Özet
Fotoakustik tomografi (FAT), optik görüntülemenin yüksek kontrast avantajını ultrasonik algılamanın derin doku penetrasyonu ile birleştiren hibrit bir biyomedikal görüntüleme yöntemidir. Bu yöntem, optik görüntüleme yöntemlerinde karşılaşılan ışığın saçılması, görüntüleme derinliğinin ve çözünürlüğünün azalması gibi dezavantajları ortadan kaldırmayı sağlamaktadır. Güçlü manyetik alanın, iyonlaştırıcı radyoizotopların kullanıldığı görüntüleme yöntemleri ile kıyaslandığında FAT daha az maliyetlidir ve radyasyon maruziyetini ortadan kaldırır. Ultrasonik görüntüleme ile karşılaştırıldığında ise FAT daha yüksek optik kontrasta sahiptir. Tüm bu avantajları sayesinde FAT, son yıllarda klinik ve deneysel çalışmalarda giderek artan bir ilgi görmektedir. FAT'da görüntülenecek doku lazer ışınları ile aydınlatılır. Dokuda bulunan optik emici kaynaklar ışığı emer ve ısı artışı oluşturur. Oluşan ısı artışının termoelastik genleşme ve basınç üretmesi sonucu fotoakustik (FA) dalga oluşur. Doku dışında konumlandırılmış ultrason dönüştürücüler tarafından FA sinyaller algılanarak FAT görüntü ve sinyaller elde edilir. FAT kaynakları arasında hemoglobin 1000 nm'nin altındaki dalga boylarında en önemli emicilerden biridir. FAT görüntülerinde kanla beslenen damarların yüksek kontrastlı görüntülenmesine ve SpO2'nin tespit edilmesine olanak tanır. FAT'ın en önemli uygulama alanlarından biri, kandaki oksijen satürasyonunun (SpO₂) belirlenmesidir. SpO₂, oksihemoglobin (HbO₂) ve deoksihemoglobin (HbR) hemoglobin formlarının oranı ile tanımlanmaktadır. Oksijen metabolizmasındaki işlev bozukluklarının, kanser, diyabet, inme ve nörodejenerasyon olmak üzere çok sayıda yaşamı tehdit eden hastalığın ortaya çıkmasında ve ilerlemesinde belirleyici bir etken olduğu kabul edilmektedir. Bu sebeple, SpO₂ tespiti ve takibi hayati önem arz etmektedir. Günümüzde SpO₂ tespiti için yaygın olarak kullanılan fotoakustik yöntemler, HbO₂ ve HbR'nin farklı dalga boylarındaki optik emilim özelliklerinden yararlanmaktadır. Bu yöntemler iki veya daha fazla dalga boyuna sahip lazer kaynaklarının kullanımını gerektirmektedir. Bu durum sistem karmaşıklığını ve maliyeti artırmakta, ayrıca dalga boyuna bağlı optik zayıflama nedeniyle görüntüleme derinliğini sınırlandırabilmektedir. Bu tez çalışmasının temel amacı, tek dalga boyuna sahip lazer ışık kaynağı kullanılarak FAT ile SpO₂ tespitine olanak sağlayan deneysel ve analitik bir yöntemin geliştirilmesidir. Çalışma kapsamında, insan veya hayvan kanı kullanımının etik kısıtlamalara neden olacağı içim, HbO₂ ve HbR hemoglobin formlarını taklit etmek amacıyla kırmızı ve mavi renk mürekkepler kullanılmıştır. Farklı oksijen satürasyon seviyelerini temsil etmesi amacıyla mürekkepler çeşitli oranlarda karıştırılarak kan modelleri oluşturulmuştur. İlk aşamada kırmızı ve mavi mürekkepler distile su ile seyreltilerek farklı konsantrasyon seviyeleri hazırlanmış, ikinci aşamada ise kırmızı ve mavi mürekkepler belirli oranlarda karıştırılarak %0-%100 aralığında değişen SpO₂ seviyelerini temsil eden beş farklı kan modeli hazırlanmıştır. Hazırlanan bu karışımlar, damar yapısını modellemek amacıyla şeffaf PVC tüplere enjekte edilmiştir. Deneysel ölçümler, 532 nm dalga boyuna sahip Nd:YAG lazer kaynağı ile uyarılan kan modeli ile gerçekleştirilmiştir. Fotoakustik sinyaller, 18 MHz merkez frekanslı doğrusal ultrason dönüştürücü (Veraconics Vantage) dizisine sahip bir FAT sistemi kullanılarak algılanmıştır. Ölçümlerin tekrarlanabilirliğini ve güvenilirliğini artırmak amacıyla her bir kan modeli için 20 defa ölçümler gerçekleştirilmiştir. Elde edilen fotoakustik sinyaller, zaman alanında genlik analizi ve frekans alanında spektral analiz yöntemleri kullanılarak değerlendirilmiştir. Genlik analizi kapsamında, farklı mürekkep konsantrasyonlarının fotoakustik sinyal genliği üzerindeki etkisi incelenmiş ve optik emilim artışı ile sinyal genliği arasında doğrusal bir ilişki olduğu gösterilmiştir. Spektral analiz aşamasında ise Welch yöntemi kullanılarak güç spektral yoğunlukları hesaplanmış; spektrumlar üzerinden y-ekseni kesişimi, orta bant uyumu, eğim (-6 dB), lob genişliği ve yarı güç bant genişliği (FWHM) gibi ayırt edici spektral parametreler elde edilmiştir. Bu parametrelerin, farklı SpO₂ seviyelerini temsil eden mürekkep