Özet

Sağlık okuryazarlığı, bireylerin sağlıkla ilgili bilgileri anlama, değerlendirme ve bu bilgileri sağlıkla ilgili kararlarında etkin biçimde kullanabilme kapasitesini ifade etmekte olup, özellikle kronik hastalıkların yönetiminde kritik bir öneme sahiptir. HBV, dünya genelinde yaygın bir enfeksiyon hastalığı olup, karaciğer sirozu, karaciğer yetmezliği ve hepatoselüler karsinom gibi ciddi sağlık sorunlarına yol açabilmektedir. Hepatit B enfeksiyonu, uzun dönem izlem ve tedavi gerektiren, bireysel ve toplumsal etkileri yüksek bir hastalık olduğundan, hastaların sağlık okuryazarlığı düzeylerinin doğru biçimde değerlendirilmesi ve modellenmesi gerekmektedir. Bu tez çalışmasının amacı, Hepatit B enfeksiyonuna özgü sağlık okuryazarlığını ölçebilecek geçerli ve güvenilir bir ölçek geliştirmek ve bu ölçekten elde edilen veriler kullanılarak sağlık okuryazarlığı düzeyinin istatistiksel ve makine öğrenmesi temelli yöntemlerle tahmin edilmesini sağlamaktır. Bu bağlamda, HBV'ye özel bir sağlık okuryazarlığı ölçeği geliştirilmesi, hem klinik uygulamalar hem de halk sağlığı politikaları açısından önemli bir boşluğu doldurmayı hedeflemektedir. Araştırmanın örneklemini, Bursa Yüksek İhtisas Eğitim ve Araştırma Hastanesi Enfeksiyon Hastalıkları Polikliniği'ne başvuran 219 kronik HBV hastası oluşturmaktadır. Veri toplama aracı olarak iki bölümden oluşan bir anket formu kullanılmıştır. Birinci bölümde katılımcıların sosyodemografik bilgileri yer alırken, ikinci bölümde HBV sağlık okuryazarlığını ölçmeye yönelik 5'li Likert tipi 16 soru yer almaktadır. Ölçek puanlamasında Avrupa Sağlık Okuryazarlığı Konsorsiyumu'nun önerdiği standardize indeks yöntemi kullanılmıştır. Ölçeğin içerik geçerliliği enfeksiyon hastalıkları ve klinik mikrobiyoloji alanındaki akademisyenler tarafından değerlendirilmiş kapsam geçerlilik oranları ile hesaplanmıştır. Ölçeğin faktör analizine uygunluğu Kaiser-Meyer-Olkin ve Bartlett's testleriyle ispatlanmıştır. Yapısal geçerliliği faktör analizleri ile incelenmiş, ölçeğin üç faktörlü yapısı istatistiksel olarak desteklenmiştir. Bu faktörler“Hastalığı Tanıma”,“Tedaviyi Anlama”ve“Sağlığın Geliştirilmesi”olarak isimlendirilmiştir. Ölçeğe ait soruların faktör yük dağılımları incelenmiş olup hiçbir faktör içerisinde tam olarak kendisini tanımlayamayan 11.sorunun ölçekten çıkarılmasına karar verilmiştir. Faktör analizinin ardından, her bir faktörün iç tutarlılığını ölçmek için Cronbach's Alpha katsayısı hesaplanmıştır ve elde edilen bulgular bütün faktörler için bu değerin > 0.70 olduğunu, ölçeğin geçerli ve güvenilir bir ölçüm aracı olduğunu göstermiştir. Verilerin dağılım özellikleri incelendiğinde sağlık okuryazarlığı skorlarının normal dağılıma uymadığı Shapiro-Wilk testi sonucunda belirlenmiştir. Normalleştirme yöntemlerinden anlamlı sonuçlar elde edilememesi üzerine sağlık okuryazarlığı düzeyi düşük, orta ve yüksek olmak üzere kategorilendirilmiştir. Sağlık okuryazarlığı düzeyinin tahmin edilmesine yönelik olarak öncelikle sıralı lojistik regresyon modeli uygulanmış, ancak model varsayımlarının sağlanamadığı görülmüştür. Bunun üzerine multinomial lojistik regresyon modeli kurulmuş ve modelin %97 yüksek doğruluk oranı ile başarılı bir sınıflandırma performansı sergilediği belirlenmiştir. Sağlık okuryazarlığı düzeyini en çok etkileyen faktörler sırasıyla sağlığın geliştirilmesi, tedaviyi anlama ve hastalığı tanıma olarak bulunmuştur. Çalışmanın devamında, sağlık okuryazarlığı skorunun sürekli bir değişken olarak tahmin edilmesi amacıyla Random Forest regresyon modeli oluşturulmuştur. Model %92,8 oranında başarılı tahmin yapmış ve modelin tahmin standart sapması yaklaşık ±2.6 puan olarak hesaplanmıştır. Model sonuçları, gerçek sağlık okuryazarlığı skorları ile tahmin edilen değerler arasında yüksek düzeyde bir uyum olduğunu göstermiştir. . Karşılaştırma amacıyla kullanılan K-en yakın komşu (KNN) algoritmasının ise sınıf dengesizliği içeren veri yapısında sınırlı bir performans sergilediği gözlemlenmiştir. Bu tez çalışması, Hepatit B sağlık okuryazarlığının ölçülmesi ve modellenmesine yönelik özgün bir ölçek sunmasının yanı sıra, sağlık verilerinde farklı istatistiksel ve makine öğrenmesi yöntemlerinin karşılaştırmalı olarak değerlendirilmesine katkı sağlamaktadır. Elde edilen bulguların, sağlık okuryazarlığını artırmaya yönelik müdahalelerin planlanması ve benzer sağlık alanı çalışmalarına yöntemsel bir rehber oluşturması beklenmektedir.

