Özet

ÖZET Giriş ve Amaç Fetal Büyüme Kısıtlılığı (FGR), prematüre bebeklerin akciğer gelişimini olumsuz yönde etkileyerek RDS, BPD ve diğer solunum komplikasyonları riskini artırır. Literatürde FGR, çoğunlukla SGA terimiyle ele alınmakta, antenatal ölçümlerle tanımlanmakta ve postnatal dönemde yüksek mortalite ile gelişim geriliğiyle ilişkilendirilmektedir. Mevcut solunum destek protokolleri, FGR bebeklere özel düzenlemeler sunmamaktadır. Bu çalışma ile FGR'li çok düşük doğum ağırlıklı (ÇDDA) prematüre bebeklerde, standart surfaktan replasman tedavisinin sonuçlarını FGR olmayan akranlarıyla karşılaştırarak değerlendirmek ve bu özel hasta grubunda solunum desteği yönetiminin bireyselleştirilmesi gerekip gerekmediğini belirlemek amaçlanmıştır. Gereç ve Yöntem Bu araştırma, retrospektif tasarımda bir dosya tarama çalışmasıdır. Çalışmaya, 2019 –2023 tarihleri arasında Dokuz Eylül Üniversitesi Tıp Fakültesi, Çocuk Sağlığı ve Hastalıkları Anabilim Dalı, Neonatoloji Bilim Dalı'nda izlenmiş olan, ÇDDA olan konjenital anomalisi bulunmayan 198 yenidoğan dâhil edilmiştir. Çalışmada, FGR tanısı almış olan bebekler, akranlarına göre normal büyüme gösteren ÇDDA bebekler ile karşılaştırmalı olarak değerlendirilmiştir. Olgulara ait; gestasyonel yaş, doğum ağırlığı, doppler bulguları, preeklampsi öyküsü, 1. ve 5. dakika Apgar skorları, doğum odasında resüsitasyon uygulanma durumu, doğum şekli, sürfaktan tedavisi ve antenatal- postnatal steroid tedavisi alıp almadığı, invaziv ve invaziv olmayan ventilasyon süreleri, oksijen tedavisi süresi, hava kaçağı sendromu varlığı, hastanede yatış süresi ve taburculuk anındaki vücut ağırlığı yer almaktadır. Tüm mortalite ve majör morbidite verileri (RDS, BPD, ROP, PDA, NEK, IVK, PVL, erken ve geç neonatal sepsis) ayrıca kaydedilmiştir. Bu parametrelerin, FGR varlığı ile ilişkili olabilecek klinik sonuçlara etkisi istatistiksel olarak analiz edilmiştir. Çalışmanın istatistik analizleri için SPSS (Statistical Package for Social Sciences; SPSS Inc., Chicago, IL) 27.0 paket yazılım programı kullanılmıştır. Bulgular Toplam 198 VLBW bebekten 72'si (%36,3) FGR idi. FGR'li bebeklerin ortalama gestasyonel yaşı, kontrol grubuna göre daha büyük bulunmasına ragmen (29,11 ± 2,77 vs. 27,07 ± 2,53, p<0,01), doğum ağırlıkları tanılarıyla tutarlı olarak önemli derecede düşük saptandı (912,81 ± 311,04g vs 1029,98 ± 289,95g, p=0,009). Elde edilen bulgular, FGR'li bebeklerde uteroplasental yetmezlik, preeklampsi ve maternal diyabet gibi patolojik durumların anlamlı düzeyde daha sık izlendiğini ortaya koymuş; bu değişkenlerin her birinin, FGR gelişimi açısından bağımsız ve güçlü risk faktörleri olduğunu desteklemiştir. Logistik regresyon analizine göre, FGR gelişme riski; Doppler ile saptanan uteroplasental yetmezlik bulgusu varlığında 24 kat (OR: 24,24; %95 GA: 9,74-60,35; p<0,001), annede preeklampsi varlığında 9,70 kat (OR: 9,70; %95 GA: 4,75-19,79; p<0,001) ve maternal diyabet durumunda ise yaklaşık 2,90 kat (OR: 2,90; %95 GA: 1,18-7,16; p=0,020) artmış olarak bulunmuştur. İlk 3 gün ölen ve 23 hafta ve öncesi doğan olgular çıkarıldıktan sonra kalan 178 olguda FGR olan ve olmayan gruplarda BPD hariç diğer morbiditeler ve mortalite oranları arasında istatistiksel fark saptanmadı. Ancak postkonsepsiyonel 36. GH'da BPD tanısı oranı FGR olan grupta olmayan gruba göre istatistiksel olarak anlamlı şekilde daha düşük bulundu (%41,7 vs %58,0 ve p=0,045). 