Özet

İki güncel renk anahtarının renk seçimindeki üstünlüklerinin spektrofotometrik analiz ile klinik olarak değerlendirilmesi. ?stanbul Üniversitesi Sağlık Bilimleri Enstitüsü, Protetik Dis Tedavisi ABD. Doktora Tezi. ?stanbul. Bu çalısmanın amacı; Value sistemine göre sistematik düzenlenen Vita 3D Master renk anahtarı ve Hue sistemine göre dizayn edilen geleneksel Vita Classical renk anahtarı ile seçilen renklerle islenen restorasyonların renk üstünlüklerinin arastırılmasıdır. Bu amaçla; üst sağ/sol santral disinde kuron restorasyonuna ihtiyaç duyan 66 kisi 2 gruba bölündü. Her hasta için 2 adet restorasyon yapıldı. ?lk grupta, renk seçim ve ölçümleri Vita Classical ve Vita Easyshade Spektrofotometre (SFM) ile ikinci grupta ise; Vita 3D Master ve Vita Easyshade SFM ile yapıldı. Böylece; her grup için toplam 66 In-ceram restorasyonu için renk kaydedildi. Restorasyonlar; aynı laboratuarda aynı teknisyen tarafından islendi. Renk seçimi Vita 3D Master ve Vita Classical renk anahtarı ile tamamlanan restorasyonlar, SFM ile ölçülerek CIE L*a*b* koordinat değerleri belirlendi. Her grup için renk farklılığı hesaplandı. Tamamlanan restorasyonlar; her 2 grupta da hasta ağzına yerlestirilerek; 2 dis hekimi ve bir dis teknisyeni tarafından renk uyumları görsel olarak değerlendirildi. Bu algısal değerlendirme non-parametrik bir uyum skalasına 1-10 arası değerler verilerek islendi ?statistiksel değerlendirmede; Mann Whitney u, Student t* ve Kappa katsayıları kullanıldı. Her iki renk anahtarı arasındaki ?E değeri istatistiksel olarak anlamlı bulunmustur (P<0,05). 3D Master renk anahtarı ile yapılan renk seçimleri Vita Classical' a oranla istatistiksel olarak daha basarılı bulunmustur. Gözlem grubu ile yapılan değerlendirmeler sonucunda en basarılı grup 3D Master grubu ile renk alınarak laboratuarda SFM ile islenen kuronlarda elde edilmistir. Bu çalısmanın sonucunda; renk seçiminde daha gelismis bir sistem olan 3D Master renk anahtarı kullanılarak, laboratuarda da Vita Easyshade SFM kontrolünde islenen restorasyonlarla daha basarılı estetik elde edilebileceği görülmüstür.

Clinical evaluation of two current shade guide systems of shade matching superiority by spectrophotometric analysis. Istanbul University, Institute of Health Science, Department of Prosthetic Dentistry, pHD Thesis. ?stanbul. The aim of this study was to investigate the shade matching superiority of a maxillary single right/left incisor crown when the restorations were fabricated according to data from whether a systematically arranged shade guide system with Value based Vita 3D Master or a conventional shade guide with Hue based of Vita Classical. According to this aim; 66 patient with a maxillary single right/left incisor needing a restoration divided randomly into 2 groups. In each group 2 restorations were made for each patient. In the first group; shade selections and measurements were made by Vita Classical and Vita Easyshade Spectrophotometer (SFM)and in the second group; made by Vita 3D Master and Vita Easyshade SFM. Consequently; 66 In-Ceram alumina crowns were fabricate for each group by the same technician at the same laboratory. The completed In-Ceram alumina crown restorations were fabricated according to data from Vita Classical and Vita 3D Master shade guide were measured with the SFM to determine the CIE L*a*b* coordinates. The total color difference was calculated for each group. After this measurements, completed In-Ceram crowns for each group were positioned in each patient?s mouth and evaluated for shade match by 2 prosthodontists and a technician. The perceptual evaluation was graded nonparametrically on a `Matching scale' of 1-10. All results are statistically analyzed and Mann Whitney u tests t*, Kappa coefficients were used. A significant differences was found between the two shade guides. (P<0,05).The restorations shade match that selected by 3D Master were found more succesful than Vita Classical shades. Fabricated restorations using the 3D Master data and finished with in the control of SFM in labratory had a significantly higher mean matching score by the perceptual evaluation. Within the limitations of this study, shade matching with 3D Master shade guide; a newly developed system, and finishing the restorations with in the control of SFM in labratory would achive more succesful esthetics
