Özet
Bu çalışma, Outerbridge sınıflamasına göre tek taraflı evre 3-4 kondromalazi patella tanısı nedeniyle Fulkerson osteotomi cerrahisi geçirmiş ve ardından fizik tedavi ve rehabilitasyon programı uygulanmış bireyler ile Outerbridge sınıflamasına göre tek taraflı evre 3-4 kondromalazi patella tanısı nedeniyle sadece fizik tedavi ve rehabilitasyon programı uygulanmış bireylerin; diz eklem pozisyon hissi, yürüyüş kinematikleri ve işlevsel seviyelerinin karşılaştırılması amacıyla yapıldı. Çalışmaya Fulkerson osteotomisi geçirmiş 20 hasta ile konservatif takip edilmiş 20 hasta dahil edildi. Diz eklem pozisyon hissi; 15°, 45° ve 60° hedef açılarda aktif açı tekrarlama yöntemi ile DrGoniometer® mobil uygulaması kullanılarak değerlendirildi. Yürüyüş kinematikleri, Zebris FDM-T® cihazı ile değerlendirildi. İşlevsel seviye, tek bacak öne sıçrama, tek bacak çömelme ve Kujala sübjektif diz değerlendirme formu ile belirlendi. Fulkerson osteotomi cerrahisi geçirmiş hastalar ile konservatif (sadece fizyoterapi ve rehabilitasyon uygulanmış) takip edilmiş hastaların etkilenen taraf yürüyüş kinematikleri karşılaştırıldığında, adım uzunluğu, kadans, duruş ve salınım fazı yüzdeleri arasında fark vardı (p<0,05). Fulkerson osteotomi cerrahisi yapılan hastaların etkilen taraflarına ait diz eklem pozisyon hissinde 60°' de tedavi öncesi hedef açıda kayıp bulundu (p<0,05). Tedavi sonrası değerlendirmede kayıp yoktu (p<0,05). Konservatif takip edilmiş hastaların etkilenen tarafların ait diz eklem pozisyon hissinde 60° hedef açıda hem tedavi öncesi ve hem de tedavi sonrası kayıp bulundu (p<0,05). Tek bacak öne sıçrama ve tek bacak çömelme test sonuçları incelendiğinde, her iki gruptaki hastaların etkilenen taraflarındaki öne sıçrama mesafeleri ve çömelme kabiliyetleri arasında fark yoktu (p>0,005). Kujala sonuçları incelendiğinde, Fulkerson osteotomi cerrahisi geçirenlerin skorları anlamlı olarak daha yüksekti (p<0,005). Fulkerson osteotomi cerrahisi yapılan hastaların diz eklem pozisyon hissi, yürüyüş kinematikleri ve işlevsel seviye açısından konservatif (sadece fizyoterapi ve rehabilitasyon uygulanmış) takip edilmiş hastalardan üstün olduğu bulundu.

In this study, individuals who had undergone Fulkerson osteotomy surgery due to the diagnosis of unilateral stage 3-4 chondromalacia patella according to the Outerbridge classification and then received a physical therapy and rehabilitation program and only physical therapy and rehabilitation program was applied due to the diagnosis of unilateral stage 3-4 chondromalacia patella according to the Outerbridge classification. individuals; It was performed to compare knee joint position sense, gait kinematics and functional levels. 20 patients who had Fulkerson osteotomy and 20 conservatively followed were included in the study. Knee joint position sensation; The active angle at 15°, 45° and 60° target angles was evaluated using the DrGoniometer® mobile application with the repeating method. Gait kinematics were evaluated with the Zebris FDM-T® device. Functional level was determined using the one-leg forward jump, one-leg squat, and the Kujala subjective knee assessment form. When the affected side gait kinematics of the patients who had fulkerson osteotomy surgery and those who were followed conservatively (only physiotherapy and rehabilitation was applied), there was a difference between the stride length, cadence, stance and swing phase percentages (p <0.05). It was found that the knee joint position sense of the affected sides of the patients who underwent fulkerson osteotomy surgery was lost in the target angle before surgery at 60° (p <0.05). There was no loss in the postoperative evaluation (p <0.05). Patients who were followed conservatively had a loss of 60° target angle in the knee joint position sense of the affected sides both before and after treatment (p <0.05). When the single leg forward hop and one leg squat test results were examined, there was no difference between the forward hop distances and squatting abilities on the affected sides of the patients in both groups (p> 0.005). When the Kujala results were examined, the scores of those who had Fulkerson osteotomy surgery were significantly higher (p <0.005). It was found that patients who underwent fulkerson osteotomy surgery were superior to patients who were followed conservatively (physiotherapy and rehabilitation only) in terms of knee joint position sense, gait kinematics and functional level.