36. GH'de BPD tanısı alan olguların dereceleri ise; FGR olan grupta orta-ağır BPD oranı (%64,0) iken FGR olmayan gruptaki orta-ağır BPD oranına (%31,0) göre istatistiksel olarak anlamlı bir şekilde daha yüksek bulundu (p=0,005). Doğum odası ve ilk üç gün içerisinde entübasyon ihtiyacı, mekanik ventilatör basınçları, NIV ve oksijen gereksinimi açısından iki grup benzerdi. Sürfaktan replasman tedavisi verilen 113 VLBW bebekten 40'ı (%35,4) FGR idi. Sürfaktan tedavisi verilen FGR olan ve olmayan bebekler karşılaştırıldığında; ortalama gestasyonel yaşı, kontrol grubuna göre daha büyük bulunmasına rağmen (29,2 ± 2,6 vs. 27,4 ± 2,2, p<0,05), doğum ağırlıkları anlamlı derecede daha düşük (918,9 ± 308,8g vs. 1075,0 ± 261,6g, p<0,05) saptandı. Doğum odası ve ilk üç gün içerisinde entübasyon ihtiyacı, oksijen gereksinimi ve mekanik ventilatör basınçları açısından iki grup arasında fark saptanmadı. Ancak, toplam mekanik ventilasyon süresi benzerken, 36. haftada orta-ağır BPD gelişme oranı FGR'li bebeklerde belirgin şekilde daha yüksek seyretti (%65,2 vs. %33,3, p=0,012). Diğer neonatal morbidite ve mortalite oranları ise iki grupta benzer bulundu. Sonuç ÇDDA olan bebekler arasında FGR'li olanlar, surfaktan replasman tedavisine FGR olmayan akranlarıyla benzer yanıt verse de, uzun dönem solunum morbiditesi açısından daha yüksek risk taşımaktadır. Ortalama gestasyon haftasının daha büyük olmasına rağmen, FGR bebeklerde gözlemlenen bu durum, solunum yönetimi ve BPD önleyici stratejilerin FGR'li bebeklerde daha titizlikle uygulanması gerekliliğini ortaya koymaktadır. Bu çalışma, FGR'li preterm bebeklerde optimal ventilasyon stratejilerinin belirlenmesi için ileriye dönük kapsamlı araştırmalara ihtiyaç duyulduğunu vurgulamaktadır. Bulgular, FGR'li bebeklerde doğum sonrası ilk dönemde uygulanan solunum desteği parametrelerinin FGR olmayan bebeklerle benzer klinik sonuçlar oluşturmasına rağmen, ileri dönemde ciddi akciğer morbiditelerine yol açtığını açıkça ortaya koymaktadır. Bu durum, FGR'li bebeklerin antenatal ve postnatal dönemde daha dikkatli izlenmesi, solunum desteği uygulamalarında erken müdahale stratejilerinin geliştirilmesi ve kişiye özel tedavi protokollerinin oluşturulması gerektiğini işaret etmektedir

ABSTRACT Introduction and aims Fetal Growth Restriction (FGR) adversely affects the lung development of premature infants and increases the risk of RDS, BPD and other respiratory complications. In the literature, FGR is often referred to by the term SGA, defined by antenatal measurements and associated with high mortality and developmental delay in the postnatal period. Current respiratory support protocols do not offer special arrangements for FGR infants. The aim of this study was to evaluate the outcomes of standard surfactant replacement therapy in very low birth weight (VLBW) premature infants with FGR compared with their non-FGR peers and to determine whether respiratory support management should be individualised in this special patient group. Materials and methods This study is a file review study with retrospective design. The study included 198 newborns without congenital anomalies with FGR who were followed up in Dokuz Eylül University Faculty of Medicine, Department of Paediatrics, Division of Neonatology between 2019 and 2023. In the study, infants with a diagnosis of FGR were evaluated in comparison with FGR infants with normal growth compared to their peers. Gestational age, birth weight, Doppler findings, history of pre-eclampsia, 1st and 5th minute Apgar scores, resuscitation status in the delivery room, mode of delivery, surfactant therapy and antenatal and postnatal steroid therapy, duration of invasive and non-invasive ventilation, duration of oxygen therapy, presence of air leak syndrome, duration of hospitalisation and body weight at discharge were included. All mortality and major morbidity data (RDS, BPD, ROP, PDA, NEC, IVK, PVL, early and late neonatal sepsis) were also recorded. The effect of these parameters on clinical outcomes that may be associated with the presence of FGR was statistically analysed. SPSS (Statistical Package for Social Sciences; SPSS