Özet
Plastik Cerrahide sık uygulanan ameliyatlardan biri olan rinoplastide tipprojeksiyonunun ve rotasyonunun kontrolu önemli bir faktördür. Yara iyileşmesinin dinamikbir rol oynadığı rinoplastide projeksiyon ve rotasyon sorunları gelişebilir. Bu çalışmadaamacımız rinoplastide projeksiyon ve rotasyon kontrolu amacıyla kullanılan iki tekniğin geçve erken dönemlerindeki sonuçlarını incelemektir. Kolumellar strut greft kullanımı ve septalekstensiyon greft kullanmıyla ilgili literatürde çok sayıda yayın olmasına rağmen bu ikitekniğin karşılatırıldığı bir çalışmaya rastlanılmadı. Rinoplastide amaç kalıcı sonuçlar eldeetmek olduğundan çalışmamızın bu konuda literatüre katkısı olacağını düşünmekteyiz.GEREÇ ve YÖNTEMÇalışmamızda geriye dönük olarak 36 hastada ameliyat sonrası erken ve geç dönemdeprojeksiyon ve rotasyonun ne yönde değiştiğini araştırdık. Grup 1 açık teknikle yapılan vekolumellar strut - septokolumellar sütür uygulanan 18 hastadan oluşurken, grup 2 açıkteknikle yapılan ve septal ekstensiyon greft uygulanan 18 hastadan oluşturuldu. Hastalarınhepsi aynı cerrah (Dr. Erhan Eryılmaz) tarafından ameliyat edilmişti. Değerlendirmede burunprojeksiyonunun burun uzunluğuna oranına ve nazolabial açıya bakıldı. Değerlendirmeleryapılırken hastalara ait erken (4-8. aylar) ve geç (16-24. aylar) dönemde çekilmiş profilfotoğraflarından yararlanıldı. Fotoğraflar üzerinde burun projeksiyonunun burun uzunluğunaoranı ve nzaolabial açı değerlendirildi Ölçümler `'Rhinobase'' adlı software programı ileyapıldı. Elde elden veriler SPSS 17 programı yardımı ile analiz edildi.BULGULARHer iki gruba ait ameliyat öncesi yapılan ölçümlerde iki grup arasında anlamlı farksaptanmadı. Grup 2 (septal ekstensiyon greft kullanılan) olgulardaki geç dönem projeksiyonve rotasyon kaybının Grup 1'e(kolumellar strut greft kullanılan) göre anlamlı şekilde daha azolduğu saptandı.SONUÇBu sonuçlar doğrultusunda ameliyat sırasında elde edilen projeksiyon ve rotasyondeğerlerinin septal ekstensiyon greft kullanılan olgularda geç dönemde daha kalıcı olduğusaptandı.

BACKROUNDRhinoplasty is one of the most common operations in Plastic Surgery. It?s veryimportant to control tip projection and rotation after rhinoplasty. Loss of projection androtation after surgery is a common problem. Numerous approaches and techniques have beendecribed to achieve a desired result in tip surgery, but there are a few studies that define oranalyze the changes that occur after surgery. In this study ; we compared two methods theseare used to control projection and rotation. These methods are use of columellar strut graftand use of septal extension graft. We expect this study will be beneficial to the literature.METHODSIn this study, we analyzed 36 patients who underwent primary rhinoplastyretrospectively. All the patient were operated by the same surgeon (Dr.Erhan Eryılmaz,MD).Kolumellar strut graft and septocolumellar suture were used in group 1 and septal extensiongraft was used in group 2. There were 18 patient in each goup. Analysis was based on datacollected from preoperative, 4.-8. months early postoperative and 16.-24. months latepostoperavtive lateral views. The measurements were obtained using the parametersprojection-nose length ratio according to Byrd-Hobar and nazolabial angle on standardizedphotographs with the software program `?Rhinobase??. The values were statistically analyzedwith SPSS 17 program.RESULTSThere were no statistically significant difference between two groups by thepreoperative measurements. When we compared the late views with the early views, therewere statistically significant loss of projection and rotation in Group 1.CONCLUSIONSWith these results we determined that the use of septal extension graft is more usefulthan the use of columellar strut graft to obtain a permanent result in long time.
