Özet
Amaç: Bu prospektif çalışmanın amacı kanser ön tanılı olup vertikal kesi kullanılarak jinekolojik operasyon geçiren kadınlarda profilaktik olarak subkutan retansiyon sütürü atılmasının, yüzeyel doku ayrılması ve bunun göstergesi olan akıntı ve bu durumdan kaynaklanabilecek enfeksiyon ve yatış süresinde uzama gibi komplikasyonları önlemedeki etkinliğinin araştırılması ve yara yeri komplikasyonları için risk faktörlerinin belirlenmesidir. Gereç ve yöntem: Mayıs 2018 ve Ağustos 2019 tarihleri arasında Jinekolojik Onkoloji kliniğinde cilt kalınlığı 3 cm ve üzerinde olan, subkutan retansiyon sütürü atılan 95 ve kontrol grubu 113, toplam 208 hasta çalışmaya dahil edildi. Hastaların klinik ve cerrahi özellikleri retansiyon sütürü atılan ve atılmayan grupta karşılaştırıldı. Kültürde üreme olması, yatış süresinin uzaması, yara yeri ayrılması ile ilişkili faktörler tek değişkenli analizle, yara yeri ayrılması ile ilişkili faktörler çok değişkenli analizle ve yatış süresini uzatan faktörler çok değişkenli analizle değerlendirildi. Bulgular: Yapılan tek değişkenli analizde yaş, dahili hastalık varlığı, preoperatif hemoglobin, preoperatif albümin, kilo, BMI, preoperatif kan verilmesi, postoperatif kan verilmesi, retansiyon sütürü atılması ile yara yerinden akıntı olması ve yüzeyel yara ayrılması arasında ilişkili bulundu. Yapılan tek değişkenli analizde preoperatif albümin, kilo, BMI faktörleri ile akıntı veya enfeksiyona bağlı yatış süresinde uzama ile ilişkili bulundu. Aim: The aim of this prospective study was to determine the efficacy of prophylactic subcutaneous retention sutures in the prevention of superficial wound separation and discharge that is an indicator of wound separation, and of complications that would result from these conditions such as infection and prolongation of hospitalization and to determine the risk factors for wound complications in women with a preoperative diagnosis of cancer who underwent gynecological surgery using vertical incision. Materials and Methods: A total of 208 patients with a subcutaneous tissue of 3 cm or greater composed of 95 patients who had wound closure using subcutaneous retention sutures and 113 patients who had standard wound closure that were hospitalized in Gynecological Oncology Clinic between May 2018 and August 2019 were included in the study. The clinical and surgical characteristics of the patients were compared in the groups with and without retention sutures. Factors associated with positive wound culture, prolongation of hospitalization, superficial wound separation were evaluated by univariate analysis and factors associated with prolongation of hospitalization and superficial wound separation were evaluated by multivariate analysis. Results: In a univariate analysis, age, presence of comorbid diseases, preoperative hemoglobin value, preoperative albumin value, body weight, body mass index, preoperative blood transfusion, postoperative blood transfusion and presence of retention suture were associated with the risk of discharge and separation on wound site. In another univariate analysis, preoperative albumin value, body weight and body mass index were associated with the risk of prolongation of hospital stay.
