Özet
Amaç: Bu çalışmada, akut miyeloid lösemi tanılı hastalarda Prognostik Nutrisyonel İndeks (PNI), B12/CRP Skoru (BCI), Glasgow Prognostik Skoru (GPS) skorlarının mortalite, yoğun bakım ihtiyacı ve genel sağkalımı öngörmedeki performanslarının karşılaştırılması ayrıca ELN 2022 risk sınıflaması ve akut miyeloid lösemide kullanılan klasik prognostik belirteçlerden bağımsız prognostik faktör olup olmadıklarının araştırılması amaçlandı. Gereç ve Yöntem: Bu retrospektif çalışmaya, Konya Şehir Hastanesi Hematoloji Kliniğinde 2020-2025 yılları arasında takip edilen hastalar dahil edildi ve çalışma hasta dosyaları üzerinden gerçekleştirildi. Konya Şehir Hastanesi Hematoloji Kliniğine başvuran hastalar içerisinde akut miyeloid lösemi tanısı alan, tanısal testleri eksik olmayan, tedavi alan ve tedavi sonrası takip edilen, 18 yaş ve üzeri hastalar çalışmanın örneklemini oluşturmaktadır. Akut miyeloid lösemi tanılı hastalar arasından dahil edilme kriterlerini karşılayan ardışık hastalar çalışmaya alındı. Çalışmaya katılan tüm hastaların başvurusunda demografik verileri, klinik bilgileri, görüntüleme tetkikleri ve muayeneleri bulguları incelendi. Çalışmada hastalara ait ölçümlerle elde olunan indeksleri ile beraber tedavi sürecindeki prognozları incelendi. Bulgular: Çalışmamıza dahil edilen toplam 106 hastadan 57 (%53,8) kişi kadın, 49 kişi (%46,2) kişi erkek olup cinsiyet ve eksitus-sağ ilişkisi istatistiksel olarak anlamsız bulundu (p=0,116). Ortanca tanı yaşı 53,5 ve yaş aralığı 18 ile 85 arasında değişmekte idi ve eksitus-sağ ilişkisi istatistiksel olarak anlamlı bulundu (p=0,044). Hastaneye ilk başvurusunda enfeksiyon varlığı tespit edilenlerin dağılımına göre sağ kalanlar 20 (%37,0), ölenler 33 (%63,5) hasta olup aralarında istatistiksel olarak anlamlı fark saptanmıştır (p=0,007). Hastaneye ilk başvurusunda pnömoni saptanan hastaların dağılımına göre sağ kalanlar 4 (%7,4) ölenler 17 (%32,7) hasta olup aralarında istatistiksel olarak anlamlı fark saptanmıştır (p=0,001). Birinci ay yanıtın ölen ve kalanlarla arasındaki ilişki istatistiksel olarak anlamlı fark saptandı (p<0,001). Tüm hastaların ilk başvuru anındaki laboratuvar sonuçlarından elde edilen skorlamalara bakıldığında 43 (%40,6) hasta PNI skoruna göre kötü prognozlu, 63 (%59,4) hasta iyi prognozlu olarak değerlendirildi. Glasgow Prognostik Skoruna göre 39 (%36,8) hasta iyi prognozlu, 47 (%44,3) hasta orta prognozlu, 20 (%18,9) hasta kötü prognozlu olarak değerlendirildi. B12/CRP Skoruna göre 61 (%57,5) hasta iyi prognoz, 29 (%27,4) hasta orta prognoz, 16 (%15,1) hasta ise kötü prognozlu olarak değerlendirildi. Glasgow Prognostik Skorunun ölüm ve sağkalım ile ilişkisine bakıldığında istatistiksel olarak anlamlı fark saptandı (p=0,036). Sonuç: Bu çalışma Glasgow Prognostik Skorunun AML tanısı alan hastalarda prognozu öngörmede kullanılabilecek bir skorlama olduğunu göstermektedir. BCI skorunun aynı hasta grubunda anlamlılığının kısıtlı olduğu, prognozu öngörmedeki etkisi için daha geniş örneklem gruplarına ve çok merkezli çalışmalara ihtiyaç vardır. PNI skoru incelenen grupta anlamsız bulunmuştur.

Aim: In this study, we aimed to compare the performance of the Prognostic Nutritional Index (PNI), B12/CRP Index (BCI), and Glasgow Prognostic Score (GPS) in predicting mortality, intensive care unit requirement, and overall survival in patients with acute myeloid leukemia (AML). Additionally, we sought to investigate whether these scores serve as independent prognostic factors beyond the ELN 2022 risk classification and conventional prognostic markers used in AML. Methods: In this retrospective study, patients who were followed up at the Hematology Clinic of Konya City Hospital between 2020 and 2025 were included, and the study was conducted through review of patient records. Among the patients admitted to the Hematology Clinic of Konya City Hospital, those aged 18 years and older who were diagnosed with acute myeloid leukemia, had complete diagnostic tests, received treatment, and were followed up after treatment constituted the study population. Consecutive patients with acute myeloid leukemia who met the inclusion criteria were enrolled in the study. Demographic data, clinical information, imaging studies, and physical examination findings of all patients included in the study were reviewed at presentation. In addition to the indices calculated from patient measurements, their prognoses during the treatment process were also evaluated. Results: Of the 106 patients included in our study, 57 (53.8%) were female and 49 (46.2%) were male, and no statistically significant association was found between gender and mortality/survival status (p=0.116). The median age at diagnosis was 53.5 years, ranging from 18 to 85 years, and age was found to be significantly associated with mortality/survival status (p=0.044). According to the distribution of patients with infection at initial hospital admission, 20 (37.0%) of the survivors and 33 (63.5%) of the deceased patients had documented infection, and the difference between the groups was statistically significant (p=0.007). Regarding pneumonia detected at initial admission, 4 (7.4%) of the surviving patients and 17 (32.7%) of the deceased patients had pneumonia, with a statistically significant difference observed between the groups (p=0.001). A statistically significant association was also found between first-month treatment response and mortality/survival status (p<0.001). When the prognostic scores calculated from laboratory findings obtained at initial admission were evaluated, 43 (40.6%) patients were classified as having unfavorable prognosis and 63 (59.4%) as having favorable prognosis according to the Prognostic Nutritional Index (PNI). Based on the Glasgow Prognostic Score (GPS), 39 (36.8%) patients were classified as having favorable prognosis, 47 (44.3%) as intermediate prognosis, and 20 (18.9%) as unfavorable prognosis. According to the B12/CRP score, 61 (57.5%) patients were classified as having favorable prognosis, 29 (27.4%) as intermediate prognosis, and 16 (15.1%) as unfavorable prognosis. Evaluation of the relationship between the Glasgow Prognostic Score and mortality/survival demonstrated a statistically significant association (p=0.036). Conclusion: This study demonstrates that the Glasgow Prognostic Score may be used as a prognostic scoring system for predicting prognosis in patients diagnosed with acute myeloid leukemia. In the same patient group, the prognostic significance of the B12/CRP Index (BCI) was found to be limited, and larger sample sizes as well as multicenter studies are needed to better evaluate its role in predicting prognosis. The Prognostic Nutritional Index was not found to be statistically significant in the study population.
