Özet
Total Diz Artroplasti Cerrahisinde Addüktör Kanal Bloğuna Eklenen Ipack Blok İle Adductor Magnus Kas Plan Bloğunun Etkinliğinin Karşılaştırması Amaç: Total diz artroplasti cerrahisinde addüktör kanal bloğuna eklenen ıpack blok ile adductor magnus kas plan bloğunun postoperatif analjezik tüketimine etkisini araştırmaktır. Sekonder amaç ise postoperatif ağrı skoru ve mobilizasyon süresine etkisini araştırmaktır. Materyal ve Metod: Bu çalışma, Atatürk Üniversitesi Anesteziyoloji ve Reanimasyon Anabilim Dalı ameliyathanesinde, total diz artroplastisi planlanan ortopedi hastalarında prospektif randomize olarak gerçekleştirildi. Etik kurul onayı alındıktan sonra ASA I-II-III grubu, 18-75 yaş aralığındaki hastalar çalışmaya dâhil edildi. Karaciğer, böbrek ve ileri derecede kalp yetmezliği olan, nöropatik ağrısı bulunan, rutin analjezik kullanımı olan veya son 24 saat içinde analjezik alan hastalar, ayrıca ciddi kardiyovasküler hastalığı, koagülopatisi, karaciğer fonksiyon bozukluğu bulunan, antikoagülan ilaç kullanan, koopere olunamayan ya da kullanılacak ilaçlara alerjisi olan hastalar çalışma dışı bırakıldı. Toplam 60 hasta çalışmaya alındı ve randomizasyon 1:1 bilgisayar programı ile gerçekleştirildi. Hastalar iki gruba ayrıldı:Grup IPACK (IPACK Bloğu Grubu),Grup AMM (Adductor Magnus Kas Plan Bloğu Grubu). Tüm hastalar operasyondan bir saat önce ameliyathane blok odasına alındı ve blok işlemleri burada uygulandı. Blok sırasında iğne görünürlüğü, anatomik yapıların seçilebilirliği ve blok süresi kaydedildi. Blok sonrası duyusal ve motor değerlendirmeler 5., 10., 15., 20., 30. ve 45. dakikalarda yapıldı. Tibial, sural, safen, yüzeyel peroneal ve posterior femoral kutanöz sinirlerin duyusal fonksiyonları test edildi. Motor blok, diz ve ayak bileği hareketleri gözlemlenerek üç puanlı skala (2 = normal güç, 1 = parezi, 0 = felç) ile değerlendirildi. Tüm hastalara 15 mg hipertonik bupivakain ile spinal anestezi uygulandı. Spinal anestezi öncesi 8 mg ondansetron ve 8 mg deksametazon intravenöz olarak verildi. Sedasyon amaçlı 2 mg iv midazolam yapıldı. Cerrahiden 30 dakika önce 1000 mg parasetamol, intraoperatif olarak deksketoprofen uygulandı. Postoperatif dönemde ağrı kontrolü için hastalara fentanil içeren hasta kontrollü analjezi (PCA) cihazı bağlandı (konsantrasyon: 10 mcg/mL, bolus: 25 mcg, kilitlenme süresi: 15 dk, bazal infüzyon yok). PCA cihazı 48 saat boyunca kullanıldı. Derlenme odasında VAS skoru 4 ve üzeri olan hastalara 100 mg tramadol uygulandı. Aldrette skoru 9 ve üzeri olan hastalar servise alındı. Ameliyat sonrası ağrı değerlendirmeleri 1., 2., 4., 8., 12. ve 24. saatlerde yapıldı. Postoperatif opioid tüketimi, ilk analjezi zamanı, ek analjezik gereksinimi, yan etkiler (bulantı, kusma, kaşıntı, sedasyon) ve blok komplikasyonları kaydedildi. Hastaların postoperatif mobilizasyonu, VAS skoru, TUG (Timed Up and Go) testi, quadriceps kas gücü, eklem hareket açıklığı (ROM) ve FTSST (Five-Times Sit-to-Stand Test) 1. ve 2. günlerde değerlendirildi. Olguların postoperatif takip ve