Objective: Hepatitis B infection; it is a disease with an acute or chronic clinical spectrum ranging from asymptomatic disease to fulminant hepatic failure. Approximately 800,000 people die every year in the world due to cirrhosis, hepatacellular carcinoma and extrahepatic involvement, which are the serious complications of the disease. Our aim in this study to compare the biochemical, serological and virological efficacy of treatment with Entecavir and Tenofovir in patients with Chronic Hepatitis B (CHB) infection who have not had any previous treatment experience. Material method: In our study 109 patients who had no previous treatment experience and received tenofovir or entecavir treatment for at least one year were included, who were followed up with the diagnosis of CHB infection between January 2008 and December 2019 in the Viral Hepatitis Outpatient Clinic of the Infectious Diseases and Clinical Microbiology Clinic of Health Sciences University Izmir Bozyaka Training and Research Hospital. After the patients were divided into groups according to the treatments they received, the patient files and their records in the automation system were examined and all biochemical, virological, serological and histological values included in the content were recorded in an Excel file. Both groups were evaluated retrospectively with their examinations during treatment, and compared in terms of ALT normalization time and rate, HBV DNA negative time and rate, HBeAg seroconversion time and rate. Results: The study included 109 patients, 74 male and 35 female, the average age was 50.16 ± 1.21. While the number of patients using entecavir was 79, the number of patients using tenofovir was 30. In the examinations of the patients before the treatment; there was a statistically significant increase in HBV-DNA level and fibrosis score in the tenofovir group compared to the entecavir group. There was no significant difference between the two groups in terms of pre-treatment ALT, AST and HBeAg positivity. HBeAg seroconversion rate was 18% in entecavir group and 33% in tenofovir group; the mean seroconversion time was 43 ± 23.01 months (18-112 months) in the entecavir group and 12.2 ± 3.2 months (6-24 months) in the tenofovir group. ALT normalization ratio was observed as 87%, 92%, 92% in the entecavir group and 70%, 89%, and 93% in the tenofovir group, respectively, in the 6th, 12th and 24th months of treatment; the mean normalization time was 4.452 ± 0.57 months in the entecavir group and 6.143 ± 1.29 months in the tenofovir group (p<0,1638). HBV-DNA negativity rates were observed as 59%, 86%, 94% in the entecavir group in the 6th, 12th and 24th months of the treatment, and 50%, 83%, and 93% in the tenofovir group, respectively; the mean time to become negative was 6.23 ± 0.54 months in the entecavir group and 9.1 ± 1.45 months in the tenofovir group (p<0,0209). When these data were evaluated, ALT normalization rate at the sixth month was found to be significantly higher in the entecavir group compared to the tenofovir group. In addition, the time for HBV-DNA to become negative in the entecavir group was found to be statistically shorter. There was no statistically significant difference in other parameters evaluated. Conclusion: In our study, the effectiveness of Entecavir and Tenofovir, which are potent antiviral drugs with low resistance rates due to their high genetic barriers, were retrospectively compared. HBeAg seroconversion rates in our study were similar to those in the literature. It was thought that the reason for the shorter HBV-DNA negative time observed in the entecavir group and the higher ALT normalization rate in the first six months may be due to the lower baseline HBV-DNA values and liver fibrosis scores of the patients in the entecavir group compared to the tenofovir group. It was observed that the use of Entecavir and Tenofovir in naive chronic HBV patients we followed up in our study had similar efficacy in terms of virological, biochemical and serological response, and both drugs could be preferred as first-line therapy.
