Özet
Amaç: Bu çalışmanın amacı, punktum stenozunun altında yatan patofizyolojik mekanizmanın açıklanmasına katkıda bulunmak amacı ile, primer ve rekürren punktum stenozu nedeni ile rektanguler 3-snip punktoplasti uygulanan hastalarda punktumun histopatolojik ve elektron mikroskopik incelemesinin yapılmasıdır. Elektron mikroskopi incelemesi ile punktum stenozunda hücresel düzeyde meydana gelen değişiklikler değerlendirilecek, ışık mikroskopi incelemesi ile histopatolojik değişiklikler kaydedilerek literatürde ilk kez, primer ve rekürren olgular arasında histopatolojik ve elektron mikroskopik özellikler açısından fark bulunup bulunmadığı araştırılacaktır. Ayrıca bu bulguların cerrahi başarı ile olan ilişkileri değerlendirilecektir. Gereç ve Yöntem: Ankara Etlik Şehir Hastanesi Göz Hastalıkları Kliniği'nde gerçekleştirilen prospektif, randomize, karşılaştırmalı vaka serisi olarak planlanan bu çalışmaya epifora şikayeti ile başvurarak punktum stenozu tanısı alan ve punktoplasti uygulanan toplam 45 hastanın 60 gözü dahil edildi. Daha önce punktum stenozu nedeni ile operasyon geçirmemiş hastalar primer punktum stenozu (n=51), daha önce punktoplasti geçiren ve restenoz gelişmiş olan hastalar rekürren punktum stenozu (n=9) olarak kabul edildi. Punktoplasti sonrasında elde edilen örneklerin tümüne ışık mikroskopi ile histopatolojik inceleme yapılırken, bunların arasından toplam 10 örneğe ayrıca elektron mikroskopi incelemesi yapıldı. Elektron mikroskopi incelemesi yapılan örneklerin 5 tanesi primer, 5 tanesi ise rekürren olgulardan oluşuyordu. Hastaların demografik özellikleri, anatomik ve fonksiyonel başarı oranları, elde edilen histopatoloji ve elektron mikroskopi sonuçları kaydedildi. Histopatolojik incelemede epitel tipi, goblet hücre varlığı, fibrozis ve inflamasyon kaydedilirken, elektron mikroskopi incelemesinde ince yapısal düzeydeki bulgular belirlenerek primer ve rekürren olgular arasındaki farklar ve benzerlikler karşılaştırıldı. Bulgular: Hastaların yaş ortalaması 62,22 ± 6,87 yıl olup %57,8'i kadın, %42,2'si erkekti. Ortalama Munk skoru cerrahi öncesi 3,22 ± 0,45 iken cerrahi sonrası 0,73 ± 0,84'e geriledi (p<0,001). Anatomik başarı oranı %90 (54/60), fonksiyonel başarı oranı %85 (51/60) olarak saptandı. Primer ve rekürren olgulardaki cerrahi başarı oranlarına ayrı ayrı bakıldığında ise, primer stenoz grubunda anatomik başarı oranı %94,1 ve fonksiyonel başarı %88,2 iken rekürren cerrahi grubunda ise her iki başarı oranı %66,7 olarak saptandı. Rekürren grupta anatomik başarı oranı anlamlı olarak düşük düzeyde saptandı. Histopatolojik olarak, örneklerin %91,7'sinde çok katlı yassı epitel, %55'inde goblet hücresi, %95'inde fibrozis ve %93,3'ünde inflamasyon saptandı. Goblet hücre kaybının rekürren grupta daha fazla olduğu saptandı (p=0,032). Epitel tipi, fibrozis ve inflamasyon açısından gruplar arasında fark izlenmedi. Elektron mikroskopik incelemelerde primer gruptan farklı olarak, rekürren grupta epitel hücreleri arasında belirgin ayrışmalar, interselüler ödem, aktif fibroblastlar ve yoğun kollajen birikimi gözlendi. Lojistik regresyon analizine göre yalnızca rekürren cerrahi öyküsü, nüks için anlamlı risk faktörüydü (p=0,044, Nagelkerke R²=0,229). Sonuç: Sonuç olarak çalışmamız literatürde ilk kez rekürren olgularda epitel hücreleri arasındaki mesafenin arttığı ve doku arasında yoğun ödem izlendiğini ve yine bu olgularda cerrahi sonrası restenoz gelişmesi riskinin daha yüksek olduğunu göstermiştir. Mevcut literatür ışığında, primer ve rekürren punktum stenozunu histopatolojik ve ultrastrüktürel düzeyde karşılaştıran ilk çalışma olması nedeniyle, bu araştırma hastalığın patogenezi ve restenoz gelişiminin aydınlatılmasında gelecekte yapılacak çalışmalara öncülük edebilecek bilgiler sağlamaktadır.

