir görülen ve prognostik olarak farklı seyirler gösteren nöroendokrin tümörleridir. Pulmoner karsinoid tümörler genellikle düşük malignite potansiyeline sahipken BHNK'ler daha agresif seyir gösterebilmektedir. Çalışmamızda kliniğimizde akciğer rezeksiyonu uygulanan pulmoner karsinoid tümör ve BHNK vakalarının cerrahi ve klinikopatolojik olarak mortalite, morbidite ve sağkalım oranlarının retrospektif olarak incelenmesini amaçladık. Bu amaçla toplanan veriler ve sonuçları, gruplar ve alt tipleri arasında karşılaştırma yapılarak literatür eşliğinde tartışıldı. Yöntem ve Gereç Bu çalışmada, Ocak 2009 – Aralık 2023 tarihleri arasında SBÜ Ankara Atatürk Sanatoryum Sağlık Uygulama ve Araştırma Merkezi'nde, pulmoner karsinoid tümör ve BHNK tanısıyla akciğer rezeksiyonu ve lenf nodu diseksiyonu uygulanan 189 hasta retrospektif olarak değerlendirilmiştir. Hastaların preoperatif verileri kapsamında; yaş, cinsiyet, sigara öyküsü, eşlik eden hastalıklar, Amerikan Cerrahlar Koleji Kalite İyileştirme Programı (ACS-NSQIP) skoru ve Avrupa Kalp Cerrahisi Risk Değerlendirme Sistemi (EuroSCORE II) ile belirlenen cerrahi risk skorları; tanı ve evreleme amacıyla yapılan bilgisayarlı tomografi (BT), pozitron emisyon-bilgisayarlı tomografi (PET-BT) ile maksimum standart tutulum değeri (SUVmax) ve bronkoskopi ile preoperatif olarak saptanan endobronşiyal lezyon (EBL) varlığı değerlendirilmiştir. Operatif süreçte; uygulanan cerrahi yaklaşım ve prosedürler, tümörün anatomik lokalizasyonu, lenf nodu diseksiyonu durumu, çıkarılan lenf nodu sayısı ve tümör boyutu kaydedilmiştir. Operasyon sonrası gelişen komplikasyonlar Clavien-Dindo skorlama sistemi ile derecelendirilmiştir. Postoperatif değerlendirme kapsamında patolojik incelemede; Dünya Sağlık Örgütü (DSÖ) 2021 sınıflandırmasına göre belirlenen tümör alt tipleri tipik karsinoid (TK), atipik karsinoid (AK), BHNK, kombine BHNK (KBHNK) olarak 4 gruba ayrılmıştır. Ayrıca bu gruplar mitotik aktivite, nekroz varlığı, Ki-67 proliferasyon indeksi , visseral plevra invazyonu, lenf nodu metastazı, cerrahi rezeksiyon sınırları açısından incelenmiştir. Sekizinci ve dokuzuncu Tümör, Nod, Metastaz (TNM) evreleme sistemine göre evrelendirilmiştir. Postoperatif takip süresince uygulanan adjuvan tedaviler, gelişen nüks, metastaz durumları ve sağkalım verileri analiz edilmiştir. İstatistiksel analizler Statistical Package for the Social Sciences (SPSS) 26.0 yazılımı ile gerçekleştirilmiş; gruplar arası karşılaştırmalarda Ki-kare testi ve Bonferroni düzeltmesi, korelasyonlar için Spearman analizi, mortalite için lojistik regresyon ve sağkalım analizleri için Kaplan-Meier yöntemi ile Log-Rank testi kullanılmıştır. Anlamlılık düzeyinde p<0,05 olarak kabul edilmiştir. Bulgular Çalışmaya dahil edilen 189 hastanın yaş ortalaması 51,5 olup, %82,5'i (N=156) 65 yaş