değerlendirmeleri çalışma grubu hakkında bilgisi olmayan bir araştırmacı tarafından yapıldı. Bulgular: Gruplar arasında cinsiyet, yaş, ağırlık, boy, VKİ, ASA sınıflaması, anestezi süresi ve cerrahi süre açısından istatistiksel olarak anlamlı bir fark saptanmamıştır (p>0.05). Her iki gruptaki hastaların preoperatif yürüme desteği gereksinimi, varus-valgus deformitesi, preoperatif istirahat ve hareketle ağrı skorlarında da anlamlı bir fark bulunmamıştır. Blok performansına ilişkin değerlendirilen iğne ucu görünebilirliği, iğne şaftı görünebilirliği, iğne giriş sayısı ve blok uygulama süresi parametreleri iki grup arasında benzer bulunmuştur (p>0.05). Ayrıca n. tibialis, n. peronealis, n. suralis, n. safenus ve n. posterior femoral kütanöz sinir dermatom muayenesinde gruplar arasında istatistiksel olarak anlamlı fark izlenmemiştir. Postoperatif 0–4, 4–8 ve 8–24 saatlik dönemlerde, ayrıca 24 saat sonundaki toplam fentanil tüketimi açısından gruplar arasında istatistiksel olarak anlamlı bir fark saptanmamıştır (p>0.05). İlk analjezik uygulama zamanı da iki grupta benzer bulunmuştur. Derlenme odasında hiçbir hastada ağrı skorunun 4 ve üzerinde olmaması nedeniyle ek analjezik gereksinimi olmamıştır. Opioid tüketimine bağlı bulantı, kusma, antiemetik gereksinimi, kabızlık, kaşıntı, idrar retansiyonu ve ağız kuruluğu gibi yan etkiler açısından iki grup arasında anlamlı fark bulunmamıştır (p>0.05). Hastaların PACU'da ve postoperatif 1., 2., 4., 8., 12., 24. ve 48. saatlerdeki istirahat ve hareketle VAS skorları da anlamlı farklılık göstermemiştir (p>0.05). Gruplar arasında ilk mobilizasyon süresi ile preoperatif ve postoperatif 1. ve 2. günlerde uygulanan TUG testi, postoperatif 1. ve 2. günlerde uygulanan FTSST testi ve preoperatif ile postoperatif 1. ve 2. günlerde değerlendirilen eklem hareket açıklığı (ROM) skorları açısından istatistiksel olarak anlamlı bir fark saptanmamıştır (p>0.05). Preoperatif ve postoperatif 1. ve 2. günlerde ölçülen anterior tibialis ve quadriceps kas gücüne ait motor fonksiyon değerlendirildiğinde IPACK ve AMM grupları arasında benzer bulunmuştur (p>0.05). Sonuç: Bu prospektif randomize çalışma, total diz artroplastisi sonrası adduktör kanal bloğuna eklenen IPACK bloğu ile adductor magnus kas plan bloğunun, standart multimodal analjezi protokolü altında postoperatif opioid tüketimi, ağrı skorları ve fonksiyonel iyileşme parametreleri açısından benzer klinik etkinlik sağladığını göstermiştir. Her iki teknik de motor fonksiyonları belirgin şekilde etkilemeden posterior diz analjezisi sunmuş ve erken mobilizasyon göstergeleri üzerinde karşılaştırılabilir sonuçlar vermiştir. Bu bulgular, hızlı iyileşme (fast-track/ERAS) protokolleri kapsamında IPACK ve AMM bloklarının total diz artroplastisinde birbirine alternatif motor-koruyucu analjezik stratejiler olarak değerlendirilebileceğini düşündürmektedir. Bununla birlikte, bu sonuçların doğrulanması ve klinik uygulamadaki yerlerinin daha net tanımlanabilmesi için daha geniş örneklemli ve çok merkezli çalışmalara ihtiyaç vardır.

