Hastaların 21'i kadın (%45.7), 25 'ide (%54,3) erkekti.
INTRODUCTION Acute cholecystitis is a common disease in the community. The prevalence of gallstone disease in the United States is estimated to be 10-15%, affecting 20 million people. There is still no definitive consensus on the treatment of acute cholecystitis. When we look at the literature, some studies say that it is important to determine the severity of acute cholecystitis and that the severity is important in determining the treatment. In this study, we aimed to investigate the importance of SuPAR efficacy during treatment compared to other inflammatory biochemical markers in patients with acute cholecystitis, and to investigate whether it can be used as a biochemical marker in the classification and definition of acute cholecystitis. METHOD Patients with a history of hospitalization due to acute cholecystitis in our general surgery clinic between January 2020 and December 2021 were evaluated. Between these dates, a total of 268 patients were hospitalized due to acute cholecystitis. Of these, 46 patients with acute cholecystitis and 34 healthy volunteers who met the study criteria (to evaluate SUPAR sensitivity-specificity) were included in the study. The patients' dermographic data, blood parameters, intraoperative findings, pre-operative imaging features, and clinical presentations were analyzed. RESULTS He means age of the patients was 46.7±10.3 (26-65) years. Of the patients, 21 (45.7%) were female and 25 (54.3%) were male. According to Tokyo Grade, there were 13 (28.3%) Grade 1 patients, and Grade 2 and above 33 (71.7%) patients. According to the Parkland Grade scale, 4 (8.7%) in Grade 1, 8 (17.4%) in Grade 2, 7 (15.2%) in Grade 3, 11 (23.9%) in Grade 4), there were 16 (34.8%) Grade 5 patients. In our study, it was found that preoperative CRP, WBC, total bilirubin, sedimentation, procalcitonin, NLR and SuPAR statistically increased as the grade increased according to the Tokyo Grade scale. In the classification made according to the Parkland Grade scale, it was observed that preoperative SuPAR, NLR, Procalcitonin and age were statistically correlated with grade. For the cut-off value of the SuPAR parameter >2,055, the sensitivity and specificity values were found to be 97.83% and 96.97%, respectively. In terms of imaging features, it was found that the finding of submucosal edema on CT was statistically significant with Parkland Grade, and the finding of submucosal edema was seen more as the Grade increased. CONCLUSİON We found that the SuPAR parameter can be used safely in the treatment of acute cholecystitis and is more effective than other biochemical markers.
