Özet
ÖZET Renovasküler hipertansiyon şüphesinde kaptoprilli renosintigrafi sıklıkla başvurulan tanı yöntemidir. Bu çalışmanın amacı; renovasküler hipertansiyon düşünülen hastalarda kaptopril öncesi ve kaptopril sonrası, Tc99m-DTPA renosintigrafisindeki sayısal değerler, kaptopril öncesi ve kaptopril sonrası Tc99m-DMSA renosintigrafisindeki sayısal değerler ve kaptopril öncesi ve kaptopril sonrası plazma renin aktivitesindeki sayısal değerler karşılaştırılarak diagnostik önemlerini araştırmaktır. Çalışmaya klinik olarak renovasküler hipertansiyondan şüphe edilen yirmi hasta dahil edildi. Bütün hastalara kaptopril öncesi ve kaptopril sonrası rutin protokolümüz uygulanarak Tc99m- DTPA renosintigrafisinde split renal fonksiyon, pik aktivite zamanı, T 1/2 ye glomerüler filtrasyon hızı, Tc99m-DMSA renosintigrafisinde her iki böbreğin uptake yüzdeleri ve RIA yöntemiyle plazma renin aktivite değerleri çalışıldı. Elde edilen sayısal değerlere istatistiksel olarak Wilcoxon signed Ranks testi ile değerlendirildi. Prekaptopril ve post kaptopril Tc99m-DTPA renosintigrafisinde split renal fonksiyon, TTP ve GFR istatiksel olarak anlamsız bulunmuştur(sırasıyla; p=0,984, p=0,187, p=0,264). Prekaptopril ve postkaptopril T 1/2 değerleri ise istatiksel olarak anlamlı bulunmuştur(p=0,005). Pre ve postkaptopril Tc99m-DMSA renosintigrafisinde uptake değerleri istatiksel olarak anlamsız bulunmuştur(p=0,809). Kaptopril öncesi ve sonrası PRA değerleri de istatiksel olarak anlamlı bulunmuştur(p=0,004). Sonuç olarak Tc99m-DTPA renosintigrafisiyle değerlendirilen Tl/2dk. parametresinin ve PRA ölçümünün renovasküler hipertansiyon şüphesi olan hastalarda kullanılabilen kolay, non-invaziv, ucuz bir tanı yöntemleri olduğu sonucuna varılmıştır.

ABSTRACT Scintigraphy with captopryl is used frequently in suspect of renovascular hypertension The aim of this study is; to investigate and compare the diagnostik importance of numeral values in Tc99m-DTPA and Tc99m-DMSA renoscintography and plazma renin aktivity before and after captopryl. 20 patiens, who were suspected of having renavascular hypertension, included of this study. Split renal function in Tc99m- DTPA scintography, peak activation time, T lA and glomerular filtrasyon rate; uptake percents in Tc99m-DMSA renoscintography and plazma renin activity values with RIA method were applied before and after captopryl to the patients. The numeral values where evaluated statistically with Wilcoxon signed ranks test. Precaptopryl and postcaptopryl Tc99m-DTPA split renal function, TTP and GFR were found unimportant, statistically(p=0.984, p=0.187, p=0,264 with order). Precaptopryl and postcaptopryl Tl/2 values were found statistically important(p=0.005). Precaptopryl and postcaptopryl Tc99m-DMSA uptake values were found unimportant(p=0,809). Pre and postcaptopryl PRA values were statistically important(p=O.004). As a result we can say that Tl/2 parameter which was evaluated with Tc99m-DTPA renoscintography and PRA measurement are easy, non-invasive and cheap diagnostic methods which can be used in patients with suspect of renovascular hypertension.
