Özet

Amaç: Bu çalışma serratus anterior kasını koruyucu torakotomi (SKT) geçiren olguların, preoperatif ve postoperatif dönemde omuz normal eklem hareket açıklığı, ağrı şiddeti, göğüs kafesi hareketliliği ve solunum kapasitesini değerlendirmek ve ilk değerlerine ne kadar yaklaştığını belirlemek amacıyla planlandı. Gereç-Yöntem: Çalışmaya yaş ortalamaları 52.07±14.46 olan; 25'i erkek, 18'i kadın 43 olgu dahil edildi. SKT geçiren olgularda, preoperatif dönem ve postoperatif 1., 2., 3. ve 5. günlerde; Vizüel Analog Skala (VAS) ile ağrı değerlendirmesi, gonyometre ile omuz normal eklem hareket açıklığı ölçümü, mezura ile göğüs çevre ölçümü ve spirobank spirometre ile solunum fonksiyon testi yapıldı. Bulgular: Ölçümler sonucunda preoperatif değerlerle karşılaştırıldığında postoperatif 1. günde, omuz fleksiyon ve abduksiyon açıları, VAS değerleri, göğüs çevre ölçüm değerleri ve solunum fonksiyon testi parametrelerinde azalma yönünde istatistiksel olarak anlamlı fark gözlendi (p<0,05). Preoperatif (preop) değerlerle karşılaştırıldığında postoperatif (postop) 1., 2., 3. ve 5. günde, omuz internal ve eksternal rotasyon dereceleri ölçümünde istatistiksel olarak anlamlı fark saptanmadı. Postop 5. gün omuz fleksiyon ve abduksiyon açısı ayrıca ağrı derecesi preop değerlere çok yaklaşmasına rağmen; göğüs çevre ölçümü (GÇÖ) ve solunum fonksiyon testi (SFT) sonuçları preop değerlere çok uzak kaldı. Postop 5 gün içerisinde en iyi düzelme gösteren ise abduksiyon açısı oldu. Sonuç: Torakotomi kesisi geçiren olgularda, şiddetli ağrıdan dolayı kooperasyonun zor olması ve olguların yeterince göğüs hareketine izin vermemesi göğüs çevre ölçümünü zorlaştırmaktadır ve bu ölçümü başarısız kılmaktadır. Pulmoner kapasitenin daha kısa sürede toparlanması için çok kapsamlı ve yoğun solunum fizyoterapi programı uygulanması gerekmektedir. Fonksiyonel kapasitenin objektif olarak ölçülmesi için olguların uzun dönem takibi gerekmektedir.

Objectives: This study assesses the shoulder range of motion, pain level, chest wall mobility and respiratory capacity of patients who, in the postoperative and preoperative terms, underwent muscle-sparing thoracotomy for their serratus anterior muscles (SST). It compares these findings to the patients' initial states. Material and Method: This study involved 25 male and 18 female participants (43 in total) who underwent SST surgery, with an average age of 52.07±14.46. For all, an assessment of pain via visual analogue scale (VAS), measurement of shoulder range of motion by goniometer, measurement of chest girth by measuring tape, and respiratory function testing by spirometer were carried out. Results: After measurements were taken, the results were compared with the preoperative rates. A significant statistical difference (p<0.05) was observed one day post-operation, in terms of range of shoulder flexion and abductıon, VAS, chest girth measurement rates, and respiration function parameters. When compared with preoperative rates, the significant differences were not observed in internal and external shoulder rotation degree evaluation. At five days postoperative, patients' range of shoulder flexion and abduction and also pain levels approached preoperative rates, while chest girth measurements and respiratory function parameters did not. During the postoperative five days, the best improvement was found by abduction rates. Conclusion: In cases of thoracotomy, difficulty in cooperation due to intense pain and insufficient chest motion cause failures in the measurement of chest girth. Intense and extensive respiratory physical therapy must be carried out to recover functional capacity in a shorter time. For the objective evaluation of functional capacity, cases need to be followed over the long term.
