Özet
Bu araştırma, intravezikal BCG immünoterapi uygulanan hastaların deneyimlerini incelemek amacıyla derinlemesine görüşme yöntemi kullanılarak kalitatif (niteliksel) olarak gerçekleştirilmiştir. Araştırma, Sağlık Bilimleri Üniversitesi İstanbul Gaziosmanpaşa Eğitim ve Araştırma Hastanesi'nin Üroloji servisinde ve polikliniklerinde çalışmaya katılmayı kabul eden, İntravezikal BCG İmmünoterapi uygulanan 15 hasta ile 5 Mart-10 Mayıs 2019 tarihleri arasında, derinlemesine görüşme yöntemi ile etik kurul onayı alınarak gerçekleştirilmiştir. Araştırmanın verileri sosyodemografik veri formu, yarı yapılandırılmış derinlemesine görüşme formu ve ses kayıt döküm formu ile toplanmıştır. Görüşmeler hastaların izinleri alınarak, ses kayıt cihazına kaydedilmiştir. Görüşme süresi ortalama 45-60 dakikadır. 15 hasta ile toplam 443 dakika 15 saniye ses kayıt cihazına kayıt yapılmıştır. Yapılan görüşmelerden 79 sayfa yazılı metin elde edilmiştir. Verilerin değerlendirilmesinde Maxqda 2020 programı kullanılarak betimsel analiz ve içerik analizi yöntemiyle çözümlemeler yapılmıştır. Araştırmaya katılan hastaların 12'sinin erkek, 6'sının 60-69 yaş arasında, 11'inin ilköğretim mezunu olduğu, 14'ünün evli, 7'sinin gelirinin giderine denk, 9'unun mesleki hayatta tekstil, boya, kauçuk işleriyle uğraşmamış olduğu, 12'sinin sigara kullanmadığı, 12'sinin tanı öncesi sigara kullandığı, 8'inin intravezikal BCG dozunu 12 ve üzerinde almış olduğu, 6'sının hastalık ve tedavi süreci hakkında kısmen bilgi sahibi olduğu ve 8'inin BCG işlemi hakkında bilgi almadığı bulundu. Yapılan görüşmelerin analizinden 3 tema, 9 kategori ve 45 kod elde edilmiştir. Birinci Tema Tedaviyle İlgili Duygular; BCG Aşısının Tedavide Kullanılmasına Yönelik Duygular, İşlem (BCG) Öncesi Duygular ve İşlem (BCG) Sonrası Duygular olmak üzere üç kategoriye ayrılmıştır. BCG Aşısının Tedavide Kullanılmasına Yönelik Duygular kategorisinin altında; Mutluluk, Umut ve Hayata Bağlanma kodları bulunmuştur. İşlem (BCG) Öncesi Duygular kategorisinin altında; Fayda Sağlayacağına İnanma, Belirsizlik/Endişe/Korku, Stres, Rahatlık/Sakinlik, Cesaret, Utanma ve Cinsel İktidarı Kaybetme Kaygısı kodları bulunmuştur. İşlem (BCG) Sonrası Duygular kategorisinin altında ise Tedaviyi Tanıma/Rahatlama, Şükür, Tedavinin Etkililiği Kaygısı, İyi Olacağına İnanma ve Acı Hissetme kodları bulunmuştur. İkinci Tema Tedavi Süreci; BCG Tedavi Süreci, BCG ile İlgili Bilgi Düzeyi ve BCG'nin Yan Etkileri olmak üzere üç kategoriye ayrılmıştır. BCG Tedavi Sürecinin altında; BCG İmmünoterapi ve Sistoskopi kodları bulunmuştur. BCG ile İlgili Bilgi Düzeyi kategorisinin altında; Bilgi Sahibi Olmama ve Bilgi Sahibi Olma kodları bulunmuştur. BCG'nin Yan Etkileri kategorisinin altında ise Yanma, Sık İdrara Çıkma, İdrarda Pıhtı, Ağrı, İdrarı Tutamama, Yorgunluk/Halsizlik, Ateş, Üşüme/Terleme, İdrar Zorluğu, İştah Kapanması, Cinsel Soğukluk, Döküntü/Kaşıntı, İştah Artışı, Cinsel İsteklilik ve Bulantı kodları bulunmuştur. Üçüncü Tema Tedavi Sonrası Yaşam; İşlem (BCG) Sonrası Dikkat Edilenler, İşlem (BCG) Sonrası Değişimler ve Süreç Memnuniyeti olmak üzere üç kategoriye ayrılmıştır. İşlem (BCG) Sonrası Dikkat Edilenler kategorisinin altında; Hijyen, Ayrı Tuvalet Kullanımı, Cinsel Perhiz ve İzolasyon kodları bulunmuştur. İşlem (BCG) Sonrası Değişimler kategorisinin altında; Kendini İyi Hissetme, BCG Dozları Arasındaki Farklar, Faydalı Bulma, Takıntı ve İlişkilerin Güçlenmesi kodları bulunmuştur. Süreç Memnuniyeti kodunun altında ise; Memnuniyetsizlik ve Memnuniyet kodları bulunmuştur. Bu araştırmayla ülkemizde ilk defa intravezikal BCG immünoterapi uygulanan hastaların deneyimleri incelemiştir. Hastaların BCG tedavisi aldıktan sonra nükslerin geciktiği, sosyal yaşamlarında daha rahat hareket ettikleri bulunmuştur. BCG tedavisinin kısa süreli yan etkileri olduğu ve hemen hemen her hastanın tedaviden sonra yan etki yaşadığı bulunmuştur. Hastaların BCG tedavisiyle ilgili yeterli bilgilerinin olmadığı ve bilgi gereksinimi duydukları bulunmuştur.