Health literacy refers to individuals' ability to access, understand, evaluate, and effectively use health-related information in order to make appropriate health decisions and maintain or improve their health status. In recent decades, health literacy has emerged as one of the most important determinants of public health outcomes. Individuals with higher levels of health literacy are more capable of understanding disease processes, following treatment recommendations, adopting preventive health behaviors, and interacting effectively with healthcare systems. Conversely, inadequate health literacy has been associated with poor disease management, increased hospitalization rates, higher healthcare costs, and unfavorable health outcomes. The importance of health literacy becomes even more pronounced in the management of chronic diseases. Chronic illnesses require long-term monitoring, continuous treatment adherence, lifestyle modifications, and effective communication with healthcare providers. Patients must be able to understand medical information, interpret treatment instructions, and make informed decisions regarding their health. Therefore, evaluating and improving health literacy levels among individuals with chronic diseases has become a priority in modern healthcare systems. Hepatitis B virus (HBV) infection represents a major global health problem affecting millions of individuals worldwide. HBV is a highly infectious virus that primarily affects the liver and can lead to severe complications such as chronic hepatitis, liver cirrhosis, liver failure, and hepatocellular carcinoma. According to global health estimates, billions of people have been exposed to HBV, and hundreds of millions of individuals live with chronic infection. HBV-related complications continue to cause a substantial number of deaths every year. Due to its chronic nature and potential long-term consequences, effective management of HBV infection requires regular medical follow-up, adherence to antiviral treatment when necessary, lifestyle adjustments, and preventive measures to reduce transmission. The management of HBV infection is closely related to patients' understanding of their disease and their ability to follow medical recommendations. Patients with higher levels of disease-specific health literacy are more likely to adhere to treatment protocols, attend regular follow-up visits, understand transmission risks, and adopt protective health behaviors. In contrast, inadequate knowledge and misunderstanding of the disease may lead to poor treatment adherence, delayed medical care, and increased risk of complications or transmission. Despite the recognized importance of health literacy in disease management, many studies rely on general health literacy scales that may not adequately capture disease-specific knowledge and competencies. Generic health literacy instruments may overlook critical aspects related to specific conditions, such as treatment adherence, disease monitoring, and preventive behaviors. In the context of Hepatitis B, the lack of a disease-specific health literacy measurement tool represents a significant limitation for both clinical practice and research. Therefore, developing a scale specifically designed to measure HBV-related health literacy can provide more accurate insights into patients' understanding of their disease and their ability to manage it effectively. The primary aim of this thesis is to develop a valid and reliable measurement scale for assessing Hepatitis B–specific health literacy and to model health literacy levels using both statistical and machine learning–based approaches. By integrating traditional statistical modeling techniques with modern machine learning methods, the study aims to provide a comprehensive evaluation of health literacy prediction and classification in patients with chronic HBV infection. The study was conducted among patients diagnosed with chronic Hepatitis B who applied to the Infectious Diseases Outpatient Clinic of Bursa Yüksek İhtisas Training and Research Hospital. The study sample consisted of 219 individuals who met the inclusion criteria and voluntarily agreed to participate in the research. Data were collected using a structured questionnaire developed specifically for the study. The questionnaire consisted of two main sections. The first section included questions related to sociodemographic characteristics such as age, gender, education level, marital status, place of residence, income level, alcohol consumption, and clinical information related to the duration of HBV diagnosis and follow-up frequency. The second section contained items designed to measure HBV-related health literacy. A total of 16 items were initially developed to assess patients' abilities to obtain, understand, and apply information related to Hepatitis B. These items were designed based on existing health literacy frameworks and the clinical characteristics of HBV infection. Each item was measured using a five-point Likert-type scale ranging from“very difficult”to“very easy,”with an additional response option indicating lack of knowledge. The scoring of the scale was performed using the standardized index method recommended by the European Health Literacy Consortium, which transforms raw item scores into a standardized health literacy index. The scale development process followed established methodological procedures for instrument validation. Content validity was first evaluated through expert review. Academic experts in infectious diseases and clinical microbiology were consulted to assess the relevance, clarity, and adequacy of each item. Content validity ratios were calculated based on expert evaluations, and necessary revisions were made to improve item clarity and relevance. Following the content validation stage, the suitability of the dataset for factor analysis was assessed using the Kaiser–Meyer–Olkin (KMO) measure of sampling adequacy and Bartlett's test of sphericity. The