Özet
Giriş: Nörodejeneratif süreçlerde, erken evrelerde ve hatta motor semptomlar ortaya çıkmadan önce dahi otonom sinir sistemi (OSS) değişen derecelerde etkilenebilir. OSS'in fonksiyonları sempatik deri yanıtları (SDY) ve R-R intervali değişkenliği (RRID) ile değerlendirilebilir. Bu çalışmada amaç, nörodejeneratif süreçlerin erken dönemlerinde görülebilen asimetrik kortikal atrofinin (AKA), OSS üzerindeki etkisinin incelenmesidir. Metot ve Bulgular: Yaş ve cinsiyet dağılımları benzer olan nörodejeneratif süreçlere bağlı AKA izlenen 23 hasta ve 22 sağlıklı katılımcı çalışmaya dahil edildi ve elektrofizyolojik incelemeleri yapıldı. SDY'de asimetriyi tanımlamak amacıyla, SDY latans ve amplitüdleri için asimetri indeksi (ASİ) oluşturuldu. Hem elektrik hem akustik uyarımla hem üst hem alt ekstremiteden elde edilen SDY amplitüdleri hasta grubunda daha düşük bulundu (p=0,037; p=0,032; p=0,034; p=0,002). Elektrik uyarımla elde edilen SDY latansları ise hasta grubunda daha uzun izlendi (p<0,001). Elektrik uyarımla üst ekstremiteden elde edilen SDY amplitüdlerinin ASİ değeri hasta grubunda daha yüksek bulundu (p=0.038). Ayrıca hasta grubunun SDY amplitüdleri, hastaların korteks atrofisiyle uyumlu olarak, atrofik tarafın kontralateralinde, elektrik uyaranla üst ve alt ekstremitede ve sesli uyaranla üst ve alt ekstremitede sırasıyla katılımcıların %83,4, %78,2 %69,56 ve %60,86'sında düşük bulundu. Hiperventilasyon sırasında kaydedilen RRID, hasta grubunda anlamlı olarak düşük izlendi (p<0,001). Benzer şekilde istirahat / hiperventilasyon RRID oranı iki grup arasında farklılık göstermekteydi (p<0,001). Sonuç: Asimetrik SDY saptanması, klinik şüphe olan hastalarda nörodejeneratif süreçleri düşündüren bir bulgu olabileceği; ayrıca SDY ve RRID gibi tekniklerin nörodejeneratif hastalıkların erken tanısında biyobelirteç olarak faydalı olabileceği düşünülmektedir. Daha yüksek katılımcı sayılarına ulaşan geniş çalışmalara ihtiyaç vardır.

Introduction: In neurodegenerative processes, the autonomic nervous system (ANS) can be affected to varying degrees in the early stages, even before the onset of motor symptoms. The functions of the ANS can be evaluated by sympathetic skin responses (SSR) and R-R Interval variation (RRIV). The aim of this study is to investigate the effect of asymmetric cortical atrophy (ACA), which can be observed in the early stages of neurodegenerative processes, on the ANS. Method and Findings: Twenty-three patients with ACA due to neurodegenerative processes and 22 healthy participants with similar age and gender distributions were included in the study, and their electrophysiological examinations were performed. To define asymmetry in SSR, an asymmetry index (AI) was created for SSR latencies and amplitudes. SSR amplitudes obtained from both upper and lower extremities with both electrical and acoustic stimulation were found to be lower in the patient group (p=0.037; p=0.032; p=0.034; p=0.002, respectively). SSR latencies obtained with electrical stimulation were observed to be longer in the patient group (p<0.001). The AI value for SSR amplitudes obtained from the upper extremity with electrical stimulation was found to be higher in the patient group (p=0.038). Furthermore, consistent with the patients' cortical atrophy, SSR amplitudes in the patient group were found to be lower on the side contralateral to the atrophy. Specifically, with electrical stimulation, amplitudes were low in 83.4% of participants for the upper extremity and 78.2% for the lower extremity. With acoustic stimulation, they were low in 69.56% for the upper extremity and 60.86% for the lower extremity. RRIV recorded during hyperventilation was observed to be significantly lower in the patient group (p<0.001). Similarly, the rest/hyperventilation RRIV ratio differed between the two groups (p<0.001). Conclusion: It is suggested that the detection of asymmetric SSR may be a finding suggestive of neurodegenerative processes in patients with clinical suspicion. Furthermore, techniques such as SSR and RRIV are thought to be potentially useful as biomarkers in the early diagnosis of neurodegenerative diseases. Large-scale studies with a higher number of participants are needed.