This study was carried out qualitatively using the in-depth interview method to examine the experiences of patients who received intravesical BCG immunotherapy. The study was carried out with 15 patients who accepted to participate in the study in the Urology service and outpatient clinics of the University of Health Sciences Istanbul Gaziosmanpaşa Training and Research Hospital, who received Intravesical BCG Immunotherapy, between March 5 and May 10, 2019, with in-depth interview method, with the approval of the ethics committee. The data research collected with sociodemographic data form, semi-structured in-depth interview form and voice recording form. The interviews were recorded on a tape recorder after obtaining the patients' permission. The average interview time is 45-60 minutes. A total of 443 minutes and 15 seconds of recording was recorded with 15 patients. 79 pages of written text were obtained from the interviews. In the evaluation of the data, descriptive analysis and content analysis method were used by using the Maxqda 2020 program. Of the patients participating in the study, 12 were male, 6 were between 60-69 years old, 11 were primary school graduates, 14 were married, 7 were equal to their income, 9 were not dealing with textile, paint or rubber in their professional life, It was found that 12 were non-smokers, 12 smoked before diagnosis, 8 had received the intravesical BCG dose of 12 or more, 6 had partial information about the disease and the treatment process, and 8 did not receive information about the BCG procedure. From the analysis of the interviews, 3 themes, 9 categories and 45 codes were obtained. First Theme Emotions Related to Treatment; Emotions for the Use of BCG Vaccine in Treatment are divided into three categories: Emotions before the Operation (BCG) and Post-Procedure (BCG) Emotions. Under the category of Emotions Regarding the Use of BCG Vaccine in Treatment; The codes of Happiness, Hope and Attachment to Life were found. Under the Emotions Before Operation (BCG) category; Codes of Believing Will Benefit, Uncertainty / Anxiety / Fear, Stress, Comfort / Calmness, Courage, Shame and Anxiety of Loss of Sexual Power were found. Under the Emotions after the Procedure (BCG) category, the codes of Recognizing the Treatment / Relaxation, Gratitude, Anxiety about Treatment Effectiveness, Believing That Will Be Good and Feeling Pain were found. Second Theme Treatment Process; BCG Treatment Process is divided into three categories: BCG Knowledge Level and BCG Side Effects. Under BCG Treatment Process; BCG Immunotherapy and Cystoscopy codes were found. Under the category of Knowledge Level About BCG; No Information and No Information codes were found. Under the Side Effects category of BCG, Burning, Frequent Urination, Urinary Clot, Pain, Incontinence, Fatigue / Fatigue, Fever, Chills / Sweating, Urinary Difficulty, Loss of Appetite, Sexual Cold, Rash / Itching, Increased Appetite, Sexual Desire and Nausea codes were found. The Third Theme is Life After Treatment; It is divided into three categories: Considerations After the Transaction (BCG), Changes After the Transaction (BCG), and Process Satisfaction. Under the category of Attention After Operation (BCG); Hygiene, Separate Toilet Use, Sexual Abstinence and Isolation codes were found. Under the category of Post-Transaction (BCG) Exchanges; The codes of Feeling Good, Differences Between BCG Doses, Finding Beneficial, Obsessions and Strengthening Relationships were found. Under the Process Satisfaction code; Dissatisfaction and Satisfaction codes were found. With this study, the experiences of patients who received intravesical BCG immunotherapy for the first time in our country were examined. It was found that relapses were delayed and they moved more comfortably in their social life after BCG treatment. BCG treatment has been found to have short-term side effects and almost every patient experiences side effects after treatment. It was found that the patients did not have sufficient information about BCG treatment and they needed information.
