Özet

Juvenil spondiloartropati (JSpA), çocukluk döneminde başlayan ve öncelikle kas-iskelet sistemi olmak üzere çeşitli organları etkileyen kronik inflamatuar bir hastalık grubudur. Hastalık seyrinde görülen ağrı, kas güçsüzlüğü, yorgunluk ve fonksiyonel kısıtlılık, çocukların günlük yaşam aktivitelerine katılımını olumsuz yönde etkileyebilmektedir. Bu çalışma, prospektif, randomize kontrollü olup, JSpA tanılı çocuklarda core stabilizasyon egzersizlerinin ağrı, yorgunluk, fonksiyonel durum ve yaşam kalitesi üzerindeki etkilerini değerlendirmek amacıyla planlanmıştır. Çalışmaya JSpA tanısı almış 10-18 yaş aralığında olan 30 çocuk dahil edildi. Katılımcılar randomize olarak iki ayrı gruba, Grup 1 ve Grup 2 olarak, ayrılmıştır. Grup 1'e core stabilizasyon egzersizleri uygulanırken, Grup 2'ye yalnızca fiziksel aktivite danışmanlığı verildi. 12 haftalık tedavi süreci öncesinde ve sonrasında: ağrı düzeyi Vizüel Analog Skalası (VAS) ile, kas kuvveti ise manuel kas testi yöntemiyle değerlendirildi. Yorgunluk düzeyi Çok Boyutlu Yorgunluk Ölçeği (MFS) ile, alt ekstremite fonksiyonel durum 6 dakika yürüme mesafesi testi (6DYMT) ile ve Zamanlı Merdiven İnip Çıkma (TUDS) ile, yaşam kalitesi ise Pediyatrik Yaşam Kalitesi Envanteri (PedsQL) 3.0 Artrit Modülü ile değerlendirildi. Çalışma sonunda her iki grupta da çeşitli parametrelerde olumlu gelişmeler kaydedilmiş olmakla birlikte, grup 1'de bu değişimlerin istatistiksel olarak daha anlamlı olduğu görülmüştür. (p

Juvenile spondyloarthropathy (JSpA) is a group of chronic, inflammatory diseases that emerge during childhood and affect multiple systems, primarily the musculoskeletal system. Pain, muscle weakness, fatigue, and functional limitations observed during the course of the disease can negatively affect children's participation in daily life activities. This study was designed as a prospective, randomized controlled trial to evaluate the effects of core stabilization exercises on pain, fatigue, functional status, and quality of life in children diagnosed with JSpA. A total of 30 children aged between 10 and 18 years who had been diagnosed with JSpA were included in the study. Participants were randomly divided into two groups: Group 1 and Group 2. While core stabilization exercises were applied to Group 1, only physical activity counseling was provided to Group 2. Before and after the 12-week treatment period, pain levels were assessed using the Visual Analog Scale (VAS), muscle strength was evaluated with manual muscle testing, fatigue levels were measured using the Multidimensional Fatigue Scale (MFS), lower extremity functional status was assessed with the 6-Minute Walk Test (6MWT) and the Timed Up and Down Stairs Test (TUDS), and quality of life was evaluated using the Pediatric Quality of Life Inventory (PedsQL) 3.0 Arthritis Module. At the end of the study, although both groups showed improvements in various parameters, the changes observed in the intervention group were found to be statistically more significant (p < 0.05). While improvements in back muscle strength, pain scores, and 6MWT performance were noted in both the intervention and control groups, the effects of core stabilization exercises were found to be considerably greater (p < 0.05). In conclusion, core stabilization exercises stand out as an effective method for improving symptom management and enhancing functional well-being in children with JSpA.
