Özet

Giriş ve Amaç: Herhangi bir nedenle nöbet atağı tetiklenerek acil servise başvuran epilepsi hastalarının nöbet sonrası izlem sürelerini net şekilde tanımlanmamıştır. Farklı takip stratejilerinden en yaygın olanı, 4-6 saat içerisinde nöbet geçirmeyen hastaların taburcu edilmesi şeklindedir. Taburcu edilen hastalarda tekrarlayan küme nöbetler ya da status epileptikus ile karşılaşılmaması için nöbet tetikleyicilerinin tanınması önemlidir. Çalışmamızın amacı; tekrar nöbet geçirme riski olan hastaların belirlenip acil serviste takip edilmesini, riskli olmayan hastaların da uzun süre bekletilmeden güvenle taburcu edilebilmesini sağlayacak faktörlerin belirlenmesine katkı sağlamaktır. Gereç ve Yöntem: Çalışmamızın örneklemi, Sağlık Bilimleri Üniversitesi Ankara Keçiören Eğitim ve Araştırma Hastanesi acil servisine 15.10.2018-15.10.2019 tarihleri arasında jeneralize tonik klonik nöbet geçirme şikâyetiyle başvuran ve bilinen epilepsi tanısı olan 142 hastadan oluşturulmuştur. Çalışmamızda kullanılan veriler oluşturulan form kayıtlarından ve hastane işletim sisteminden elde edildi. Bu veriler SPSS for Windows® 15.0 (SPSS Inc, Chicago, USA)® istatistik programı kullanılarak analiz edildi. P < 0.05 değeri istatistiksel olarak anlamlı kabul edildi. Bulgular: Acil servise başvuran 142 hastanın yaş ortanca değeri 31.5 yıl (IQR %25-75:24-43) olarak hesaplandı. Bu hastaların 72'si (%50.7) erkekti. 22 (%15.5) hastada haftada bir kez rutin epilepsi atağı geçirme hikayesi mevcuttu. Bu 142 hastanın 31'nde (% 21.8) acil servis rutin takibi sırasında ilk 6 saatte tekrarlayan nöbet görüldü. İlk 6 saatte tekrarlayan nöbeti önceden tahmin edebilecek bağımsız risk faktörlerini belirleyebilmek için yaptığımız çok değişkenli lojistik regresyon analizine göre; düzensiz antiepileptik ilaç (AEİ) kullanımı (OR:3.044, %95CI:1.1-8.37), beyaz küre sayısı (OR:1.161, %95CI:1.027-1.312) ve başvuru anındaki nöbet sonlanımı için ilaç kullanımı ihtiyacı (OR:11.87, %95CI:2.9-47.05) bağımsız risk faktörleri olarak saptandı. Hastaların 24 saatte nöbet geçirip geçirmedikleri göz önüne alındığında ise 39 (%27.4) hastanın ilk 24 saatte tekrar nöbet geçirdiği tespit edildi. İlk 24 saatte tekrarlayan nöbeti önceden tahmin edebilecek bağımsız risk faktörlerini belirleyebilmek için yaptığımız çok değişkenli lojistik regresyon analizine göre küme tipi nöbet geçirme hikâyesi (OR:3.066, %95CI:1.162-8.085), beyaz küre sayısı (OR:1.183, %95CI:1.054-1.328), başvuru anındaki nöbet sonlanımı için ilaç kullanımı ihtiyacı (OR:9.952, %95CI:2.637-37.555) ve son AEI dozunu atlama (OR:3.113, %95CI:1.248-7.763) bağımsız risk faktörleri olarak saptandı. Sonuç: Çalışmamızda ilk 6 saatte tekrarlayan nöbet için belirleyebildiğimiz prediktör faktörler; başvuru anındaki nöbet sonlanımı için ilaç kullanımı ihtiyacı, düzensiz AEİ kullanımı ve yüksek beyaz küre sayısıdır. İlk 24 saatte tekrarlayan nöbet için belirleyebildiğimiz prediktör faktörler ise küme tipi nöbet hikayesi, başvuru anındaki nöbet sonlanımı için ilaç kullanımı ihtiyacı, son AEİ dozunu atlama ve yüksek beyaz küre sayısıdır. Bu prediktörlerin varlığı, hastaların acil serviste daha uzun süre takip edilmesi gerekliliği açısından yol gösterici olabilir.

İntroduction and Aim: Epileptic patients, who apply to the emergency department by triggering a seizure attack with any reason, were not clearly defined after the seizure. The most common of the different follow-up strategies is the discharge of patients who do not have seizures within 4-6 hours. It is important to recognize seizure triggers in order to avoid recurrent cluster seizures or status epilepticus in discharged patients. The aim of this study is to contribute to the determination of the patients who are at risk of having seizures and to be followed up in the emergency room, and to determine the factors that will ensure that patients who are not at risk can be discharged safely without waiting for a long time. Material and Method: Subjects of the study consist of 142 patients who were diagnosed with a complaint of generalized tonic clonic seizures with epilepsy between the dates of October 15th, 2018 and October 15th, 2019 to the emergency department of Health Sciences University Ankara Kecioren Training Research Hospital. Data were obtained from form records and hospital operating system. Statistical analyze of all data recorded on study form were made by using SPSS for Windows® 15.0 (SPSS Inc, Chicago, USA)® statistics software. Value of p<0.05 were accepted as significant statistically. Results: The median age of 142 patients admitted to the emergency department was calculated as 31.5 years (IQR 25-75%: 24-43). 72 (50.7%) of these patients were male. 22 (15.5%) patients had a history of having a routine epilepsy attack once a week. Recurrent seizures were observed in the first 6 hours during emergency routine follow-up in 31 of these 142 patients (21.8%). According to our multivariate logistic regression analysis to determine independent risk factors that can predict recurrent seizures in the first 6 hours; irregular antiepileptic drug (AED) use (OR: 3.044, 95% CI: 1.1-8.37), white blood cell count (OR: 1.161, 95% CI: 1.027-1.312), and need for drug use for the end of seizure at presentation (OR: 11.87,% 95CI: 2.9-47.05) were identified as independent risk factors. Considering whether the patients had seizures in 24 hours, 39 (27.4%) patients were found to have seizures in the first 24 hours. According to the multivariate logistic regression analysis we conducted to determine the independent risk factors that can predict the recurrent seizure in the first 24 hours, the history of cluster-type seizures (OR: 3.066, 95% CI: 1.162-8.085), white blood cell count (OR: 1.183, 95% CI: 1.054 -1.328), the need for drug use for the end of seizure at the time of admission (OR: 9.952, 95% CI: 2.637-37.555) and skipping the last dose of AEI (OR: 3.113, 95CI: 1.248-7.763) were independent risk factors. Conclusion: In our study, predictive factors that we can determine for recurrent seizures in the first 6 hours; the need for drug use for ending seizure, at admission is irregular AED use and high white blood cell count. The predictive factors that we can determine for recurrent seizures in the first 24 hours are the history of cluster-type seizures, the need to use medications for the end of seizure at the time of admission, the skipping of the last AED dose and the high white blood cell count. The presence of these predictors can be a guide in terms of patients' need to be followed up in the emergency room for a longer period of time.