results indicated that the dataset was appropriate for factor analysis. Exploratory factor analysis was conducted to identify the underlying factor structure of the scale. The analysis revealed a three-factor structure representing different dimensions of HBV-related health literacy. These factors were interpreted and labeled as“Disease Recognition,”“Understanding Treatment,”and“Health Promotion.”The factor structure reflects the multidimensional nature of health literacy in the context of chronic disease management. Factor load distributions were examined to determine the contribution of each item to the underlying factors. During this stage, item 11 did not demonstrate a clear loading on any factor and was therefore removed from the final version of the scale. The remaining items demonstrated satisfactory factor loadings, indicating that they effectively represent the intended health literacy dimensions. The reliability of the scale was assessed using Cronbach's alpha coefficients to evaluate internal consistency. The results indicated that all factors had alpha values greater than 0.70, which is commonly considered the acceptable threshold for reliability in social science research. These findings demonstrate that the developed scale is both valid and reliable for measuring Hepatitis B–specific health literacy. After calculating health literacy scores using the standardized index method, the distribution of the scores was examined. The Shapiro–Wilk test indicated that the health literacy scores did not follow a normal distribution. Since data transformation techniques did not successfully normalize the distribution, health literacy scores were categorized into three levels: low, medium, and high health literacy. In order to model and predict health literacy levels, several statistical and machine learning approaches were applied. Initially, an ordinal logistic regression model was considered due to the ordered nature of the health literacy categories. However, diagnostic tests indicated that the proportional odds assumption required for ordinal logistic regression was violated. As a result, a multinomial logistic regression (MLR) model was constructed as an alternative approach. The multinomial logistic regression model demonstrated a high level of classification performance, achieving an accuracy rate of approximately 97%. The model results indicated that the three factors identified in the scale—health promotion, understanding treatment, and disease recognition—played significant roles in determining health literacy levels. Among these factors, health promotion was found to have the strongest influence on health literacy classification, followed by understanding treatment and disease recognition. In addition to classical statistical modeling, machine learning techniques were also employed in order to evaluate their predictive performance. A Random Forest regression model was developed to estimate health literacy scores as a continuous variable. Random Forest is a widely used ensemble learning method capable of capturing nonlinear relationships and complex interactions among variables. The Random Forest model achieved a prediction performance of approximately 92.8%, with a prediction error corresponding to an estimated standard deviation of approximately ±2.6 points. The results demonstrated a strong agreement between the predicted health literacy scores and the actual observed values. To provide a comparative perspective, the K-nearest neighbors (KNN) algorithm was also applied as a classification method. However, the performance of the KNN model was relatively limited compared to the other approaches. The main reason for this reduced performance was the presence of class imbalance in the dataset, which negatively affected the algorithm's ability to correctly classify minority categories. As a result, the KNN model demonstrated lower predictive accuracy and weaker classification performance compared to the multinomial logistic regression and Random Forest models.Overall, the findings of this study highlight the importance of disease-specific health literacy assessment tools in the management of chronic diseases such as Hepatitis B. The developed scale provides a structured and statistically validated instrument for measuring HBV-related health literacy. By identifying the key dimensions that influence patients' ability to manage their disease, the scale can support healthcare professionals in evaluating patients' educational needs and designing targeted interventions. Furthermore, the comparative analysis of statistical and machine learning approaches demonstrates that combining traditional statistical models with modern predictive algorithms can provide valuable insights into health data. While statistical models offer interpretability and theoretical grounding, machine learning methods provide enhanced predictive capabilities, especially when dealing with complex and multidimensional datasets.In conclusion, this thesis contributes to the existing literature by introducing a novel measurement scale specifically designed for Hepatitis B health literacy and by demonstrating the application of both statistical and machine learning techniques for modeling health literacy outcomes. The developed scale has the potential to support clinical decision-making, guide patient education programs, and inform public health policies aimed at improving disease awareness and management among individuals living with chronic Hepatitis B. Future studies may expand this research by applying the scale to larger and more diverse populations across different regions and healthcare settings. Such studies would allow further validation of the scale and provide deeper insights into the determinants of health literacy among HBV patients. Additionally, integrating digital health tools and predictive analytics into healthcare systems may further enhance the monitoring and improvement of patient health literacy levels.