altındaydı. Cinsiyet dağılımı incelendiğinde hastaların %57,1'i (N=108) erkek %42,9 (N=81) kadındı. Cerrahi yaklaşımda en sık tercih edilen yöntem torakotomi olup, hastaların %66,7'sine (N=126) uygulandı. Video yardımlı torakoskopik cerrahi (VATS) uygulanan hastaların oranı %18,5 (N=35) iken, %13,2'sine (N=25) VATS+torakotomi uygulandı. Hastaların hepsine anatomik akciğer rezeksiyonu ve sistematik lenf nodu diseksiyonu uygulandı. Operasyon sonrası çıkarılan tümörlerin histolojik alt tipleri DSÖ 2021 sınıflamasına göre gruplandırıldı. Hücre tipi dağılımında en sık TK (%48,7, N=92), ardından AK (%27,0 N=51), BHNK ve KBHNK ise %24,4 (N=23) oranında izlendi. Mitotik aktiviteye göre değerlendirildiğinde, olguların %50,3'ünde (N=95), 2 ve daha az mitoz, %25,9'unda (N=49), 2-10 arası, %23,8'inde (N=45) ise 10 ve üzeri mitotik aktivite gözlendi. Ki-67 proliferasyon indeksi %53,4 hastada (N=101) %5 ve altında, %24,3 hastada (N=46) %5-30 arası, %22,2 (N=42) hastada ise %30 ve üzeri olarak tespit edildi. Nekroz varlığı %22,8 oranında, visseral plevra invazyonu ise %14,3 oranında saptandı. Ayrıca, EuroSCORE II ve ACS NSQIP skorları da preoperatif risk değerlendirmesinde kullanıldı ve mortalite ile anlamlı ilişkili bulundu. Tümör evresi dağılımında en yaygın evreler T1b, T1c ve T2a olarak belirlendi; ileri evre tümörler (T3–T4) daha nadir izlendi. Toplam nüks-metastaz oranı %8,5 (N=22) olarak bulundu. Ortalama sağkalım süresi 2 yılda 23,3 ay, 5 yılda 56,0 ay ve genel sağkalımda 79,4 ay olarak hesaplandı. Mortalite üzerine anlamlı etkisi olan faktörler arasında ileri yaş, erkek cinsiyet, agresif hücre tipi (BHNK, KBHNK), yüksek mitoz ve Ki-67 düzeyi, nekroz varlığı, visseral plevra invazyonu, artmış tümör boyutu, ileri evre, N1 lenf nodu tutulumu ve adjuvan tedavi gereksinimi yer aldı. Bu faktörlerin sağkalım üzerindeki etkisi Kaplan-Meier yöntemi ile yapılan analizlerde istatistiksel olarak doğrulandı. Sonuç Pulmoner nöroendokrin tümörler (NET) hücre tipine bağlı olarak sağkalım ve mortalite açısından belirgin farklılıklar göstermektedir. BHNK ve KBHNK alt tipleri, karsinoid tümörlere göre daha düşük sağkalım oranlarına sahiptir. EuroSCORE ve ACS-NSQIP gibi risk skorları, preoperatif mortalite tahmininde anlamlı bulunmuş; komorbidite kontrolünün önemi bir kez daha vurgulanmıştır. Sağkalımı olumsuz etkileyen başlıca faktörler; yüksek mitotik aktivite, yüksek Ki-67 indeksi, nekroz, visseral plevra invazyonu, ileri evre ve lenf nodu metastazıdır. Adjuvan tedavi, özellikle yüksek dereceli tümörlerde ve bu risk faktörleri varlığında gündeme gelmektedir. Bu karar süreci, cerrahi, onkoloji ve patoloji bölümlerinin yer aldığı multidisipliner yaklaşımı gerektirmektedir. Her hastanın tümör alt tipi ve risk profiline göre bireysel olarak değerlendirilmesi tedavi başarısı açısından kritik önemdedir.

