Özet
Amaç: Sepsis, enfeksiyona karşı gelişen düzensiz ve kontrolsüz konak yanıtı sonucu ortaya çıkan, yüksek mortalite ve morbidite ile seyreden heterojen bir klinik sendromdur. Özellikle immün hücre fonksiyonlarındaki bozulmalar ve trombosit aracılı immün etkileşimler, sepsisin patofizyolojisinde kritik rol oynamaktadır. Bu çalışmada, deneysel sepsis modelinde mezenkimal kök hücre kaynaklı eksozom tedavisinin lenfosit proliferasyonu, canlılık oranları ve hücresel dağılımları ile trombosit aktivasyonu ve trombosit yüzey reseptör bütünlüğü üzerindeki etkilerinin değerlendirilmesi amaçlanmıştır. Eksozom tedavisinin, sepsis sırasında bozulan immün hücre yanıtlarını ve trombosit ilişkili parametreleri modüle ederek değiştireceği hipotez edilmiştir. Gereç ve Yöntem: Toplam 27 adet BALB/c fare, sham kontrol (n=4), sham tedavi (n=4), sepsis kontrol (n=8), sepsis tedavi (n=8) ve sağlıklı kontrol (n=3) olmak üzere beş gruba ayrıldı. Deneysel sepsis, çekal ligasyon ve perforasyon (CLP) yöntemi ile oluşturuldu. Tedavi gruplarına CLP uygulamasından 1 saat sonra 200 μL serum fizyolojik içinde 100 μg eksozom intraperitoneal olarak uygulanırken, kontrol gruplarına aynı hacimde serum fizyolojik verildi. Deney 24. saatte sonlandırıldı ve sakrifiye edilen hayvanlardan kan ve doku örnekleri toplandı. Lenfosit canlılığı, proliferatif kapasite, immün fenotipleme ve trombosit aktivasyonu çok parametreli akım sitometrisi ile analiz edildi. Ayrıca organ doku örnekleri histopatolojik olarak değerlendirildi. Bulgular: Sepsis kontrol grubunda lenfosit canlılığında %60'ın üzerinde azalma saptanırken, mezenkimal kök hücre kaynaklı eksozom tedavisi lenfosit canlılık oranlarını sağlıklı kontrol düzeylerine (%80 üzeri) yükseltti (p <0.001). Sepsise bağlı proliferatif duraklama ve immün paralizi, tedavi grubunda anlamlı düzeyde azaldı (p = 0.004). Eksozom tedavisinin CD4+ yardımcı T lenfosit havuzunu tükenmeden koruduğu ve 10 hücresel sayısal devamlılığı sağladığı gösterildi (p = 0.011). Ayrıca, sepsiste gözlenen düzenleyici T hücre artışının eksozom tedavisi ile dengelenme eğilimine girdiği izlendi. Trombositlerde ise sepsise bağlı yüzey reseptör kaybının (CD42d shedding), non-enfeksiyöz tedavi grubunda sepsis grubuna kıyasla anlamlı derecede arttığı saptandı (p=0.003). Bununla birlikte, sepsis tedavi grubunda CD63 pozitifliği açısından sepsis kontrol grubuna kıyasla bir iyileşme eğilimi gözlenmiş ama istatiksel olarak anlamlı bulunmadı (p=0,32). Sonuç: Çalışmamızda deneysel çekal ligasyon perforasyon sepsis modelinde, mezenkimal kök hücre kaynaklı eksozom uygulaması lenfosit canlılığını ve proliferatif kapasiteyi koruyarak immün paraliziyi azaltıcı etki gösterdi. Eksozomun trombosit ligandları üzerinde olumlu etkilerinin varlığı ise gelecekte doz çalışmaları için önemli bir basamak oluşturmaktadır.

Objective: Sepsis is a heterogeneous clinical syndrome characterized by high mortality and morbidity, arising from a dysregulated and uncontrolled host response to infection. In particular, impairments in immune cell functions and platelet-mediated immune interactions play a critical role in the pathophysiology of sepsis. This study aimed to evaluate the effects of mesenchymal stem cell–derived exosome therapy on lymphocyte proliferation, viability rates, and cellular distributions, as well as on platelet activation and platelet surface receptor integrity, in an experimental sepsis model. It was hypothesized that exosome therapy would modulate and alter immune cell responses and platelet-related parameters disrupted during sepsis. Materials and Methods: A total of 27 BALB/c mice were divided into five groups: sham control (n=4), sham treatment (n=4), sepsis control (n=8), sepsis treatment (n=8), and healthy control (n=3). Experimental sepsis was induced using the cecal ligation and perforation (CLP) method. In the treatment groups, 100 μg of exosomes in 200 μL of saline were administered intraperitoneally 1 hour after CLP, while control groups received the same volume of saline. The experiment was terminated at 24 hours, and blood and tissue samples were collected from sacrificed animals. Lymphocyte viability, proliferative capacity, immune phenotyping, and platelet activation were analyzed by multiparametric flow cytometry. In addition, organ tissue samples were evaluated histopathologically. Results: In the sepsis control group, a decrease of more than 60% in lymphocyte viability was detected, whereas mesenchymal stem cell–derived exosome therapy increased lymphocyte viability rates to healthy control levels (over 80%) (p < 0.001). Sepsis- associated proliferative arrest and immune paralysis were significantly reduced in the treatment group (p = 0.004). Exosome therapy was shown to protect the CD4+ helper T lymphocyte pool from depletion and to maintain numerical cellular continuity (p = 0.011). In addition, the increase in regulatory T cells observed in sepsis tended to be balanced with exosome therapy. In platelets, sepsis-associated surface receptor loss (CD42d shedding) was found to be significantly increased in the non-infectious treatment group compared to the sepsis group (p = 0.003). However, although an improvement trend was observed in CD63 positivity in the sepsis treatment group compared to the sepsis control group, this did not reach statistical significance (p = 0.32). Conclusion: In our experimental cecal ligation and perforation sepsis model, mesenchymal stem cell–derived exosome administration reduced immune paralysis by preserving lymphocyte viability and proliferative capacity. The presence of favorable effects of exosomes on platelet ligands constitutes an important step for future dose-response studies. 12
