Özet
Oğuzhan Karayaka, Genel Anestezi İdame Yöntemlerinde Peroperatif Gelişen Atelektazinin Akciğer Ultrason Skoru ile Değerlendirilmesi, Zonguldak Bülent Ecevit Üniversitesi, Tıp Fakültesi, Anesteziyoloji ve Reanimasyon Anabilim Dalı, Tıpta Uzmanlık Tezi, Zonguldak, 2025. Amaç: Çalışmamızda, farklı genel anestezi idame yöntemlerinin peroperatif dönemde gelişen atelektazi üzerine etkilerinin akciğer ultrason skoru (LUSS) ile değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Genel anestezi altında septoplasti ve rinoplasti operasyonu uygulanacak, 18-65 yaş arası, ASA I-II risk grubunda 97 hasta çalışmaya dahil edildi. Rutin hemodinamik monitörizasyon ve anestezi indüksiyonunu takiben hastalar anestezi idamesi için sevofluran (Grup S, n=30), desfluran (Grup D, n=30) ve propofol bazlı total intravenöz anestezi (Grup P, n=30) gruplarına ayrıldı. Hastaların demografik ve hemodinamik verileri, sayısal değerlendirme ölçeği (NRS) ile analjezik ihtiyacı, sigara kullanımı ve miktarı, en son sigara içme saati ve preoperatif (T0), entübasyon sonrası 10. dk (T1), cerrahi bitiminde (T2), ekstübasyon sonrası 20. dk (T3) zaman aralıklarında LUSS değerleri kaydedildi. Bulgular: Toplam LUSS değerleri, bazal ölçümlerle karşılaştırıldığında T2 (p<0.001) ve T3 (p<0.001) ölçümlerinde tüm gruplarda artış göstermiştir. T3 ölçümlerinde gruplar arasında toplam LUSS ve ΔLUSS değerleri açısından fark saptanmamıştır (p>0.05). Toplam LUSS değerleri Grup D'de Grup S'den T1 (p=0.002) ve T2'de (p=0.007), Grup P'den ise T2'de (p=0.008) yüksek bulunmuştur. Toplam LUSS değerleri sigara içen hastalarda T0 (p<0.001) ve T1'de (p=0.002), sigara içmeyenlere kıyasla daha yüksek bulunmuştur. Sigara içen hastalarda T0'da Grup S (p=0.027) ve Grup D'de (p=0.004), içmeyenlere göre daha yüksek LUSS değerleri izlenmiştir. T1'de ise bu fark yalnızca Grup D'de anlamlı bulunmuştur (p=0.001). Toplam LUSS değerleri T1'de sigara içen hastalarda Grup D'de Grup S'ye oranla daha yüksek bulunmuştur (p=0.009). Sigara içmeyen hastalarda toplam LUSS değerleri Grup D'de; T2'de Grup S (p=0.005) ve Grup P'ye (p=0.010) göre daha yüksek, T3'te ise yalnızca Grup S'ye göre yüksek bulunmuştur (p=0.012). Entübasyon süresi en uzun Grup P'de saptanmışken (p=0.002), derlenme süresi en kısa Grup D'de izlenmiştir (p=0.001). NRS skorları T3'te Grup D'de, Grup P'ye oranla daha yüksek bulunmuştur (p=0.015). Sonuç: Çalışmamızda, her üç genel anestezi idame yöntemin peroperatif dönemde LUSS değerlerinde benzer artışa neden olduğu saptanmıştır. İntraoperatif süreçte desfluran kullanımına bağlı LUSS artışı daha belirgin görülmekle birlikte zaman içinde LUSS değişimi açısından yöntemler arasında anlamlı fark saptanmamıştır. Bu durum, farklı anestezi idame yöntemlerinin peroperatif dönemde atelektazi gelişimine etkilerinin benzer düzeyde olabileceğini düşündürmektedir. Ancak gruplar arasında sigara içme durumuna göre toplam LUSS değerleri karşılaştırıldığında desfluran kullananlarda sigara içenlerde entübasyon sonrası 10. dk'da içmeyenlerde ise cerrahi sonu değerlerinin sevofluran kullananlara oranla daha yüksek olduğu bu nedenle intraoperatif atelektazi gelişimi açısından desfluran kullanımı sırasında dikkat edilmesi gerektiği düşünüldü. Oğuzhan Karayaka, Evaluation of Peroperative Atelectasis with Lung Ultrasound Score in General Anaesthesia Maintenance Methods, Zonguldak Bülent Ecevit University, Faculty of Medicine, Department of Anaesthesiology and Reanimation, Medical Specialisation Thesis in Medicine, Zonguldak, 2025. Aim: This study aimed to evaluate the effects of different general anesthesia maintenance methods on perioperative atelectasis using the lung ultrasound score (LUSS). Materials and Methods: Ninety-seven patients aged 18–65 years, classified as ASA I–II, and scheduled for septoplasty or rhinoplasty under general anesthesia were included in the study. Following routine hemodynamic monitoring and anesthesia induction, patients were randomized into three groups based on maintenance method: sevoflurane (Group S, n=30), desflurane (Group D, n=30), and propofol-based total intravenous anesthesia (Group P, n=30). Demographic and hemodynamic data, analgesic requirement assessed by numeric rating scale (NRS), smoking status and quantity, time since last cigarette, and LUSS values at the following time points were recorded: preoperative (T0), 10 minutes after intubation (T1), at the end of surgery (T2), and 20 minutes post-extubation (T3). Results: Compared to baseline, total LUSS values increased significantly in all groups at T2 (p<0.001) and T3 (p<0.001). No significant differences in total LUSS or ΔLUSS values were observed between the groups at T3 (p>0.05). Total LUSS values in Group D were significantly higher than in Group S at T1 (p=0.002) and T2 (p=0.007), and higher than in Group P at T2 (p=0.008). Smokers had significantly higher total LUSS values than non-smokers at T0 (p<0.001) and T1 (p=0.002). In Group S and Group D, smokers had higher LUSS values than non-smokers at T0 (p=0.027 and p=0.004, respectively), with the difference at T1 being significant only in Group D (p=0.001). Among smokers, Group D had higher total LUSS values than Group S at T1 (p=0.009). Among non-smokers, Group D had higher total LUSS values than Group S (p=0.005) and Group P (p=0.010) at T2, and higher than Group S at T3 (p=0.012). The longest extubation time was recorded in Group P (p=0.002), while the shortest recovery time was observed in Group D (p=0.001). NRS scores at T3 were significantly higher in Group D compared to Group P (p=0.015). Conclusion: In this study, all three general anesthesia maintenance methods caused a similar increase in LUSS values during the perioperative period. Although desflurane was associated with a more prominent intraoperative LUSS increase, no significant difference was observed between the methods regarding temporal LUSS changes. This suggests that different maintenance agents may have similar effects on the development of perioperative atelectasis. However, subgroup analysis based on smoking status revealed that LUSS values were higher in smokers receiving desflurane at 10 minutes post-intubation and in non-smokers at the end of surgery compared to those receiving sevoflurane. Therefore, desflurane should be used cautiously in terms of intraoperative atelectasis development, especially in patients with a smoking history.