Comparison of the Effectiveness of IPACK Block Versus Adductor Magnus Muscle Plane Block When Combined with Adductor Canal Block in Total Knee Arthroplasty Surgery Purpose: The primary aim is to investigate the effect of the IPACK block and the Adductor Magnus Muscle Plane Block, when added to the Adductor Canal Block, on postoperative analgesic consumption in total knee arthroplasty surgery. The secondary aim is to evaluate their effects on postoperative pain scores and time to mobilization. Material and Method: This study was conducted prospectively and randomized in the operating room of Atatürk University, Department of Anesthesiology and Reanimation, on orthopedic patients scheduled for total knee arthroplasty. After obtaining approval from the institutional ethics committee, patients aged between 18 and 75 years and classified as ASA physical status I–II–III were included in the study. Patients with hepatic, renal, or advanced cardiac failure; those with neuropathic pain; with routine analgesic use or who had received analgesics within the previous 24 hours; and those with severe cardiovascular disease, coagulopathy, hepatic dysfunction, anticoagulant use, inability to cooperate, or known allergy to the study medications were excluded. A total of 60 patients were enrolled, and randomization was performed in a 1:1 ratio using a computer-generated sequence. The patients were divided into two groups: Group IPACK (IPACK Block Group) and Group AMM (Adductor Magnus Muscle Plane Block Group). All patients were taken to the block room of the operating theater one hour before surgery, where the block procedures were performed. During the block, needle visibility, identification of anatomical structures, and block duration were recorded. Sensory and motor assessments were conducted at the 5th, 10th, 15th, 20th, 30th, and 45th minutes following the block. The sensory functions of the tibial, sural, saphenous, superficial peroneal, and posterior femoral cutaneous nerves were tested. Motor block was evaluated using a three-point scale (2 = normal strength, 1 = paresis, 0 = paralysis) by observing knee and ankle movements. All patients received spinal anesthesia with 15 mg hyperbaric bupivacaine. Prior to spinal anesthesia, 8 mg ondansetron and 8 mg dexamethasone were administered intravenously. For sedation purposes, 2 mg of intravenous midazolam was administered. Thirty minutes before surgery, 1000 mg paracetamol was given, and dexketoprofen was administered intraoperatively. In the postoperative period, patient-controlled analgesia (PCA) devices containing fentanyl were connected for pain control (concentration: 10 mcg/mL, bolus dose: 25 mcg, lockout interval: 15 min, no basal infusion). The PCA devices were used for 48 hours. Patients with a Visual Analog Scale (VAS) score of 4 or higher in the recovery room received 100 mg tramadol. Those with an Aldrete score of 9 or higher were transferred to the ward. Postoperative pain assessments were performed at the 1st, 2nd, 4th, 8th, 12th, and 24th hours. Postoperative opioid consumption, time to first analgesia, need for additional analgesics, side effects (nausea, vomiting, pruritus, sedation), and block-related complications were recorded. Postoperative mobilization, VAS scores, Timed Up and Go (TUG) test, quadriceps muscle strength, range of motion (ROM), and Five-Times Sit-to-Stand Test (FTSST) were evaluated on postoperative days 1 and 2. All postoperative follow-ups and evaluations were performed by a researcher blinded to the group allocation. Results: No statistically significant differences were identified between the groups regarding sex, age, weight, height, BMI, ASA classification, anesthesia duration, or surgical duration (p>0.05). Preoperative walking support requirements, presence of varus–valgus deformity, and preoperative rest and movement pain scores were also comparable between the two groups. Regarding block performance parameters—needle tip visibility, needle shaft visibility, number of needle insertions, and block performance time—no statistically significant differences were observed between the groups (p>0.05). Additionally, dermatomal assessments of the tibial, peroneal, sural, saphenous, and posterior femoral cutaneous nerves did not show significant intergroup differences. Postoperative fentanyl consumption during the 0–4, 4–8, and 8–24 hour intervals, as well as total fentanyl consumption at 24 hours, did not differ significantly between the groups (p>0.05). The time to first analgesic administration was also similar. No patient in either group reported a pain score of 4 or above in the recovery room; therefore, no additional analgesics were required. Opioid-related adverse effects—including nausea, vomiting, need for antiemetics, constipation, pruritus, urinary retention, and dry mouth—were comparable between the groups (p>0.05). Rest and movement VAS scores recorded in the PACU and at postoperative 1, 2, 4, 8, 12, 24, and 48 hours also showed no statistically significant differences (p>0.05). No significant differences were observed between the groups in time to first mobilization, in TUG test results measured preoperatively and on postoperative days 1 and 2, in FTSST scores on postoperative days 1 and 2, or in ROM measurements recorded preoperatively and on postoperative days 1 and 2 (p>0.05). Motor function, assessed by anterior tibialis and quadriceps muscle strength measured preoperatively and on postoperative days 1 and 2, was found to be similar between the IPACK and AMM groups (p > 0.05). Conclusion: This prospective randomized study demonstrated that, under a standard multimodal analgesia protocol, the IPACK block added to the adductor canal block and the adductor magnus muscle plane block provided comparable clinical efficacy in terms of postoperative opioid consumption, pain scores, and functional recovery parameters following total knee arthroplasty. Both techniques offered posterior knee analgesia without significantly affecting motor function and yielded comparable outcomes with respect to early mobilization indicators. These findings suggest that, within the framework of fast-track/ERAS protocols, IPACK and AMM blocks may be considered alternative motor-sparing analgesic strategies in total knee arthroplasty. Nevertheless, larger-scale, multicenter studies are required to confirm these results and to more clearly define their role in clinical practice."
