Özet
Giriş ve Amaç: Kemik tümörleri, pediatrik yaş grubunda nadir görülen tümörler arasında olup sağ kalım oranları erişkine kıyasla çok daha yüksektir. Antrasiklinler, kemik tümörlerinin tedavi protokollerinde önemli bir yer tutmakta olup kardiyotoksisite, bu ilaçların iyi bilinen bir yan etkisidir. Çalışmamızda, antrasiklin kardiyotoksisitesini etkileyebilecek faktörleri ve kardiyotoksisiteyi saptamada kardiyak manyetik rezonans (CMR) görüntülemenin yerini değerlendirdik. Gereç ve Yöntem: Merkezimizde 1991-2024 yılları arasında doxorubisin içeren kemoterapi protokolleri almış olan 0-25 yaş aralığındaki 208 kemik sarkomlu hastanın dosyaları ve EKO bulguları retrospektif olarak incelendi. Hastaların cinsiyeti, tanısı, tanı yaşı, primer lezyon yeri, prematürite öyküsü, daha önce pnömoni ve/veya miyokardit geçirme öyküsü, covid-19 geçirme öyküsü, metastaz varlığı ve varsa yeri, hastalığın risk grubu, hastaya uygulanan kemoterapi protokolü ve bu protokole ait aldığı ilaçların kümülatif dozları, hastanın aldığı toplam eritrosit transfüzyon sayısı, ferritin düzeyi, radyoterapi dozu kaydedildi. Tedavi sonrası CMR görüntüleri de olan 31 hastanın CMR verileri tedavi sonrası ekokardiyografi verileri ile kıyaslandı. Bulgular: Tedavi sonrası ortanca 20 aylık izlemde 15 hastada kardiyotoksisite geliştiği görüldü. Hastaların tedavi öncesi ve sonrası EF(%) ortalaması 68.51±4.68, ve 64.99±6.89 olarak (z=5.039,p<0.001), tedavi öncesi ve sonrası LVDd ortalaması 38.31±5.72mm ve 40.54±5.81mm olarak saptandı(z=2.794,p=0.005). Kemoterapiye bağlı LVEF değerlerinde %60'ın altına düşme riskleri incelendiğinde myokardit öyküsü olanlarda bu riskin 10.9 kat(p=0.004), eritrosit transfüzyon sayısının ise 1.1 kat arttırdığı görüldü(p=0.018). Kemoterapiye bağlı LVEF değerlerinde yaşa göre sistolik disfonksiyon sınırının altına düşme riskleri incelendiğinde ise uygulanan kemoterapi protokolü dışında (HR:4.16, p=0,044) risk faktörü saptanmadı. Kardiyotoksisiteyi saptamada tedavi sonrası bakılan EKO'da duyarlılık %53.33, seçicilik %100.00, pozitif tahmin değeri %100.00, negatif tahmin değeri %96.39 olarak, CMR için duyarlılık %100.00, seçicilik %100.00, PPV %100.00, NPV %100.00 olarak bulundu. Sonuç: Çalışmamız doksorubisin uygulanan hastalarda tedavi sonrasında ekokardiyografi bulgularına göre LVEF değerlerinde anlamlı düşüşlerin olduğunu göstermektedir. Bu kardiyotoksisiteyi tanımada CMR görüntülemenin duyarlılık ve seçiciliği, ekokardiyografiye göre anlamlı ölçüde yüksek saptanmıştır. Eritrosit transfüzyon sayısındaki artış ve geçmişte myokardit öküsü olması, tedavi sonrası LVEF'nin %60'ın altına düşüşünde etkili bulunmuştur.

Introduction and Aim: Bone tumors are rare tumors in the pediatric age group, and survival rates are much higher than in adults. Anthracyclines have an important place in the treatment protocols of bone tumors, and cardiotoxicity is a well-known side effect of this disease. In our study, we evaluated the factors that may affect anthracycline cardiotoxicity and the role of cardiac magnetic resonance (CMR) imaging in detecting cardiotoxicity. Materials and Methods: The files and echocardiographic findings of 208 bone sarcoma patients aged 0-25 years who received doxorubicin-containing chemotherapy protocols in our center between 1991-2024 were retrospectively reviewed. The gender, diagnosis, age at diagnosis, primary lesion location, history of prematurity, history of previous pneumonia and/or myocarditis, history of COVID-19, presence of metastasis and location if any, risk group of the disease, chemotherapy protocol applied to the patient and cumulative doses of drugs received under this protocol, total number of erythrocyte transfusions received by the patient, ferritin level, and radiotherapy dose were recorded. CMR data of 31 patients who also had post-treatment CMR images were compared with post-treatment echocardiography data. Results: During a median follow-up of 20 months after treatment, cardiotoxicity was observed in 15 patients. The mean EF(%) of the patients before and after treatment was 68.51±4.68 and 64.99±6.89 (z=5.039, p<0.001), and the mean LVDd before and after treatment was 38.31±5.72mm and 40.54±5.81mm (z=2.794, p=0.005). When the risks of decreasing below 60% in LVEF values due to chemotherapy were examined, it was seen that this risk increased 10.9-fold in those with a history of myocarditis (p=0.004), and 1.1-fold with the number of erythrocyte transfusions (p=0.018). When the risks of falling below the systolic dysfunction limit in LVEF values due to chemotherapy according to age were examined, no risk factor was detected except the applied chemotherapy protocol (HR: 4.16, p=0.044). In the ECHO performed after treatment in detecting cardiotoxicity, sensitivity was found to be 53.33%, selectivity was 100.00%, positive predictive value was 100.00%, negative predictive value was 96.39%, and for CMR, sensitivity was found to be 100.00%, selectivity was 100.00%, PPV was 100.00%, and NPV was found to be 100.00%. Conclusion: Our study shows that there are significant decreases in LVEF values in patients treated with doxorubicin according to echocardiography findings after treatment. The sensitivity and specificity of CMR imaging in recognizing this cardiotoxicity were found to be significantly higher than echocardiography. The increase in the number of erythrocyte transfusions and the presence of myocarditis in the past were found to be effective in the decrease of LVEF below 60% after treatment. In the ECHO&CMR performed after treatment in detecting cardiotoxicity, sensitivity was found to be 53.33%&100.00% selectivity was 100.00%&100.00% positive predictive value was 100.00%&100.00% negative predictive value was 96.39%&100.00%. Conclusion:Our study shows that there are significant decreases in LVEF in patients receiving doxorubicin and that CMR better detects developing cardiotoxicity.It was found that increasing the number of erythrocyte transfusions and a history of myocarditis were effective in decreasing LVEF below 60% after treatment.
