Özet
Amaç: Mide kanseri nedeniyle opere edilen hastalarda gastrektomi sonrası gelişen komplikasyonlar hastaların yaşam kalitesini ve sağkalımını önemli ölçüde etkilemektedir. Bu çalışmada, mide kanseri nedeniyle gastrektomi yapılan hastalarda, postoperative komplikasyonlarla ilişkili risk faktörlerini Clavien-Dindo sınıflandırmasına dayalı olarak değerlendirmeye ve literatürde belirlenen çalışmalarla kıyaslamaya amaclandık. Gereç ve Yöntem: Ocak 2015-Ocak 2020 tarihleri arasında Çukurova Üniversitesi Tıp Fakültesi Balcalı hastanesinde mide kanseri nedeniyle gastrektomi uygulanmış toplam 189 hastanın klinik ve patolojik verileri retrospektif olarak incelendi. Postoperatif komplikasyonların şiddeti Clavien-Dindo sınıflandırmasıyla değerlendirildi ve hastalar iki gruba ayrıldı: Grup 1: Clavien-Dindo I- II (n=144). Grup 2: Clavien-Dindo ˃ II (III-IV-V derecesindeki (n=45). Komplikasyonların gelişimini etkileyen bağımsız risk faktörlerini belirlemek amacıyla tek değişkenli ve çok değişkenli lojistik regresyon analizleri uygulandı. Gruplar arasındaki istatistiksel farklılıkların değerlendirilmesinde Log-rank testi kullanıldı. Bulgular: Vücut kitle indeksi, özellikle BMI >30 kg/m² olan hastalarda, mortalite riskini anlamlı şekilde artıran güçlü bir bağımsız risk faktörü olarak belirlenmiştir (HR=3.31; GA: 1.39-7.89; p=0.007). Benzer şekilde, uzamış operasyon süresi (>240 dakika) hem postoperatif komplikasyonlar hem de mortalite açısından bağımsız bir risk faktörü olarak saptanmıştır (HR=2.09; GA: 1.25-3.51; p=0.005). Bu bulgu, cerrahi planlamanın deneyimli ekiplerce yapılmasının ve operasyon süresini kısaltmaya yönelik stratejilerin önemini vurgulamaktadır. Univaryant ve multivaryant Cox regresyon analizlerinde, neoadjuvan tedavinin diğer klinik ve patolojik değişkenlerden bağımsız olarak sağkalımı olumsuz etkilediği saptanmıştır. Multivaryant analizde HR yine 3.54 (95% GA: 1.93-6.48, p <0.001). Çalışmamızda histolojik olarak İyi diferansiye tümörlerde mortalite riski anlamlı olarak düşüktü (HR=0.54; GA: 0.26-1.29; p=0.039). İndiferansiye ve orta diferansiye tümörlerde anlamlı fark görülmedi. Sağkalım analizleri incelendiğinde, Clavien-Dindo sınıflandırmasına göre oluşturulan gruplar arasında belirgin farklılıklar izlenmiştir. Kaplan-Meier analizinde Grup 1'de medyan sağkalım süresi 45,6 ay iken, Grup 2'de bu süre 9,7 ay olarak hesaplanmış ve gruplar arasındaki fark istatistiksel olarak anlamlı bulunmuştur (p = 0,042). Özellikle erken dönem sonuçlar değerlendirildiğinde, majör komplikasyon gelişen hastalarda 1 yıllık sağkalım oranının anlamlı derecede düşük olduğu saptanmıştır. Buna karşın 5 yıllık sağkalım oranları arasındaki fark istatistiksel anlamlılık düzeyine ulaşmamıştır. Bu durum, postoperatif komplikasyonların özellikle erken dönem mortalite üzerinde güçlü bir etkiye sahip olduğunu, uzun dönem sağkalımın ise daha çok tümör biyolojisi, patolojik evre ve neoadjuvan tedaviye yanıt gibi faktörler tarafından belirlendiğini düşündürmektedir. Sonuç: Analizlerimizde BMI≥30 kg/m², tümör histopatolojik derecesi, neoadjuvan kemoterapi alması ve operasyon süresinin uzaması postoperatif komplikasyon gelişimi ile anlamlı şekilde ilişkili bulunmuştur. Bu belirtecler ğüclü bağımsız faktör olarak belirlenmiştir.

Aim: Postoperative complications following gastrectomy in patients operated on for gastric cancer have a significant impact on patients' quality of life and survival. The aim of this study was to evaluate the clinical, surgical, and pathological risk factors associated with the development of postoperative complications in patients undergoing gastrectomy for gastric cancer based on the Clavien-Dindo classification system, and to compare the obtained results with those reported in the existing literature. Materials and Methods: Between January 2015 and January 2020, the clinical and pathological data of a total of 189 patients who underwent gastrectomy for gastric cancer at Çukurova University Faculty of Medicine, Balcalı Hospital, were retrospectively reviewed. The severity of postoperative complications was assessed using the Clavien-Dindo classification system, and patients were divided into two groups: Group 1, Clavien-Dindo grades I-II (n = 144), and Group 2, Clavien-Dindo grades >II (grades III-V), (n = 45). Univaryant and multivaryant logistic regression analyses were performed to identify independent risk factors associated with the development of postoperative complications. The log-rank test was used to evaluate statistical differences between the groups. Results: Body mass index was identified as a strong independent risk factor significantly associated with increased mortality, particularly in patients with a BMI >30 kg/m² (HR = 3.31; 95% CI: 1.39-7.89; p = 0.007). Similarly, prolonged operative time (>240 minutes) was found to be an independent risk factor for both postoperative complications and mortality (HR = 2.09; 95% CI: 1.25-3.51; p = 0.005). These findings underscore the importance of surgical planning by experienced teams and the implementation of strategies aimed at reducing operative duration. In both univaryant and multivaryant Cox regression analyses, neoadjuvant therapy was shown to adversely affect survival independently of other clinical and pathological variables. In the multivaryant analysis, the hazard ratio remained significant (HR = 3.54; 95% CI: 1.93-6.48; p < 0.001). Histopathological evaluation demonstrated that patients with well-differentiated tumors had a significantly lower risk of mortality (HR = 0.54; 95% CI: 0.26-1.29; p = 0.039), and tumor differentiation was identified as an independent prognostic factor. In contrast, no statistically significant differences in mortality were observed in patients with moderately differentiated or poorly differentiated tumors. Survival analyses revealed marked differences between groups stratified according to the Clavien-Dindo classification. Kaplan-Meier analysis showed that the median overall survival was 45.6 months in Group 1, compared with 9.7 months in Group 2, with the difference reaching statistical significance (p = 0.042). Notably, evaluation of early outcomes demonstrated a significantly lower 1-year survival rate in patients who developed major postoperative complications. However, the difference in 5-year survival rates between the groups did not reach statistical significance. These findings suggest that postoperative complications exert a strong impact on early mortality, whereas long-term survival appears to be more strongly influenced by tumor biology, pathological stage, and response to neoadjuvant therapy. Conclusion: Our analyses demonstrated that BMI ≥30 kg/m², histopathological grade of the tumor, receipt of neoadjuvant chemotherapy and prolonged operative time were significantly associated with the development of postoperative complications. These parameters were identified as strong independent risk factors for postoperative complications.
