Özet
ÖZET Amaç: Çalışmamızın amacı, Ankara Etlik Şehir Hastanesi Onkoloji Servisinde yatarak tedavi gören geriatrik evre 3–4 kanser hastalarında modifiye Glasgow prognostik indeksinin (mGPS) mortalite ile ilişkisini değerlendirmektir. Sistemik inflamasyonu ve beslenme durumunu yansıtan mGPS'nin ileri evre kanserli geriatrik hastalarda prognozu öngörmedeki değerinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Bu çalışma, Ankara Etlik Şehir Hastanesi Onkoloji Servisinde takip edilen evre 3–4 solid tümör tanılı geriatrik hastaların dahil edildiği retrospektif gözlemsel bir araştırmadır. Çalışmaya 65 yaş ve üzeri, evre 3 veya evre 4 kanser tanısı bulunan ve onkoloji servisinde yatarak tedavi gören hastalar dahil edildi. Hastaların demografik özellikleri, klinik bulguları, komorbiditeleri, metastaz durumları ve laboratuvar parametreleri hastane kayıt sisteminden elde edildi. Hastaların sistemik inflamasyon ve beslenme durumunu değerlendirmek amacıyla modifiye Glasgow prognostik indeksi (mGPS) hesaplandı. mGPS hesaplanmasında serum C-reaktif protein (CRP)ve albümin düzeyleri kullanıldı.Tedavi öncesi C-reaktif protein (CRP) ve serum albumin düzeylerine göre mGPS hesaplandı ve hastalar üç gruba ayrıldı: mGPS 0 (CRP ≤10 mg/L), mGPS 1 (CRP >10 mg/L ve albumin ≥35 g/L) ve mGPS 2 (CRP >10 mg/L ve albumin <35 g/L). Glasgow prognostik indeksi gruplarına göre klinik ve demografik özellikler karşılaştırıldı. Kısa dönem mortalite (30 ve 90 günlük mortalite) ile mGPS arasındaki ilişki analiz edildi. Sağkalım süreleri Kaplan–Meier yöntemi ile değerlendirildi ve gruplar arası fark log-rank testi ile analiz edildi. Genel sağkalım ile ilişkili prognostik faktörler Cox regresyon analizi ile incelendi. İstatistiksel analizler SPSS programı kullanılarak yapıldı ve p<0,05 değeri istatistiksel olarak anlamlı kabul edildi. Bulgular: Çalışmaya toplam 1128 geriatrik kanser hastası dahil edildi. Hastaların 820'si (%72.7) 75 yaş altı, 308'i (%27.3) 75 yaş ve üzeriydi. Çalışma grubunun 717'si (%63.6) erkek, 411'i (%36.4) kadın hastalardan oluşmaktaydı. Modifiye Glasgow Prognostik Skoruna (mGPS) göre hastaların 81'i (%7.2) mGPS 0, 352'si (%31.2) mGPS 1 ve 695'i (%61.6) mGPS 2 grubunda yer aldı. Cinsiyet dağılımı mGPS grupları arasında anlamlı farklılık göstermekteydi (p=0.005) ve yüksek mGPS grubunda erkek hastaların oranı daha fazlaydı. Tüm popülasyonda en sık görülen kanser grubu gastrointestinal sistem tümörleri (%34.6) olup bunu akciğer kanseri (%29.1) izledi. Hastaların %27.9'unda kemik metastazı, %20.1'inde akciğer metastazı, %25.7'sinde karaciğer metastazı ve %9.9'unda beyin metastazı saptandı. Kemik metastazı varlığı mGPS grupları arasında anlamlı farklılık göstermekteydi (p=0.018). Laboratuvar parametreleri incelendiğinde medyan beyaz küre sayısı 6,7×10⁹/L, hemoglobin 10,7 g/dL, trombosit 222×10⁹/L, CRP 35,1 mg/L ve albumin 32,6 g/L olarak bulundu. Kısa dönem mortalite analizlerinde mGPS skorları ile mortalite arasında anlamlı ilişki saptandı. 30 günlük mortalite mGPS 0 grubunda %8.6, mGPS 1 grubunda %4.8, mGPS 2 grubunda ise %20.9 olarak bulundu (p<0.001). 90 günlük mortalite oranları ise sırasıyla %30.9, %21.6 ve %51.7 olarak saptandı (p<0.001). Sağkalım analizlerinde tüm kohort için medyan genel sağkalım süresi 4.27 ay olarak hesaplandı. mGPS gruplarına göre medyan sağkalım süreleri mGPS 0 grubunda 4.63 ay, mGPS 1 grubunda 9.30 ay ve mGPS 2 grubunda 2.83 ay olarak bulundu. Kaplan–Meier analizinde mGPS grupları arasında genel sağkalım açısından anlamlı farklılık saptandı (Log-rank χ²=128,9; p<0.001). Tek değişkenli Cox regresyon analizinde yaş ≥75 (HR:1.40; p<0.001), erkek cinsiyet (HR:1.35; p<0.001), sigara öyküsü (HR:1.29; p=0.003) ve mGPS skoru (HR:1.90; p<0.001) genel sağkalım ile anlamlı ilişkili bulundu. Çok değişkenli analizde yaş ≥75 (aHR:1.32; p<0.001), sigara öyküsü (aHR:1.22; p=0.021) ve mGPS skoru (p<0.001)bağımsız prognostik faktörler olarak saptandı. mGPS 2 grubunda ölüm riski mGPS 0 grubuna göre anlamlı derecede yüksek bulundu (aHR:1.58; %95 GA:1.17–2.13; p=0.003). Sonuç: Sonuç olarak, bu çalışma ileri evre kanser tanısı bulunan geriatrik hastalarda modifiye Glasgow prognostik skorunun (mGPS) kısa dönem mortalite ve genel sağkalım ile anlamlı şekilde ilişkili olduğunu göstermektedir. Yüksek mGPS skoruna sahip hastalarda mortalite oranlarının daha yüksek ve sağkalım sürelerinin daha kısa olduğu saptanmıştır. mGPS'nin geriatrik onkoloji hastalarında prognozun öngörülmesinde basit, kolay uygulanabilir ve klinik açıdan değerli bir prognostik belirteç olabileceği düşünülmektedir.