Objective: Our study aims to contribute to the explanation of the pathophysiological mechanism underlying punctal stenosis by performing histopathological and electron microscopic examinations of the punctum in patients who underwent rectangular 3-snip punctoplasty due to primary or recurrent punctal stenosis. Cellular-level alterations in punctal stenosis will be evaluated through electron microscopy, while histopathological changes will be documented using light microscopy. For the first time in the literature, differences in histopathological and electron microscopic features between primary and recurrent cases will be investigated. Furthermore, the relationship between these findings and surgical success will be evaluated. Materials and Methods: A total of 60 eyes from 45 patients who presented with complaints of epiphora and were diagnosed with punctal stenosis and underwent punctoplasty were included in this prospective, randomized, comparative case series conducted at the Department of Ophthalmology, Ankara Etlik City Hospital. Patients who had not undergone any previous surgery for punctal stenosis were classified as primary punctal stenosis (n=51), while patients who developed restenosis after previous punctoplasty were considered as recurrent punctal stenosis (n = 9). All tissue samples obtained following punctoplasty were subjected to histopathological examination under light microscopy, and among these, a total of 10 samples were further examined using electron microscopy. Five of the electron microscopy samples were from primary cases, and five were from recurrent cases. Demographic characteristics of the patients, anatomical and functional success rates, as well as the results of histopathological and electron microscopic analyses were recorded. In the histopathological evaluation, the type of epithelium, presence of goblet cells, fibrosis, and inflammation were documented. In the electron microscopic evaluation, ultrastructural findings were identified and the differences and similarities between primary and recurrent cases were compared. Results: The mean age of the patients was 62.22 ± 6.87 years, with 57.8% being female and 42.2% male. The mean preoperative Munk score was 3.22 ± 0.45, which significantly decreased to 0.73 ± 0.84 postoperatively (p<0.001). The anatomical success rate was 90% (54/60), and the functional success rate was 85% (51/60). When surgical success rates were analyzed separately for primary and recurrent cases, the anatomical success rate was 94.1% and the functional success rate was 88.2% in the primary stenosis group, while both success rates were 66.7% in the recurrent surgery group. Anatomical success was significantly lower in the recurrent group. Histopathologically, stratified squamous epithelium was observed in 91.7% of samples, goblet cells in 55%, fibrosis in 95%, and inflammation in 93.3%. Goblet cell loss was found to be significantly higher in the recurrent group (p=0.032). No significant differences were observed between groups in terms of epithelial type, fibrosis, or inflammation. In electron microscopic examinations, in contrast to the primary group, the recurrent group showed marked intercellular separation among epithelial cells, intercellular edema, active fibroblasts, and dense collagen accumulation. According to logistic regression analysis, a history of recurrent surgery was the only significant risk factor for restenosis (p=0.044, Nagelkerke R²=0.229). Conclusion: In conclusion, our study is the first in the literature to demonstrate that, in recurrent cases, there is an increased intercellular distance among epithelial cells and prominent interstitial edema, along with a higher risk of postoperative restenosis. Based on the current body of literature, this study represents the first to compare primary and recurrent punctal stenosis at both histopathological and ultrastructural levels, providing valuable insights that may contribute to clarifying the pathogenesis of the disease and informing future research on restenosis.
