Background: Pain is one of the most common phenomena in children and the evidence show that pain may be shaped related to cultural factors. Therefore to measure and treat children' pain depends on to understand how the culture influence on pain perception, assessment and expression of children. Aims: The aim of this systematic review was to identify cross-cultural differences and similarities on the pain perception, expression and assessment of children during paediatric procedures. Methods: Medline, Cinahl, Central, University of Sheffield Library, search engine and reference lists were used to search studies written in English, and Turkish between 1984 and 2015. Results: Totally twenty studies were identified based on inclusion criteria. The studies examined pain perception, expression, assessment, description, colour, language, coping strategies of Palestinian, Jordanian, Lebanese, Egyptian, Syrian, Saudi Arabic, Thai, Taiwanese, Mi'kmaq (Aboriginal), English, Kurdish, Hispanic, African- American, Anglo- American, European- American, Latino, Canadian, Chinese, Japanese, Dutch, Mexican-American, American, Kuwaitis, Levantine and Lebanese children during paediatric medical, experimental, recurrent, or chronic pain procedures. Furthermore, findings were summarised into two themes; first, cultural influences on self- report pain assessment and behavioural pain assessment and psychological responses of children; second, cultural influences on pain language and coping strategies of children. The methodology of included studies was also analysed and fourteen quantitative, five qualitative and one mixed methods studies were identified. 4 Conclusion: Examined studies showed that there is not cross- cultural difference in children by using pain assessment tools however pain perception and expression of children were found different related to cultural differences. There should be more cultural sensitive pain tools for non- Western children and a theoretical framework to highlight cross- cultural differences in paediatric pain. Furthermore, to develop a valid and reliable universal pain tool is also required for clinical usage in multi- cultural societies
