Özet
Amaç: Bu çalışmada seramik-on-metal total kalça protezi ile tedavi edilen primer ve sekonder dejeneratif kalça osteoartritli olguların kısa dönem fonksiyonel sonuçları değerlendirilmiştir.Gereç ve Yöntem: Ankara Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Kliniği'nde, 2007-2009 yılları arasında, seramik-on-metal total kalça protezi uygulanan ve düzenli takipleri yapılabilen 42 hastanın 44 kalçası çalışmamızın konusu olmuştur. Olguların 23'ü kadın (%54.8), 19'u erkek (%45.2) olup yaş dağılımı 24 ile 55 arasında ve ortalama yaş 43.4 olarak hesaplanmıştır. Bütün olgularda posterior yaklaşım tercih edilmiştir. Postoperatif 1.5 , 3 , 6 , 12. aylar ve daha sonra yılda bir kontrole çağırılan olguların sonuçları klinik olarak Harris kalça skoru ve D'Aubigne-Postel kalça skorlama sistemine göre değerlendirildi.Elde edilen skorlar ile cinsiyet, tanı, vücut kitle indeksi ve kullanılan implant büyüklükleri arasındaki ilişki araştırıldı.Ayrıca hastaların postoperatif dönemdeki röntgenogramlarında kemik implant arası ilişki Gruen'in 7 zonu ve Delee-Charnley'in 3 zonu dikkate alınarak değerlendirildi.Bulgular: Hastalar 18-49 (ortalama 34.8 ) ay arası takip edilmiş, sonuçlar klinik olarak değerlendirilmiştir. Ameliyat öncesi ortalama 28.2 olan Harris skoru ameliyat sonrası 79.8 olarak bulundu. Buna göre olguların 29'unda (%69.5) mükemmel, 8'inde (%17.4) çok iyi, 5'in de (% 13.1) iyi sonuç elde edilmiştir.Ameliyat öncesi D'aubigne-Postel skoru ortalama 3.7 olan hastaların ameliyat sonrası skoru ortalama 11.8 olmuştur.Hastaların Harris ve D'Aubigne-Postel skorlarındaki artış miktarı istatistiksel olarak anlamlı bulunmuştur (p0.05). Olguların tamamının X-ray incelemelerinde femoral ve asetabular komponentlerin stabil olduğu tespit edilmiştir. Hastalarımızın hiçbirinde ameliyat sonrası dönemde derin ven trombozu ve ek komplikasyon görülmedi.Sonuç: Seramik-on-metal total kalça protezi, uygun endikasyon ve cerrahi teknikle yapıldığında, özellikle genç ve aktif hastalarda fonksiyonel eklem hareketlerini en üst düzeyde yapabilmesine imkan vermesi ve kısa dönem takiplerinde sağ kalım oranının yüksek olması nedeniyle günümüzde güvenle kullanılabilecek etkili bir materyaldir.

Purpose: In this study, it was evaluated the functional short-term results of Ceramic-on-metal total hip arthroplasty in cases primary and secondary degenerative hip osteoarthritis.Materials and Methods: Ankara University Faculty of Medicine at the Clinic of Orthopedics and Traumatology, 2007-2009 between the years, ceramic-on-metal total hip prosthesis that can be applied and made regular follow-up 44 hips of 42 patients has been the subject of our study. 23 (%54.8) of the cases were female and 19 (%45.2) were male, age distribution was between 24 and 55 years, mean age was 43.4 years. It had been performed the posterior approach in all cases. The results of the cases who were postoperatively called for 1.5 , 3 , 6 , 12th months and afterwards yearly controls. The patients were clinically evaluated according to Harris Hip Scale and D?Aubigne-Postel Functional Hip Scale .Relationship that between the scores and gender,diagnosis, body mass index ,size of implants that have been implanted was researched.Bone-implant relation was evaluated by Gruen?s 7 zones and Delee-Charnley?s 3 zones on X-rays.Results: Patients 18-49 (mean 34.8) months have been followed, the results were evaluated clinically. While preoperative Harris score was mean 28.2 then this score was found to 79.8 in final controls. In this opinion, it was reached clinically exellent results in 29 (%69.5), very good results in 8 (%17.4), good results in 5 (%13.1) of the cases.Patients? preoperative D?Aubigne-Postel score was meanly 3.7 and postoperative score was meanly 11.8. Scores? amount of increasing was significant statistically (p0.05).All femoral and acetabular components were established as stabile. None of the patients with deep vein thrombosis and any complication were not seen.Conclusion: Ceramic-on-metal total hip prosthesis when perform at appropriate indications and surgical technique particularly in young and active patients with functional joint motion made at the highest level allows short-term follow-up and survival rate is high because today could be used safely is an effective material.
