Özet

ÖZET Bu çalışmada klinik ataçman seviyesini ve kemik rejenerasyonunu arttırmak amacıyla koronale yerleştirilmiş flep tekniği incelenmiştir. Çalışma grubunu sistemik hastalığı olmayan mandibuler molar bukkal sınıf II furkasyon defektine sahip 31 yetişkin periodontitisli birey oluşturmuştur. Deney grubunda 20 bireye koronale yerleştirilmiş flep tekniği uygulanarak toplam 23 defekt tedavi edilmiştir. Kontrol grubunda ise 11 bireyde toplam 16 defekte geleneksel flep tekniği uygulanmıştır. Hastaların cerrahi sonrası iyileşmesi, plak kontrolü ve idame bakımları 6 ay süresince takip edilmiştir. 6. ayın sonunda tüm klinik parametreler tekrar değerlendirilmiş ve kemik defekti ölçümleri için reentry operasyonu gerçekleştirilmiştir. Her iki grupta da tedavi edilen furkasyon defektlerinde kemik ile tamamen dolum gözlenmemiştir. Bu çalışmanın sonuçları yumuşak ve sert doku ölçümleri bakımından deney ve kontrol grupları arasında istatistiksel olarak önemli değişiklikler ortaya koymuştur. Cerrahi sonrası 6. ayda deney grubunda kontrol grubuna kıyasla gingival çekilme görülmemiş ve mandibuler bukkal sınıf II furkasyon defektlerinde horizontal boyuttaki ölçümlerde ve ataçman seviyesinde anlamlı kazançlar elde edilmiştir. 59

SUMMARY The present study was conducted in order to clinically analyze the efficacy of coronally positioned flaps, performed to enhance the gain in clinical attachment levels and osseous regeneration. Thirty-one patients with no known systemic disease and were diagnosed as having adult periodontitis and additionally presenting buccal class II furcation involvements in mandibular molars, comprised the study group. In the experimental group a total of 23 defects in 20 subjects were treated by use of coronally positioned flap procedure. The control group consisted of 11 subjects and in this group a total of 16 defects were treated by conventional flap procedure. Patients were checked for postoperative healing, plague control and maintenance of oral hygiene attempts for a period of 6 months. At the end of six months all of the clinical parameters were again recorded and reentry procedures were performed to measure the bony defects. In each group, there was no evidence of a complete fill of the treated furcation defects by bone tissue. However, the findings of the present study demonstrated that there were statistically significant differences between experimental and control molars when recorded soft and hard tissue measurements were concerned. Six months following surgery, contrary to the control teeth the experimental, molars revealed no gingival recession and furthermore presented significant gain in attachment levels as well as in horizontal measurements of the mandibular buccal class II furcation defects. 60
