Background: Cardiac rehabilitation has extensive benefits for patients with coronary heart disease. It reduces mortality rates, improves quality of life, promotes active and healthy lifestyles and reduces stress and anxiety levels, yet, participation rates still remain low worldwide at 20%-50% (BACPR, 2012). Objectives: To gain insight into the influences on low attendance rates and explore the facts that affect patients' participation. Methods: A qualitative systematic review utilising the Joanna Briggs Institute (JBI) approach was conducted. Multiple databases including MEDLINE, EMBASE, CINAHL, JBI Library and Cochrane Library were systematically searched. Searches of article reference lists and keyword searching on the internet were conducted. Results: 18 studies were included. 155 findings were extracted from these studies and categorised. These categories were based on commonalities and five main themes emerged in relation to barriers: those related to the health care system and service, physical difficulties to attending, personal attitudes and beliefs, cultural and ethnic background and specific influences on women's attendance. Conclusions: Key issues related to the barriers affecting patients' attendance have been explored in this review. This work provides insight into the nature of the obstacles to attendance and could assist those planning to design cardiac rehabilitation programmes in the future, as recommendations are made for good practice. Strategies developed to overcome the identified barriers would need to be investigated and tested in further research studies.
