Özet

AMAÇ: Primer santral sinir sistemi lenfoması (PSSSL), santral sinir sisteminde gelişip burada sınırlı kalan, agresif gidişli, sıklıkla DBBHL morfolojisinde bir tümördür ve tüm SSS neoplazilerinin %2-3'ünü oluşturur. Sık relapslarla seyreden kötü prognoza sahiptir ve bu duruma kendisine özgü moleküler değişikliklerin neden olduğu düşünülmektedir. Kanser gelişimi ve ilerlemesinde immün sistem anahtar rol oynamaktadır ve kanser hücrelerinin bu sistemi alt edecek stratejiler geliştirebildiği bilinmektedir. Bu mekanizmalardan biri, PD-1/PD-L1 yolağıdır. PD-L1 ifade eden tümör hücreleri, T hücre yanıtını baskılayarak immün sistemden kaçış sağlarlar. Tanımlanan bu ilişki, PD-1/PD-L1 yolağının farklı kanser çeşitlerinde tedavi seçeneği olarak hedeflenmesine neden olmuştur. PSSSL'de literatürde çeşitli düzeylerde PD-1 ve PD-L1 ifadeleri bildirilmektedir. Bu tedavi seçeneğini araştırmayı amaçlayan yeni bir çalışma 4 nüks/refrakter primer santral sinir sistemi lenfoması hastasında anti-PD-1 tedavi ile tüm hastalarda tam ya da parsiyel klinik ve radyolojik yanıt ile, 13-17 ay arasında değişen progresyonsuz sağ kalım ortaya koymuştur. Çok merkezli bir faz 2 çalışma ile PSSSL hastalarını kapsayan tek merkezli bir çalışma da anti-PD-1 tedavi etkinliğini hali hazırda araştırmaktadır. Tedavi seçeneklerinin sınırlı, tedavi yan etkilerinin yüksek, kür oranının düşük olduğu bu lenfoma tipinde, immünoterapi hedefi olan belirteçlerin hastalık gelişimindeki rolü net olarak ortaya konmamıştır. Bu konuda yürütülmekte olan çalışmalar, sınırlı sayıda hasta grubunu içermekte ve konfirmasyona ihtiyaç duymaktadır. Yeni tedavi seçeneklerinin ve tedavide başarı için hastaların buna göre sınıflandırılmasının yerini araştıracağımız bu çalışmamızda Ankara Üniversitesi ve Hacettepe Üniversitesi Tıp Fakültesi Tıbbi Patoloji Anabilim dallarında PSSSL tanısı almış geniş bir hasta serisinin incelenerek, olguların retrospesktif klinik analizi, tümör dokusunda PD-1 ve PD-L1 ifadesinin analizi ile bunların moleküler karşılığının in-situ hibridizasyon yöntemi ile araştırılması ve histopatolojik, prognostik parametrelerle ilişkilendirilmesi amaçlanmıştır. Bu sayede kısa sürede ölümcül sonlanan bu hastalıkta yaşam süresinin arttırılmasında alternatif tedavilerin belirlenmesi ve tanı anında yapılması gereken güvenilir testlerin neler olması gerektiğinin de ortaya konması hedeflenmiştir. MATERYAL ve METOD: Ankara Üniversitesi Tıp Fakültesi Tıbbi Patoloji Anabilim Dalı ve Hacettepe Üniversitesi Tıp Fakültesi Tıbbi Patoloji ABD'nda PSSSL tanısı almış 57 vaka çalışmaya dahil edilmiştir. Kontrol grubu olarak santral sinir sistemi lenfomalarına benzer fenotipik özelliklere sahip 45 aktive immünfenotip DBBHL olgusu dahil edilmiştir. Tümör dokularında Beecher Instruments Manuel mikro-array cihazı ile seçilen alanlardan 1 mm çaplı, her hastadan 2 kor şeklinde temsil edilen örnekleme ile 3 adet doku mikro-array bloğu hazırlanmıştır. Hazırlanan bloklardan alınan kesitlerde PD-1, CD3, CD8, CD68, PD-L1 (siyah) + PAX5 (kırmızı) immünhistokimyasal, EBER in-situ hibridizasyon ve PD-L1/PD-L2 gen bölgesinde amplifikasyon ve translokasyon varlığı FISH yöntemi ile incelenmiştir. Olgulara ait klinik bilgiler hastane bilgi sistemleri aracılığı ile taranarak kayıt altına alınmıştır. İstatistiksel analizlerde SPSS ver.20 ile p<0.05 anlamlı kabul edilmiştir. SONUÇLAR: PSSSL olgularının, nodal DBBHL'ye kıyasla belirgin şekilde daha düşük CD3, CD8+ TIL ve CD68+ histiyosit içeren bir mikroçevreye sahip olduğu, bu sonucun