karışımları ile anlamlı bir ilişki gösterdiği deneysel olarak gösterilmiştir. Analiz sonuçları, tek dalga boyu lazer kullanılarak elde edilen fotoakustik sinyallerin hem genlik hem de spektral özelliklerinin oksijen satürasyonu değişimlerine duyarlı olduğunu göstermiştir. Özellikle spektral analizden elde edilen parametrelerin, yalnızca mekanik yapı özellikleriyle değil, aynı zamanda optik emilim farklarıyla da ilişkili olduğu belirlenmiştir. Elde edilen sonuçlar, spektral analiz tabanlı fotoakustik analizlerin, SpO₂ tespitinde kullanılabileceğini göstermektedir. Sonuç olarak bu tez çalışmasında, tek dalga boyuna sahip bir lazer kaynağı kullanılarak FAT ile SpO₂ tespiti yapılabileceğini gösteren, deneysel olarak doğrulanmış bir yöntem sunulmuştur. Geliştirilen yaklaşım, sistem maliyetini ve karmaşıklığını azaltması, girişimsel olmayan bir yöntem ve geniş bir görüntüleme alanında oksijen satürasyonu bilgisi sağlayabilme potansiyeli açısından literatürdeki mevcut yöntemlere bir alternatif sunmaktadır. Elde edilen bulguların, fotoakustik tabanlı oksijenasyon takibine yönelik gelecekte yapılacak klinik ve deneysel çalışmalara katkı sağlaması beklenmektedir.

Photoacoustic tomography (PAT) is a hybrid biomedical imaging modality that combines the high optical contrast of optical imaging with the superior penetration depth and spatial resolution of ultrasonic detection. By converting absorbed optical energy into acoustic signals, PAT overcomes the fundamental limitations of conventional optical imaging techniques, such as strong light scattering in biological tissues and the consequent reduction in imaging depth and resolution. Compared to imaging modalities that rely on strong magnetic fields or ionizing radiation, such as magnetic resonance imaging (MRI) and positron emission tomography (PET), PAT provides a lower cost, nonionizing, and safer alternative. In comparison with conventional ultrasound imaging, PAT offers significantly higher contrast due to its sensitivity to optical absorption properties of tissue constituents. Owing to these advantages, PAT has attracted increasing interest in both experimental and clinical research fields in recent years. In PAT, the tissue of interest is illuminated with short-pulsed laser light. Optical absorbers within the tissue absorb the incident photons and convert the absorbed energy into heat, resulting in a rapid and localized temperature rise. This temperature increase induces thermoelastic expansion, which generates transient pressure waves known as photoacoustic (PA) waves. These PA waves propagate through tissue with minimal scattering and are detected by ultrasound transducers positioned outside the imaging region. The detected signals are then reconstructed to obtain PAT images and analyzed to extract functional and structural information. Among endogenous optical absorbers, hemoglobin is one of the most dominant chromophores at wavelengths below 1000 nm. Consequently, PAT enables high contrast visualization of blood vessels and provides a powerful tool for assessing blood oxygenation levels. One of the most important applications of PAT is the determination of blood oxygen saturation (SpO₂). SpO₂ is defined as the ratio of oxyhemoglobin (HbO₂) to the total hemoglobin concentration, which consists of oxyhemoglobin (HbO₂) and deoxyhemoglobin (HbR). Oxygen saturation is a vital physiological parameter that reflects the balance between oxygen supply and metabolic demand in tissues. Impairments in oxygen metabolism are closely associated with the onset and progression of numerous life-threatening diseases, including cancer, diabetes, stroke, and neurodegenerative disorders. Therefore, accurate detection and continuous monitoring of SpO₂ play a critical role in clinical diagnosis, disease monitoring, and treatment planning. Current photoacoustic approaches for SpO₂ estimation primarily rely on the wavelength-dependent optical absorption differences between HbO₂ and HbR. These methods typically require the use of two or more laser wavelengths to spectrally separate the contributions of different hemoglobin forms. While multiwavelength techniques provide reliable oxygenation measurements, they inevitably increase system complexity, cost, and hardware requirements. In