Özet
Amaç: Pankreas bezinin ekzokrin drenajını sağlayan ductus pancreaticus ve ductus pancreaticus accessorius morfolojisi ve morfometrisinin ortaya konulması amaçlanmıştır. Gereç ve Yöntem: 30 adet duodenum-pankreas piyesi üzerinde pigmentle renklendirilmiş poliüretan epoksi ile doldurulmasının ardından diseksiyon çalışmaları yapılmıştır. Processus uncinatus ekzokrin drenajı tariflenerek sınıflandırılmıştır. İstatistiksel yöntemler ile verilerin analizi yapılmıştır. Bulgular: Papilla duodeni major ve minor arası mesafe 21,6±7,17 mm, duodenum duvarı içerisinde ilerleyen ductus pancreaticus uzunluğu 10,2±3,81 mm olarak tespit edilmiştir. Papilla duodeni minor yerleşimi, papilla duodeni major'un konumuna göre tarif edildiğinde 20 piyeste superior-anterior ve 7 piyeste ise inferior-posterior olarak iki sınıfta toplanmıştır. Processus uncinatus ekzokrin drenajı tariflenmiş ve ilk defa çalışmamızda kullanılan 5 tipten oluşan bir sınıflandırma tariflenmiştir. Tip A; ön bölge ductus pancreaticus accessorius, arka bölge ductus pancreaticus tarafından drene edilen, tip B; tamamı ductus pancreaticus accessorius tarafından drene edilen, tip C; tamamı ductus pancreaticus tarafından drene edilen, tip D; büyük çoğunluğu ductus pancreaticus accessorius tarafından drene edilen, tip E; büyük çoğunluğu ductus pancreaticus tarafından drene edilen olarak tariflenmiştir. Bu sınıflamaya göre diseksiyonlarımızda 11 adet (%36,6) pankreasta tip A, 6 adet (%20) pankreasta tip B, 8 adet (%26,6) pankreasta tip C, 2 adet (%6,66) pankreasta tip D, 3 adet (%10) pankreasta tip E şeklinde processus uncinatus drenajı izlenmiştir. Sonuç: Bu çalışmada processus uncinatus drenajı ortaya konulmuş ve beş tipten oluşan bir sınıflama önerilmiştir. Ductus pancreaticus, ductus pancreaticus accessorius, papilla duodeni major ve papilla duodeni minor çapları literatür ile uyumlu bulunmuş olup etnik köken ve cinsiyetin bu ölçümler üzerinde etkisi olmadığı gösterilmiştir. Aynı şekilde papilla duodeni major ve papilla duodeni minor arası mesafe de literatür ile uyumlu bulunmuştur. Ductus pancreaticus'un duodenum duvarı içerisinde ilerleyişinin uzunluğunun minimum 4,58 mm, maksimum ise 19,2 mm olduğu görülmüş olup bu ilerleyişin mesane duvarında ilerleyen üreter benzeri bir fizyolojik mekanizmasının olup olmadığının ve bu durumun klinik korelasyonunun araştırılmasının klinikte pratikte önemi olabileceğini düşünmekteyiz. Embriyolojik caput pancreatis ön bölümü ve ductus pancreaticus accessorius dorsal pankreatik tomurcuktan gelişirken, caput pancreatis arka bölümü ve ductus pancreatis ise ventral pankreatik tomurcuktan gelişmektedir. Inferior-posterior yerleşimli görülen papilla duodeni minor ve ductus pancreaticus bu embriyolojik gelişim paternine uymamaktadır.

Objective: The aim was to determine the morphology and morphometry of the pancreatic duct and accessory pancreatic duct, which provide exocrine drainage of the pancreas. Materials and Methods: Dissection studies were performed on 30 duodenum-pancreas specimens after filling them with pigmented polyurethane epoxy. The exocrine drainage of the uncinate process was described and classified. Data were analyzed using statistical methods. Results: The distance between the major and minor duodenal papillae was 21.6±7.17 mm, and the length of the pancreatic duct progressing within the duodenal wall was 10.2±3.81 mm. The location of the minor duodenal papilla, when described relative to the position of the major duodenal papilla, was grouped into two classes: superior-anterior in 20 specimens and inferior-posterior in 7 specimens. The exocrine drainage of the uncinate process was described, and a classification consisting of 5 types, used for the first time in our study, was defined. Type A was defined as being drained by the accessory pancreatic duct in the anterior region and the pancreatic duct in the posterior region; type B as being drained entirely by the accessory pancreatic duct; type C as being drained entirely by the pancreatic duct; type D as being drained predominantly by the accessory pancreatic duct; and type E as being drained predominantly by the pancreatic duct. According to this classification, in our dissections, 11 (36.6%) type A, 6 cases (20%) type B, 8 cases (26.6%) type C, 2 cases (6.66%) type D, and 3 cases (10%) type E uncinate process drainage in the pancreas. Conclusion: This study demonstrated uncinate process drainage and proposed a classification consisting of five types. The diameters of the pancreatic duct, accessory pancreatic duct, major duodenal papilla, and minor duodenal papilla were found to be consistent with the literature, and it was shown that ethnic origin and gender had no effect on these measurements. Similarly, the distance between the major duodenal papilla and the minor duodenal papilla was also found to be consistent with the literature. The length of the pancreatic duct's progression within the duodenal wall was found to be a minimum of 4.58 mm and a maximum of 19.2 mm. We believe that investigating whether this progression has a physiological mechanism similar to the ureter progressing within the bladder wall and its clinical correlation could be important in clinical practice. The anterior part of the embryological head of pancreas and the accessory pancreatic duct develop from the dorsal pancreatic bud, while the posterior part of the head of pancreas and the pancreatic duct develop from the ventral pancreatic bud. The inferior-posteriorly located minor duodenal papilla and pancreatic duct do not conform to this embryological development pattern.