preoperatif steroid tedavisinden bağımsız olduğu gösterilmiştir. PSSSL olgularının, sistemik DBBHL'ye kıyasla tümör mikroçevresinde sitotoksik T lenfositlerden daha zengin olduğu gösterilmiştir. PSSSL olgularında, mikroçevredeki hücresel yoğunluğun genel sağkalım ile ilişkisiz olduğu sonucuna varılmıştır. EBV pozitif olguların mikroçevreden daha yoğun olduğu, daha yüksek sitotoksik T lenfosit bulundurduğu gösterilmiştir. EBV pozitifliği, daha yüksek PD-L1 ifadesi ile ilişkili saptanmıştır. PSSSL'de tümörde %31, mikroçevrede %96; nodal DBBHL'de tümörde %24, mikroçevrede %100 oranında PD-L1 ifadesi izlenmiştir. PSSSL'de ve nodal DBBHL'de PD-1 ifadesi yüksek oranlarda saptanmıştır (%89 vs. %75). Nodal DBBHL'de saptanan daha yoğun mikroçevre ile korele olarak, mikroçevrede PD-L1 ifadesi daha yüksek bulunmuştur. Mikroçevrede hem PD-1 hem de PD-L1 ifadesi, lenfosit ve histiyosit sayıları ile koreledir. PD-1 ve PD-L1 ifadeleri sağkalım ile ilişkili bulunmamıştır. 44 PSSSL olgusunda %11 (5) polizomi, %2.5 (1) kopya sayısı artışı, %2.5 (1) amplifikasyon varlığı ve %4.5 (2) translokasyon; 25 aktive immünfenotip nodal DBBHL olgusunun ise %20'sinde (5) CNA, %4'ünde (1) amplifikasyon (1), %4'ünde polizomi (1) saptanmıştır. Genetik değişiklikler ile PD-L1 ifade düzeyi, mikroçevre yoğunluğu ve genel sağkalım arasında ilişki saptanmamıştır. TARTIŞMA: PSSSL olgularında, nodal DBBHL'ye kıyasla belirgin şekilde daha düşük yoğunlukta izlenen mikroçevrenin, santral sinir sisteminin sahip olduğu, çeşitli bariyerler ile korunan özel mikroanatomisi sayesinde geliştiği ve immün sistemden kaçışa sağladığı katkı ile benzer fenotipteki lenfomalara (sistemik DBBHL) kıyasla gösterdiği kötü prognozun nedenlerinden biri olduğu düşünülmüştür. Mikroçevrenin sağkalım ile ilişkisiz olmasının, SSS'nin özel mikroanatomiye sahip, immün sistemden kaçışı sağlayan koruyucu çevresi ve immün kontrol noktası moleküllerini yüksek oranda ifade etmesine bağlı olduğu düşünülmüştür. Bu sayede tümör, mikroçevrenin hücresel yoğunluğundan bağımsız olarak hayatta kalmayı başarmaktadır. Bu nedenle, mikroçevreyi immün baskılanmadan kurtaracak, immün kontrol noktası moleküllerine yönelik inhibitörlerin, hedeflenmiş tedavi seçenekleri arasında daha da önem kazanacağı düşünülmüştür. EBV pozitif olgularda izlenen yoğun mikroçevrenin, viral yüke bağlı salınan sitokinler ile ilişkili olduğu sonucuna varılmıştır. EBV pozitifliği, tümörde beklendiği şekilde daha yüksek PD-L1 ifadesi ile ilişkili bulunmuştur. Mikroçevrede PD-L1 ifadesinin, lenfosit ve histiyosit sayıları ile korele olması, immün hücrelerden salınan sitokinlere bağlı gelişirken, tümörde böyle bir korelasyon izlenmemesi, tümörde PD-L1 ifadesinin mikroçevreden bağımsız mekanizmalarla geliştiğini desteklemiştir. PD-L1 ifadesi direkt olarak kötü prognozla ilişki bulunmamış olsa da bu agresif seyirli lenfoma tipinde daha sık PD-L1 pozitifliği bulunması, etyopatogenez ve kötü prognoz açısından anlamlı gözükmektedir. Saptanan genetik değişikliklerin, PD-L1 ifadesi ile korelasyon göstermemesi, hedeflenmiş tedaviye uygun adayların seçiminde FISH yönteminin etkin olmadığını ortaya koymuştur. Sonuçta PSSSL, devam etmekte olan klinik çalışmalar ve literatürdeki veriler eşliğinde, PD-1/PD-L1 aksını hedefleyen tedavilere aday, agresif gidişli bir hastalıktır. İmmün kaçışı sağlayan moleküllerin ifadesi bu tümörlerde yüksek düzeylerde gözlenmektedir. Tanı esnasında bu belirteçlerin ifadesini belirlemek hastalara potansiyel tedavi seçeneği açısından yardımcı olacaktır.

