Özet
Total Diz Artroplastisi Sonrası Erken Dönem Nöromusküler Elektriksel Stimülasyon Uygulamasının Son Durum Üzerine Etkisi. Nurcihan Çiftci. Marmara Üniversitesi, Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı. Danışman: Prof. Dr. İlker Yağcı. Giriş ve Amaç: Çalışmamızın amacı total diz artroplastisi (TDA) sonrası uygulanan tedavi programına eklenen Nöromusküler Elektriksel Stimülasyon (NMES) uygulamasının ağrı, eklem hareket açıklığı, kas kuvveti ve yaşam kalitesi üzerine etkilerinin araştırılmasıdır. Gereç ve Yöntem: Çalışmamıza osteoartrit nedeniyle total diz artroplastisi yapılan 30 hasta dahil edildi. Hastalar randomize olarak 2 gruba ayrılarak 15 hasta çalışma, 15 hasta da kontrol grubuna alındı. Çalışma grubundaki hastalara sürekli pasif hareket cihazı, egzersiz, soğuk uygulama ve NMES uygulanırken kontrol grubundaki hastalara NMES hariç diğer tedaviler uygulandı. Değerlendirmeler postop erken dönem, 10.seans sonrası ve 3. ay sonrası olmak üzere üç kere yapıldı. Tüm katılımcılar kısa form-36 (SF-36), Western Ontario and Mc Master Universities Osteoarthritis Index (WOMAC OA indeksi), visuel analog skalası (VAS), gonyometrik ölçüm ve manuel kas testi ile değerlendirildiler. Bulgular: Çalışma grubunda postop erken dönem ve 10.seans sonrası ölçümlerde diz ekstansiyon açısı daha fazla bulundu (p<0,05). Ancak başlangıç değerlerine baktığımızda çalışma grubuna diz ekstansiyonu daha iyi olan hastaların yığıldığı ve istatistik anlamın bu farktan kaynaklandığı saptandı. Diğer ölçümlerde grupiçi değerlerde anlamlı değişimler olurken, gruplar arası karşılaştırmada anlamlı bir fark olmadığı görüldü. Sonuç: Total diz artroplastisi sonrası uygulanan fizik tedavi hastalarda anlamlı değişimler olmasını sağlarken, programa eklenen NMES uygulamasının tedavide üstünlüğü bulunamadı.

The Effect of Early Neuromuscular Electrical Stimulation Application on the Final Situation After Total Knee Arthroplasty. Nurcihan Ciftci. Introduction: The aim of our study is to investigate the effects of Neuromuscular Electrical Stimulation (NMES) application added to the treatment program after total knee arthroplasty (TKA) on pain, range of motion, muscle strength and quality of life. Materials and Methods: Thirty patients who underwent total knee arthroplasty for osteoarthritis were included in our study. Patients were randomly divided into 2 groups, 15 patients were included in the study group and 15 patients in the control group. Continuous passive motion device, exercise, cold application and NMES were applied to the patients in the study group, while the patients in the control group received other treatments except NMES. Evaluations were made three times: early postop, after the 10th session and after the 3rd month. All participants were evaluated by short form-36 (SF-36), Western Ontario and Mc Master Universities Osteoarthritis Index (WOMAC OA index), visuel analog scale (VAS), goniometric measurement and manual muscle test. Results: In the study group, knee extension angle was found to be higher in the early postoperative period and after the 10th session (p<0.05). However, when we looked at the baseline values, it was found that patients with better knee extension were stacked in the study group and the statistical significance was due to this difference. While there were significant changes in the in-group values in other measurements, it was observed that there was no significant difference between the groups. Conclusion: While physical therapy applied after total knee arthroplasty provides significant changes in patients, NMES application added to the program has not been found to be superior in treatment.
