Özet

Amaç: Majör hepatektomi, vücutta inflamatuvar sistemlerin aşırı uyarılmasına yol açarak posthepatik karaciğer yetmezliği ve çeşitli komplikasyonların görülme oranlarını artırmaktadır. Antiinflamatuvar özellikleri bilinen steroidlerin bu yanıtı baskılayarak komplikasyonları azaltabileceği düşüncesiyle bu konuda çalışmalar başlatılmıştır. Bu çalışmada, majör hepatektomi öncesinde uygulanan preoperatif tek doz metilprednizolonun postoperatif komplikasyonlar, laboratuvar parametreleri ve mortalite üzerindeki etkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Ankara Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda yürütülen bu randomize kontrollü, çift kör ve prospektif klinik çalışmada, majör hepatektomi uygulanan 41 hasta ve canlı donör olgusu incelenmiştir. Katılımcılar, preoperatif tek doz 250 mg metilprednizolon uygulananlar (St⁺) ve uygulanmayanlar (St⁻) olmak üzere iki ana gruba; ayrıca hasta ve donör alt gruplarına ayrılmıştır. Operasyon süresi, kanama miktarı, dren sayısı, dren çekilme zamanı, komplikasyon türleri, laboratuvar parametreleri (ALT, AST, total/direkt bilirubin, INR, Hb, lökosit, CRP), yoğun bakım ve hastanede kalış süreleri karşılaştırılmıştır. Veriler SPSS 22.0 programı ile analiz edilmiş; Mann-Whitney U, Kruskal-Wallis ve Ki-Kare testleri kullanılmıştır. İstatistiksel anlamlılık düzeyi p<0,05 olarak kabul edilmiştir. Bulgular: Tüm olgular değerlendirildiğinde, 90 günlük toplam komplikasyon oranı St⁺ grupta %31,6, St⁻ grupta ise %40,9 olarak saptanmıştır(p=0,746). Hasta alt grubunda komplikasyon oranı St⁺ hastalarda %44,4, St⁻ hastalarda %57,1(p=0,680); donör alt grubunda ise sırasıyla %20 ve %12,5 olarak belirlenmiştir(p=0,671). Mortalitenin yalnızca St⁻ hasta grubunda üç olguda (%13,6) izlendiği, St⁺ grupta ise mortalite saptanmadığı görülmüştür(p=0,235). Postoperatif CRP ve lökosit düzeyleri genel olarak benzer seyretmekle birlikte, St⁻ grubunda beşinci günde CRP düzeylerinde artış eğilimi gözlenmiştir (p=0,059). ALT, AST, bilirubin ve INR düzeyleri arasında anlamlı fark izlenmemiştir. Sonuç: Preoperatif tek doz metilprednizolon uygulaması, majör hepatektomi sonrası erken dönemde inflamatuvar yanıtı azaltma ve karaciğer enzim düzeylerinin daha normale yakın seyretmesine katkı sağlama eğilimi göstermiştir. Bununla birlikte, komplikasyon sıklığını anlamlı biçimde değiştirmemiş; mortalite yalnızca steroid uygulanmayan grupta gözlenmiştir. Bu bulgular, uygun hasta seçimiyle birlikte preoperatif kortikosteroid uygulamasının güvenli ve potansiyel olarak koruyucu bir seçenek olabileceğini düşündürmektedir. Ancak bu etkinin doğrulanabilmesi için daha geniş örneklemli, randomize kontrollü çalışmalara ihtiyaç vardır.

Objective: Major hepatectomy leads to excessive activation of systemic inflammatory pathways, thereby increasing the incidence of posthepatectomy liver failure and various postoperative complications. Given the known anti-inflammatory properties of corticosteroids, it has been hypothesized that these agents may attenuate the inflammatory response and reduce complication rates. This study aimed to evaluate the effects of a single preoperative dose of methylprednisolone on postoperative complications, laboratory parameters, and mortality in patients undergoing major hepatectomy. Materials and Methods: This randomized, controlled, double-blind, and prospective clinical study was conducted at the Department of General Surgery, Ankara University Faculty of Medicine. A total of 41 patients and living donors who underwent major hepatectomy were included. Participants were divided into two main groups: those receiving a single preoperative dose of 250 mg methylprednisolone (St⁺) and those not receiving steroids (St⁻); additionally, patient and donor subgroups were analyzed separately. Operative time, intraoperative blood loss, number and removal time of drains, complication types, laboratory parameters (ALT, AST, total/direct bilirubin, INR, hemoglobin, leukocyte count, CRP), and lengths of stay in the intensive care unit and hospital were compared. Data were analyzed using SPSS 22.0 software, and the Mann-Whitney U, Kruskal-Wallis, and Chi-square tests were applied. A p-value <0.05 was considered statistically significant. Results: Evaluation of all cases revealed that the 90-day overall complication rate was 31.6% in the St⁺ group and 40.9% in the St⁻ group (p=0.746). In the patient subgroup, the complication rate was 44.4% in St⁺ patients and 57.1% in St⁻ patients (p=0.680), while in the donor subgroup, the rates were 20% and 12.5%, respectively (p=0.671). Mortality occurred exclusively in the St⁻ patient group, with three deaths (13.6%), whereas no mortality was observed in the St⁺ group (p=0.235). While postoperative CRP and leukocyte levels showed generally similar trends between groups, an upward tendency in CRP levels on postoperative day 5 was noted in the St⁻ group (p=0.059). No significant differences were detected in ALT, AST, bilirubin, or INR levels. Conclusion: A single preoperative dose of methylprednisolone demonstrated a tendency to attenuate the early postoperative inflammatory response and to maintain liver enzyme levels closer to normal following major hepatectomy. However, it did not significantly alter the overall complication rate, and mortality occurred only in patients who did not receive steroids. These findings suggest that preoperative corticosteroid administration may represent a safe and potentially protective option when applied to appropriately selected patients. Nevertheless, larger randomized controlled trials are required to confirm these effects.
