Genetik tanı öncesi ve sonrası nöbet sıklıkları karşılaştığında, genetik tanının nöbet kontrolü üzerinde istatiksel olarak anlamlı ve olumlu bir etkisi olduğu görüldü (0.001>p). Sonuç: Gelişimsel ve epileptik ensefalopatili çocuklarda genetik tanı, yalnızca etşiyolojik değerlendirme açısından değil aynı zamanda tedavi stratejilerinin belirlenmesi ve nöbet kontrolü sağlanması açısından dabelirleyici bir role sahiptir. Genetik tanı sonrasında tedavinin daha rasyonel ve hedefe yönelik hale gelmesi, nöbet sıklığında anlamlı azalma ile ilişkilidir. Bu sebeple, GEE tanılı hastalarda erken dönemde genetik tanıya ulaşılması ve bireyselleştirilmiş tedavi yaklaşımlarının oluşturulması klinik yönetimde öncelikli hedefler arasında yer almalıdır.

Aim: Developmental and epileptic encephalopathies (DEE) are severe epilepsy syndromes characterized by early -onaet epileptic seizures accompanied by developmental impairment and are most commonly associated with genetic etiologies. Advances in genetic diagnosis on treatment strategies. The aim of this study was to evaluate the changes in treatment approaches before and after genetic diagnosis and to asses the effect of these changes on seizure control in children with genetically diagnosed DEE. Materials and Methods: In this retrospective study, patients diagnosed with developmental and epileptic encephalopathy and followed at the Pediatric Neurology Clinic of Kahramanmaraş Sütçü İmam University Faculty of Medicine were evaluated. A total of 46 patients were included in the study. Demographic characteristics, age at seizure onset, age at genetic diagnosis, results of genetic analyses, antiepileptic drugs used before and after genetic diagnosis, and seizure frequencies were recorded. Seizure frequencies before and after genetic diagnosis were compared to assess the impact of genetic diagnosis on treatmen were outcomes. Results: The mean age of the patients was 10.85 ± 5.86 years. The median age at seizure onset was 7 months (range: 0–168 months), and the median age at genetic diagnosis was 36 months (range: 0–216 months). Genetic analyses revealed that the most frequently identified pathogenic variant was in the SCN1A gene in 16 patients (34.8%), followed by DEPDC5 mutations in 5 patients (10.9%) and AP3B2 mutations in 4 patients (8.7%). Prior to genetic diagnosis, most patients were treated with empirical antiepileptic drug regimens. Following genetic diagnosis, significant modifications in treatment strategies were observed. After genetic diagnosis, complete seizure control was achieved in 11 patients (23.9%), while a reduction in seizure frequency was observed in 24 patients (52.2%). Seizure frequency remained unchanged in 8 patients (17.4%) and increased in 3 patients (6.5%). Comparison of seizure frequencies before and after genetic diagnosis demonstrated a statistically significant improvement in seizure control following genetic diagnosis (p < 0.001). Conclusion: In children with developmental and epileptic encephalopathies, genetic diagnosis plays a critical role not only in determining the underlying etiology but also in guiding treatment strategies and improving seizure control. The transition to more rational and targeted treatment approaches after genetic diagnosis is associated with a significant reduction in seizure frequency. Therefore, early genetic diagnosis and the implementation of individualized treatment strategies should be considered a priority in the clinical management of patients with DEE.