ABSTRACT Purpose Pulmonary carcinoid tumors and large cell neuroendocrine carcinomas (LCNEC) are rare neuroendocrine tumors of the lung that exhibit distinct prognostic behaviors. While pulmonary carcinoid tumors typically have low malignant potential, LCNECs tend to follow a more aggressive clinical course. This study aimed to retrospectively evaluate the clinical, surgical, and clinicopathological outcomes—specifically mortality, morbidity, and survival—of patients with pulmonary carcinoid tumors and LCNEC who underwent lung resection in our center. The collected data and results were compared across tumor subgroups and discussed in light of current literature. Materials and Methods In this retrospective study, 189 patients who underwent lung resection and lymph node dissection for pulmonary carcinoid tumor or large-cell neuroendocrine carcinoma (LCNEC) between January 2009 and December 2023 at the University of Health Sciences, Ankara Atatürk Sanatorium Health Practice and Research Center were evaluated. Eleven patients were excluded due to incomplete electronic or archival data. Preoperative variables included age, sex, smoking history, comorbidities, surgical risk scores based on the American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) and the European System for Cardiac Operative Risk Evaluation (EuroSCORE II), computed tomography (CT), maximum standardized uptake value (SUVmax) from positron emission tomography–computed tomography (PET-CT), and preoperatively detected endobronchial lesion (EBL) via bronchoscopy. During the operative period, the surgical approach and procedure performed, anatomical tumor localization, lymph node dissection status, number of resected lymph nodes, and tumor size were recorded. Postoperative complications were graded using the Clavien-Dindo classification system. In the postoperative evaluation, tumors were histopathologically classified according to the 2021 World Health Organization (WHO) classification as typical carcinoid (TC), atypical carcinoid (AC), LCNEC, and combined LCNEC (c-LCNEC). These groups were further analyzed in terms of mitotic activity, presence of necrosis, Ki-67 proliferation index, visceral pleural invasion, lymph node metastasis, and resection margin status. Tumors were staged according to the 8th and 9th editions of the Tumor, Node, Metastasis (TNM) classification system. Data on adjuvant therapies administered in the postoperative period, recurrence, metastasis, and survival were also collected and analyzed. Statistical analyses were performed using the Statistical Package for the Social Sciences (SPSS) version 26.0. The chi-square test with Bonferroni correction was used for group comparisons, Spearman correlation was applied for relationship analyses, logistic regression was used to assess mortality, and Kaplan-Meier with Log-Rank test was utilized for survival analysis. A p-value of <0.05 was considered statistically significant. Results The mean age of the 189 patients included in the study was 51.5 years, with 82.5% under the age of 65. Of the patients, 57.1% were male. The most frequently used surgical approach was thoracotomy (66.7%), followed by video-assisted thoracoscopic surgery (VATS) in 18.5% and combined VATS-thoracotomy in 13.2% of cases. All patients underwent anatomical lung resection with systematic lymph node dissection. Histopathological classification of resected tumors was made according to the 2021 WHO criteria. Tumor type distribution showed typical carcinoid (TC) as the most frequent (48.7%), followed by atypical carcinoid (AC) (27.0%), and LCNEC or c-LCNEC (24.4%). Regarding mitotic activity, 50.3% of tumors had ≤2 mitoses, 25.9% had 2–10, and 23.8% had ≥10 mitoses. The Ki-67 proliferation index was ≤5% in 53.4% of patients, moderate in 24.3%, and high in 22.2%. Necrosis was observed in 22.8% of tumors, and visceral pleural invasion in 14.3%. EuroSCORE II and ACS NSQIP scores were used in preoperative risk stratification and were found to be significantly associated with mortality. The most frequent tumor stages were T1b, T1c, and T2a, while advanced-stage tumors (T3–T4) were less common. The overall recurrence/metastasis rate was 8.5%. Mean survival was calculated as 23.3 months at 2 years, 56.0 months at 5 years, and 79.4 months for overall survival. Factors significantly associated with mortality included older age, male sex, aggressive histologic subtype (LCNEC, c-LCNEC), high mitotic count and Ki-67 index, necrosis, visceral pleural invasion, larger tumor size, advanced stage, N1 lymph node involvement, and need for adjuvant therapy. The prognostic impact of these factors on survival was statistically confirmed by Kaplan-Meier survival analysis. Conclusion Pulmonary neuroendocrine tumors (NETs) show significant differences in survival and mortality based on histologic subtype. LCNEC and c-LCNEC subtypes are associated with lower survival rates compared to carcinoid tumors. Risk scores such as EuroSCORE II and ACS NSQIP were found to be significant in predicting preoperative mortality, emphasizing the importance of managing comorbidities. The main factors negatively affecting survival were high mitotic activity, elevated Ki-67 index, necrosis, visceral pleural invasion, advanced stage, and lymph node metastasis. Adjuvant therapy should be considered in high-grade tumors and in the presence of these risk factors. Treatment planning should involve a multidisciplinary approach including thoracic surgery, oncology, and pathology, with individual evaluation based on tumor subtype and patient risk profile being critical for optimal outcomes.
