Özet

Bu çalışmanın amacı, ÖÇB yetmezliği tanısı konulan, otojen hamstring tendon grefti ve iki farklı femoral tespit yöntemi kullanılarak ÖÇB rekonstrüksiyonu yapılan 83 hastanın fonksiyonel sonuçlarının değerlendirilmesidir. 43 hasta transfiksasyon, 40 hasta endobutton tekniği kullanılarak ameliyat edildi. Hastaların 72' si erkek (%86.7), 11' i kadındı (%13.3). Hastaların 48'i sağ (%57.8), 35' i sol dizdi (%42.2). Hastaların ortalama yaşı 27.2 idi (16-46). Hastaların ameliyat öncesi öyküleri alınıp, Ön çekmece, Lachman, Pivot Shift testleri ve genel muayeneleri yapılarak, röntgen grafileri ile manyetik resonans görüntüleri elde edildi. Hastaların fonksiyonel değerlendirmeleri ameliyat öncesinde ve en son kontrolde Lysholm skoru ve İnternasyonel Diz Dokumantasyon Komitesi Değerlendirme Formuna (IKDC) göre yapıldı. Ameliyat öncesi 57 olan (46-74) Lysholm skor ortalaması ameliyat sonrası transfiksasyon uygulanan grupta 93.81, endobutton uygulanan grupta 94.48 olarak bulundu. Hastaların ameliyat öncesi ve sonrası Lysholm değerleri arasında anlamlı farklılık bulundu (p˂0,001). İki grup arasında Lysholm değerleri açısından anlamlı farklılık saptanmadı (p>0.05). IKDC değerlendirme sistemine göre ameliyat öncesi 6 hasta grup B, 42 hasta grup C, 35 hasta grup D iken ameliyat sonrası 58 hasta grup A, 25 hasta grup B olarak değerlendirildi. Hastaların ameliyat öncesi ve sonrası IKDC değerleri arasında anlamlı farklılık bulundu (p0.05) Sonuç olarak doğru hasta seçimi ile birlikte çift katlı hamstring tendon grefti kullanılarak yapılan artroskopik ÖÇBR' da endobutton ve transfiksasyon cerrahi tekniklerinin doğru uygulanması, erken dönemde kontrollü rehabilitasyon programı uygulanması ve sık poliklinik kontrolleri ile yüz güldürücü sonuçlar elde edilebilmektedir.

The aim of this study is the evaluation of the outcomes of 83 patients. Diagnosed as anterior cruciate ligament insufficiency which reconstructed by using autogenous hamstring tendon graft and two different fixation method. 43 patients were operated by transfixation method, 40 patients were operated by endobutton method. 40 (48,2%) of the patients operated by transfixation method were male, 3 (3,6%) of them were female. Transfixation method was applied to 31 patient's rigt knee(37,3%) , 12 patient's left knee(14,5%).Endobutton method was applied to 17 patient's right knee, 23 patient's left knee. At time of the surgery, minimal age was 27.2 (16- 46). Before the surgery, the patient's history was taken, local and general examination was performed, X-Ray and MR images were achieved Functional evaluation was performed before the surgery and at last check up was done ia lysholm score and international knee documantation comiteeevaluation form Lyshom score that is 57 (46-74) before surgery was found 93.81 in transfixation group, 94.48 in endobutton group. There was significant difference in Lysholm score measured before and after surgery. There was no significant difference between two groups. (p>0.05) According to IKDC evaluation system, 6 patients were group B, 42 patients were group C and 35 patients were group d before surgery, 58 patient were group A, 25 patients were group B after the surgery. There was significant difference in IKDC values measured before and after surgery. There was no significant difference between two groups. (p>0.05) As a conclusion, promising results can be obtained with proper patient selection, proper application of the arthroscopic ACL reconstruction surgery with double layered hamstring tendon graft, application of controlled rehabilitation proggramme in the early period and frequent utpatient controls.