Molecular development and classification of brain lymphomas AIM: Primary central nervous system lymphoma (PCNSL) is an aggressive tumour which develops and limited inside the central nervous system. It usually has the diffuse large B cell lymphoma morphology and constitutes 2-3% of all CNS neoplasias. It has a poor prognosis with frequent relapses and this is thought to be caused from its unique molecular changes. Immune system has a critical role for cancer development and progression and it is well known that cancer cells can produce mechanisms to escape immune system. PD-1/PD-L1 axis is one of these mechanisms. Tumor cells expressing PD-L1, supress T cell answer and escape from immune system. Defining this relationship provided a treatment alternative as targeting PD-1/PD-L1 for various cancer types. Different PD-1 and PD-L1 expression levels are reported in the literature for PCNSL. Complete or partial clinical and radiological response and 13-17 months of progression-free survival was achived in 4 relapsed/refractory PCNSL patients with anti-PD-1 treatment in a recent study. Currently there is a multicentered phase 2 and one-centered study is investigating the anti-PD-1 treatment efficacy in PCNSL patients. PCNSL has limited treatment modalities with high frequencies of adverse affects and low cure levels and the role of immunotherapy targeted markers in development of this disease couldn't presented exactly. The studies in this subject includes limited patient numbers and needs confirmation. We aimed to investigate markers of potential alternative therapies for PCNSL patients in this study. Patients diagnosed with PCNSL in Ankara University and Hacettepe University Pathology departments were retrospectively analysed for clinical parameters, PD-1/PD-L1 expression and molecular alterations regarding to PD-L1/PD-L2 gene locus. We aimed to identify prognostic and therapeutic markers in order to enhance survival in these aggressive disease. MATERIAL and METHOD: Total 57 patients diagnosed with PCNSL in Ankara University and Hacettepe University Pathology departments were included to the study. As the control group, 45 DLBCL (non-germinal center phenotype) were included. 3 micro-array tissue blocks were constructed with the help of Beecher Instruments Manuel micro-array device. 2 core per patient with 1 mm diameter were sampled. PD-1, CD3, CD8, CD68, PD-L1 (brown) + PAX5 (red) immunohistochemical stainings, EBER in-situ hybridization staining and amplification/translocation conditions in PD-L1/PD-L2 gene region with FISH method were performed on the sections from constructed tissue blocks. Clinical information regarding patients were recorded using the hospital information systems. SPSS ver.20 was used for statistical analysis and p<0.05 was accepted as significant. RESULTS: PCNSL cases had a microenvironment with lower CD3, CD8+ T lymphocytes and CD68+ histiocytes compared to nodal DLBCL, independent from preoperative steroid treatment effect. Microenvironment in PCNSL cases was enriched with cytotoxic T lymphocytes compared to nodal DLBCL. The density of microenvironment was not related to overall survival in PCNSL cases. EBV positive cases had denser microenvironment that is enriched with cytotoxic T lymphocytes compared to negative ones. EBV positivity was related to higher PD-L1 expression. We found tumoral PD-L1 expression 31% and microenvironment PD-L1 expression 96% in PCNSL; while it was 24% and 100% in nodal DLBCL, respectively. PD-1 expression was high both in PCNSL and nodal DLBCL cases (89% vs. 75%). PD-L1 expression was significantly higher in nodal DLBCL, compared to PCNSL, correlated with the higher density of microenvironment. PD-L1 expression in microenvironment was correlated with both lymphocyte and histiocyte numbers. Both PD-1 and PD-L1 expression were not related with overall survival. We detected 11% (5) polysomy, 2.5% (1) copy number alteration, 2.5% (1) amplification and 4.5% (2) translocation in 45 PCNSL cases; and 20% (5) copy number alteration, 4% (1) amplification, 4% (1) polysomy in 25 DLBCL lymphomas. Genetic alterations were not correlated with tissue PD-L1 expression levels, microenvironment density or overall survival. DISCUSSION: The lower density of microenvironment in PCNSL compared to DLBCL, was thought to be the result of special protected microanatomy of CNS. With this protection, PCNSL achieves immune evasion and has a worse prognosis compared to systemic DLBCL with similar phenotype. It is hypothesized that the escape of CNS from immune system with the help of immune checkpoint molecules is the reason of independence of survival from microenvironment. Thus, tumor survives without the effect of density of microenvironment. For this reason, it is thought that immune checkpoint inhibitors could remove suppresion of microenvironment and would benefit patients as a targeted therapy alternative. The higher density of microenvironment in EBV positive cases is thought to be the result of secreted cytokines for viral load. EBV positivity was found to be related with higher PD-L1 expression as expected. Positive correlation of microenvironment PD-L1 expression with lymphocytes and histiocytes is the result of cytokines released from immune cells. However there is no such a correlation with tumor PD-L1 expression which shows PD-L1 expression in tumor is activated with mechanisms independent from microenvironment. Although we couldn't find a direct relationship with PD-L1 expression and survival, detection of PD-L1 positivity more often in this aggressive lymphoma type supports this mechanism is important in both etiopathogenesis and prognosis. Detected genetic alterations were not correlated with tissue protein expression. FISH analysis is not the most effective test for detecting suitable candidates for targeted treatment. In conclusion, PCNSL is an aggressive disease which is suitable for PD-1/PD-L1 targeted therapies, in the light of current data and literature. The markers helping immune evasion are detected in high levels in PCNSL. To identify these biomarkers in the time of diagnosis would help the patients for alternative treatment modalities.
