Özet
Polikistik Over Sendromu (PKOS); hiperandrojenizm ve anovülasyon ile karakterize, üreme fonksiyonlarını ve metabolik dengeyi bozan kompleks bir endokrin patolojidir. Güncel veriler, sirkadiyen ritim bozukluklarının PKOS patofizyolojisinde tetikleyici bir rol oynayabileceğine işaret etse de moleküler saat genleri ile over steroidojenik aktivitesi arasındaki mekanizmalar henüz tam olarak aydınlatılamamıştır. Bu çalışma, insan granüloza hücrelerinde (KGN) sirkadiyen saat genlerinin zamansal ekspresyon modellerini ve BMAL1 geninin steroidogenez üzerindeki düzenleyici etkisini PKOS odağında incelemeyi amaçlamaktadır. Deksametazon ile senkronize edilen KGN hücrelerinde çekirdek saat genleri (BMAL1, CLOCK, PER1/2/3, CRY1/2) ile steroidojenik enzimlerin (StAR, CYP19A1) ritmik ekspresyon profilleri qPCR ve Western blot yöntemleriyle belirlenmiştir. Dihidrotestosteron (DHT) ile oluşturulan in vitro PKOS modelinde BMAL1 salınımı LumiCycle ile incilenmiştir; eş zamanlı olarak BMAL1'in CYP19A1 promotör bölgesine doğrudan bağlanma düzeyi ChIP-qPCR ile analiz edilmiştir. Ayrıca, CRISPR/Cas9 tekniğiyle BMAL1 geninin nakavt moleküler saat gen hiyerarşisi ile CYP19A1 ve StAR temel steroidojenik enzimlerin ekspresyon düzeyleri üzerindeki etkileri değerlendirilmiştir. Araştırma bulguları, kontrol grubu granüloza hücrelerinde saat genleri ile StAR ve CYP19A1 enzimlerinin mRNA ve protein düzeyinde senkronize bir zamansal ekspresyon sergilediğini kanıtlamıştır. Buna karşın, PKOS benzeri modelde ve BMAL1 nakavt grubunda bu moleküler saat zamansal ekspresyonu bozulduğu ve steroidojenik enzimlerin zamansal dağılımının bozulduğu tespit edilmiştir. ChIP-qPCR analizleri, BMAL1'in CYP19A1 promotörüne doğrudan bağlandığını ortaya koymuştur. Sonuç olarak bu çalışma, BMAL1'in insan granüloza hücrelerinde molekuler saat ile steroidojenik aktiviteyi birbirine bağlayan kritik bir moleküler aracı olduğunu göstermektedir. Bu mekanizmanın aydınlatılması, PKOS tedavisinde kronoterapi tabanlı yeni yaklaşımların geliştirilmesine temel teşkil etmektedir.

Polycystic ovary Syndrome (PCOS) is a complex endocrine disorder in women of reproductive age, characterized by hyperandrogenism, anovulation, and altered steroidogenesis, leading to reproductive and metabolic dysfunction. Emerging evidence suggests that disruption of circadian rhythms may trigger PCOS pathophysiology; however, the underlying molecular mechanisms linking circadian clock genes to functional ovarian steroidogenic activity remain unclear. Accordingly, this study aimed to investigate circadian clock gene expression patterns and their regulatory roles in steroidogenesis in human granulosa cells, particularly the effect of BMAL1 on steroidogenic regulation and its involvement in the pathophysiology of PCOS. To explore, first, human granulosa cells (KGN) were synchronized with dexamethasone and harvested at Zeitgeber time (ZT) points over 48 hours. Quantitative Real-Time qPCR and Western blot analyses were utilized to profile the rhythmic expression of core clock genes, including BMAL1, CLOCK, PERs1/2/3, and CRYs1/2, and to assess the rhythmic expression of steroidogenic enzymes; StAR and CYP19A1. Subsequently, KGN cells were induced with dihydrotestosterone (DHT) to establish an in vitro PCOS-like model. The rhythmic oscillation of BMAL1 bioluminescence was investigated using a luciferase reporter machine (LumiCycle) in the control group (KGN) and the PCOS-like group. Next, chromatin immunoprecipitation (ChIP)-qPCR was used to detect direct binding of BMAL1 to CYP19A1 regulatory promoter region in the control group. Subsequently, CRISPR/Cas9-mediated BMAL1 knockout was induced in the control group to evaluate whether this would exhibit PCOS-like characteristics. The results elucidated the rhythmic expression of BMAL1, PERs 1/2/3, and CRY2 in control granulosa cells. Steroidogenic enzymes CYP19A1 and StaR exhibited time-dependent changes in expression at both the mRNA and protein levels. Conversely, the PCOS-like group exhibited altered temporal expression of molecular clock and StAR and CYP19A1, findings that were also observed in the BMAL1 knockout group. The ChIP-qPCR analyses revealed the direct binding of BMAL1 to the CYP19A1 regulatory promoter region. Furthermore, this study demonstrated that BMAL1 is a regulator of other core clock genes and steroidogenic activity in human granulosa cells; its knockout disrupted the temporal expression of molecular clock and altered steroidogenic activity, thereby promoting PCOS-like characteristics. To conclude, this study highlights the substantial role of the molecular pathway linking the molecular clock and steroidogenesis in human granulosa cell physiology; thereby suggesting the potential development of chronotherapeutic approaches for the management of PCOS.