ABSTRACT Aim: The aim of our study is to evaluate the relationship between the modified Glasgow Prognostic Index (mGPS) and mortality in geriatric stage 3-4 cancer patients hospitalized in the Oncology Department of Ankara Etlik City Hospital. The study aimed to investigate the value of mGPS, which reflects systemic inflammation and nutritional status, in predicting prognosis in geriatric patients with advanced-stage cancer. Materials and Methods: This study is a retrospective observational research involving geriatric patients diagnosed with stage 3–4 solid tumors followed up in the Oncology Department of Ankara Etlik City Hospital. Patients aged 65 years and older, diagnosed with stage 3 or stage 4 cancer, and receiving inpatient treatment in the oncology department were included in the study. Demographic characteristics, clinical findings, comorbidities, metastasis status, and laboratory parameters of the patients were obtained from the hospital record system. The modified Glasgow Prognostic Index (mGPS) was calculated to assess the patients' systemic inflammation and nutritional status. Serum C-reactive protein (CRP) and albumin levels were used in mGPS calculation. mGPS was calculated based on pre-treatment C-reactive protein (CRP) and serum albumin levels, and patients were divided into three groups: mGPS 0 (CRP ≤10 mg/L), mGPS 1 (CRP >10 mg/L and albumin ≥35 g/L), and mGPS 2 (CRP >10 mg/L and albumin <35 g/L). Clinical and demographic characteristics were compared according to mGPS groups. The relationship between short-term mortality (30 and 90-day mortality) and mGPS was analyzed. Survival times were evaluated using the Kaplan–Meier method, and differences between groups were analyzed using the log-rank test. Prognostic factors associated with overall survival were examined using Cox regression analysis. Statistical analyses were performed using SPSS software, and a p-value <0.05 was considered statistically significant. Results: A total of 1128 geriatric cancer patients were included in the study. Of the patients, 820 (72.7%) of the patients were under 75 years of age, and 308 (27.3%) were 75 years and older. Additionally, 717 (63.6%) of the study group were male, and 411 (36.4%) were female. According to the Modified Glasgow Prognostic Score (mGPS), 81 (7.2%) of the patients were in the mGPS 0 group, 352 (31.2%) in the mGPS 1 group, and 695 (61.6%) in the mGPS 2 group. The gender distribution showed a significant difference between mGPS groups (p=0.005), with a higher proportion of male patients in the higher mGPS group. The most common cancer group in the entire population was gastrointestinal tumors (34.6%), followed by lung cancer (29.1%). Bone metastases were detected in 27.9% of patients, lung metastases in 20.1%, liver metastases in 25.7%, and brain metastases in 9.9%. The presence of bone metastases showed a significant difference between mGPS groups (p=0.018). When laboratory parameters were examined, the median white blood cell count was 6.7×10⁹/L, hemoglobin 10.7 g/dL, platelet count 222×10⁹/L, CRP 35.1 mg/L, and albumin 32.6 g/L. In short-term mortality analyses, a significant relationship was found between mGPS scores and mortality. 30-day mortality was 8.6% in the mGPS 0 group, 4.8% in the mGPS 1 group, and 20.9% in the mGPS 2 group (p<0.001). The 90-day mortality rates were found to be 30.9%, 21.6%, and 51.7%, respectively (p<0.001). In survival analyses, the median overall survival time for the entire cohort was calculated as 4.27 months. Median survival times according to mGPS groups were found to be 4.63 months in the mGPS 0 group, 9.30 months in the mGPS 1 group, and 2.83 months in the mGPS 2 group. In the Kaplan–Meier analysis, a significant difference was found in overall survival among mGPS groups (Log-rank χ²=128.9; p<0.001). In univariate Cox regression analysis, age ≥75 (HR:1.40; p<0.001), male gender (HR:1.35; p<0.001), smoking history (HR:1.29; p=0.003), and mGPS score (HR:1.90; p<0.001) were found to be significantly associated with overall survival. In multivariate analysis, age ≥75 (aHR:1.32; p<0.001), smoking history (aHR:1.22; p=0.021), and mGPS score (p<0.001) were identified as independent prognostic factors. The risk of death was significantly higher in the mGPS 2 group compared to the mGPS 0 group (aHR:1.58; 95% CI:1.17–2.13; p=0.003). Conclusion: In conclusion, this study shows that the modified Glasgow Prognostic Score (mGPS)was significantly associated with short-term mortality and overall survival in geriatric patients diagnosed with advanced-stage cancer. Patients with higher mGPS scores were found to have higher mortality rates and shorter survival times. It is thought that mGPS could be a simple, easily applicable, and clinically valuable prognostic marker for predicting prognosis in geriatric oncology patients.