addition, wavelength-dependent optical attenuation in biological tissues may limit imaging depth and reduce measurement accuracy. These challenges motivate the development of alternative approaches that can enable SpO₂ estimation using simplified system configurations. The primary objective of this thesis is to develop an experimental and analytical method that enables SpO₂ estimation using photoacoustic tomography with a single-wavelength laser source. By eliminating the need for multiple excitation wavelengths, the proposed approach aims to reduce system complexity and cost while maintaining sensitivity to oxygenation-related changes in photoacoustic signals. In this study, the use of human or animal blood was avoided due to ethical considerations. Instead, red and blue inks were employed to mimic the optical absorption characteristics of HbO₂ and HbR, respectively. To represent different oxygen saturation levels, ink based blood mimicking models were created by mixing the inks in various proportions. In the first stage, red and blue inks were diluted with distilled water to obtain different concentration levels, allowing the investigation of the relationship between optical absorption and photoacoustic signal amplitude. In the second stage, red and blue inks were mixed at predefined ratios to generate five distinct blood models representing SpO₂ levels ranging from 0% to 100%. These mixtures were injected into transparent polyvinyl chloride (PVC) tubes to simulate vascular structures. Experimental measurements were performed using a photoacoustic tomography system excited by a 532 nm Nd:YAG laser source. Photoacoustic signals were detected using a linear ultrasound transducer array (Verasonics Vantage) with a center frequency of 18 MHz. To ensure measurement reliability and repeatability, each blood mimicking model was measured 20 times under identical experimental conditions. The acquired photoacoustic signals were analyzed in both the time and frequency domains. In the time-domain analysis, photoacoustic signal amplitudes were evaluated to investigate the effect of ink concentration on signal strength. The results demonstrated a clear and approximately linear relationship between optical absorption and photoacoustic signal amplitude, confirming the suitability of the ink-based models for oxygenation-related analysis. In the frequency-domain analysis, power spectral densities (PSDs) of the photoacoustic signals were computed using the Welch method. Several discriminative spectral parameters were extracted from the PSDs, including the y-axis intercept, midband fit value, spectral slope at -6 dB, lobe width, and full width at half maximum (FWHM). These parameters were analyzed to determine their sensitivity to variations in ink composition corresponding to different SpO₂ levels. The experimental results indicate that both amplitude-based and spectral features of photoacoustic signals obtained using a single wavelength laser are sensitive to changes in oxygen saturation. In particular, spectral analysis parameters were found to be influenced not only by the mechanical properties of the target structure but also by differences in optical absorption associated with varying ink ratios. This observation suggests that spectral analysis-based photoacoustic approaches can provide valuable information for SpO₂ estimation even in the absence of multiwavelength excitation. In conclusion, this thesis presents an experimentally validated method demonstrating that blood oxygen saturation can be estimated using photoacoustic tomography with a single-wavelength laser source. The proposed approach offers a promising alternative to conventional multiwavelength photoacoustic methods by reducing system complexity and cost while maintaining sensitivity to oxygenation-related changes. Furthermore, the noninvasive nature of the method and its ability to provide oxygen saturation information over a wide imaging field make it suitable for future biomedical applications. The findings of this study are expected to contribute to ongoing research on photoacoustic-based oxygenation monitoring and to support the development of simplified and cost effective photoacoustic imaging systems for clinical and experimental use.
