Özet
Giriş:Hekimlik uygulamalarının önemli bir bölümünü oluşturan konsültasyon sisteminin düzenli işlemesi özellikle iş yükünün çok fazla olduğu acil servislerde çok büyük önem arz etmektedir. Ülkemizde konsültasyon prosedürlerinin hangi yasal çerçevelerde gerçekleşeceğine dair kurallar ve iç hizmet yönergeleri olmasına rağmen hekimlerin konsültasyon görüşmelerinin içermesi gereken prensiplere veya standardize konsültasyon uygulamalarına dair herhangi bir çalışmaya rastlanmadı. Gelişmekte olan ülkelerin bir kaçında son yıllarda uygulanmakta olan konsültasyon modellerinden olan Amerika'da yaygın kullanılan Kessler tarafından geliştirilmiş 5-C konsültasyon modelinin hastanemiz Acil Tıp Kliniği'ndeki uzman ve asistan hekimlerinde etkinlik ve uygulanabilirliğini değerlendirmeyi amaçladık. Gereç ve Yöntem: Prospektif, tek kör olarak tasarlanmış deneysel nitelikteki bu araştırmaya Fatih Sultan Mehmet Eğitim ve Araştırma Hastanesi Acil Tıp Kliniği'nde çalışan acil tıp asistan ve uzman hekimleri dahil edildi. Katılımcılara 5-C konsültasyon modeli eğitimi verilmesinden hem öncesi hem de sonrasında, hekimlerin telefonla konsültasyon görüşmeleri esnasında 'Hekimin Konsültasyon Değerlendirmesinde 5-C Modeli Kontrol Listesi', 'Hekimin Konsültasyon Değerlendirmesi için Global Oylama Skorları (GOS) puanlama sistemi' ve 'Konsültasyonda Kendini Değerlendirme Anketi' uygulanarak değerlendirmeler yapıldı. Bulgular: Çalışmaya toplam 33 gönüllü hekim dâhil edildi. 5-C konsültasyon modeli eğitimi sonrası 'Konsültasyonda Kendini Değerlendirme GOS skoru' (t:-4,474 sig: 0,00<0,05) ve 'Hekimin Konsültasyon Değerlendirmesinde Performans Genel GOS Skorun'da (t:-5,490 ve sig: 0,00<0,05) anlamlı değişiklikler saptandı. GOS değerleri olumlu yönde en çok artan grubun 12 aydan az deneyimli hekimler olduğu görüldü. 5-C modelinin hasta hakkında tüm bilgileri içerdiği ve konsültan hekimin beklentilerini karşıladığı, hasta sonlanımında hız kazandırdığı, hasta bakım kalitesini ve hastanın güvenliğini arttığı, acil servisteki her hasta için uygulanabilir ve yeterli bir model olarak algılandığı saptandı. Tüm hekimlerin eğitim sonrası 5-C modeli prensiplerini oluşturan 7 ölçeği daha etkili kullandıkları ve performans etkinliklerini olumlu yönde artırdıkları izlendi (sig<0.05). Sonuç: 5-C konsültasyon modelinin hastanemiz Acil Tıp Kliniğinde etkin ve uygulanabilir bir konsültasyon modeli olduğu sonucuna varılmıştır.

Introduction and Purpose: The regular functioning of the consultation system, which constitutes an important part of the medical practice, is of great importance especially in busy emergency services. In our country, there were rules about the legal framework of consultation procedures, but no study was conducted on the principles or standardized consultation practices that physicians should have in consultation. We aimed to evaluate the effectiveness and applicability of the 5- C consultation model developed by Kessler, which is widely used in recent years in the developing countries, in the emergency medicine clinic of our hospital. Material and Methods:This prospective single blinded experimental study designed in the Emergency Medicine Clinic of Fatih Sultan Mehmet Training and Research Hospital and Emergency Medicine assistant and specialist physicians were included. The 5-C Model Checklist, the Global Rating Score (GRS) for the Consultation Evaluation of the Physician, and the Self- Assessment Questionnaire in Consultation were performed during the consultation of physicians both before and after the 5-C consultation model training. Results: A total of 33 volunteer physicians were included in the study. After the 5-C consultation model training, the 'Self-Assessment GOS score' at the consultation (t: -4,474 sig: 0,00<0,05) and the 'Performance General GOS score' at the physician's consultation evaluation (t: -5,490 and sig: 0.00 <0.05) significant changes were detected. The group with the most positive increase in GOS values was less than 12 months. It was determined that the 5-C model included all the information about the patient and met the expectations of the consultant physician, accelerated the patient outcome, improved the quality of the patient care and the safety of the patient, and was perceived as a feasible and adequate model for each patient in the emergency department. It was observed that all physicians used the 7 scale 5-C model principles more effectively and increased their performance activities positively (sig <0.05). Conclusion: It was concluded that the 5-C consultation model was an effective and applicable consultation model in the emergency medicine clinic of our hospital.