Inc., Chicago, IL) 27.0 software package was used for statistical analyses. Findings Of a total of 198 VLBW infants, 72 (36.3%) were FGR. Although the mean gestational age of babies with FGR was found to be older than the control group (29.11 ± 2.77 vs. 27.07 ± 2.53, p<0.01), their birth weights were significantly lower consistent with their diagnosis (912.81 ± 311.04g vs. 1029.98 ± 289.95g, p=0.009). The findings revealed that pathological conditions such as uteroplacental insufficiency, pre-eclampsia and maternal diabetes mellitus were significantly more frequent in infants with FGR and supported that each of these variables were independent and strong risk factors for the development of FGR. According to logistic regression analysis, the risk of developing FGR was 24 times higher in the presence of uteroplacental insufficiency detected by Doppler (OR: 24.24; 95% CI: 9.74-60.35; p<0.001), 9.70 times higher in the presence of maternal pre-eclampsia (OR: 9.70; 95% CI: 4.75-19.79; p<0.001) and approximately 2.90-fold in the presence of maternal diabetes (OR: 2.90; 95% CI: 1.18-7.16; p=0.020). After excluding the patients who died in the first 3 days and those born at 23 weeks and before, no statistical difference was found between the morbidities and mortality rates except BPD in the groups with and without FGR in the remaining 178 patients. However, the rate of BPD diagnosis at 36th postconceptional GH was statistically significantly lower in the group with FGR compared to the group without FGR (41.7% vs 58.0% and p=0.045). At 36th GA, the rate of BPD was found to be statistically significantly higher in the group with FGR (64.0%) compared to the rate of moderate-severe BPD in the group without FGR (31.0%) (p=0.005). The need for intubation in the delivery room and in the first three days, mechanical ventilator pressures, NIV and oxygen requirements were similar in the two groups. Of 113 VLBW infants who received surfactant replacement therapy, 40 (35.4%) had FGR. When the babies with and without FGR who received surfactant therapy were compared, although the mean gestational age was found to be older than the control group (29.2 ± 2.6 vs. 27.4 ± 2.2, p<0.05), the birth weights were significantly lower (918.9 ± 308.8g vs. 1075.0 ± 261.6g, p<0.05). There was no difference between the two groups in terms of intubation requirement, oxygen requirement and mechanical ventilator pressures in the delivery room and in the first three days. However, while the total duration of mechanical ventilation was similar, the rate of moderate to severe BPD at 36 weeks was significantly higher in infants with FGR (65.2% vs. 33.3%, p=0.012). Other neonatal morbidity and mortality rates were similar in the two groups. Results Among infants with VLBW, those with FGR have a similar response to surfactant replacement therapy as their non-FGR peers, but are at higher risk for long-term respiratory morbidity. This was observed in FGR infants despite a larger mean gestational age, suggesting that respiratory management and BPD prevention strategies should be applied more rigorously in FGR infants. This study emphasises the need for comprehensive prospective studies to determine optimal ventilation strategies in preterm infants with FGR. The findings clearly demonstrate that respiratory support parameters applied in the first postnatal period in infants with FGR lead to severe pulmonary morbidities in the advanced period, although they produce similar clinical results to infants without FGR. This situation indicates that babies with FGR should be monitored more carefully in the antenatal and postnatal period, early intervention strategies should be developed in respiratory support applications and personalised treatment protocols should be established.
