Özet

Atatürk Üniversitesi Txp Fakültesi Araştırma Hastahanesi Genel Cerrahi Anabilim Dalı'nda, 1 Temmuz 1986 ile 1 Ocak 1987 tarihleri arasındaki 6 aylık süre içerisinde akut apandisit ön tanı sı ile ameliyat edilen 60 hastanın, semptomların başlangıcı ile ameliyat edildiği ana kadar geçen sürenin akut apandisit'in klinik seyir ve histo-patolojik durumu ile olan ilişkisi araştırıldı. Bu gruptaki hastaların yaş ortalaması 20,7'olup, kadın/ - erkek oranı 2/3'dür. Ameliyat edilen hastaların 14 (% 23,3) 'ünde daha önceden akut apandisit' i düşündüren benzer semptomlar tesblt edildi. Ameliyat öncesi geçirilen süre uzadıkça, appendlks'deki inflamasyonun ilerlediği (akut kataral apandisit 'il hastalar 21,4* saat, akut süpüratif apandisit'li hastalar 25,6*saat,akut gangrenöz apandisit 'H hastalar ise 30,5'saat gecikme ile müracaat ettiler)f beyaz küre sayısının yükseldiği, ameliyat sonrası komplikasyonlarının arttığx,hastahanede yatış süresinin uzadığı, kadınlarda menstrual siklus değişikliklerinin appendiksteki inflamasyonun tipine etkili olduğu, gebe hastalarda akut apandisit seyrinin gebe olmayanlara benzer olduğu tesbit edildi. Akut apandislt'de erken tanı ve erken cerrahi müdahalenin morbidite ve mortaliteyl azaltmak yönünden en etkili faktör olduğu tesbit edildi.

This study was performed by the department of general surgery at the medicine faculty of Atatürk University in a period of six months time between 1st, July 1986 and IOthttanuary 1987. At the same period of time sixty patients underwent to operation with a- cute appendicitis and the effect of time, between the duration of symtoms and operation, on the clinical progress and histopatholo- gical variations of acute appendicitis waB investigated. In this group the mean age of the patientswas 20,7 and the ra tio of women to the men was two over three. Similar symptoms of a cute appendicitis were established previously at the fourteen of the patients (23.3 per cent)* If the duration of time he£OTe, operation was longest, it is established that the inflammation of appendix progresses, (in this study the patients with catharal appedicitis were delayed 21.4 hours to apply to the hospital, the patients with suppurative ap pendicitis were delayed 25.6 hours and the patients with gangrenous appendicitis were delayed 30.5 hours) the number of leucocytes increases, postoperative complications develop and the duration of hospitalization is prolonged. It was also established that menst- ruel cyclus of women effected on the type of the inflamraationof acute appendicitis there was a similar clinical progress of appefldicitis between the pregnant and no n- pregnant. It is indicated that early diagnosis and attempt to operation are the most effectual to decrease the morbidity and mortality of acute appendicitis*
