Introduction and Aim: Rectal cancer is the 7th most common type of cancer among non-melanoma cancers in both sexes and constitutes 4,7% of all cancers. Although neoadjuvant chemoradiotherapy reduces local recurrence in rectal cancer and increases the sphincter protection rate, it also contributes to survival in locally advanced rectal cancer. In this study, it was aimed to evaluate the pathological, radiological and clinical characteristics of patients who received neoadjuvant chemoradiotherapy for rectal cancer and to reveal the factors affecting the neoadjuvant treatment response. Materials and Methods: The data of 68 patients who were operated on at Ankara Dışkapı Yıldırım Beyazıt Training and Research Hospital between 01.01.2018 and 15.05.2022 with the diagnosis of rectal cancer after neoadjuvant chemoradiotherapy treatment were retrospectively analyzed. The relationship between tumor regression score and clinical, demographic and pathological characteristics of the patients was analyzed. Results: 68 patients with a mean age of 63.32±11.09 years were included in the study. In the cross-comparisons and univariate analysis performed in our study, it was determined that the rate of poor response was 77.8% in those with serum CEA values >5 ng/ml before neoadjuvant treatment, while it was 31.7% in those with ≤ 5 ng/ml, and the difference was statistically significant (p<0.001). While the rate of poor response was 87.5% in patients with free tumor nodules in pathology, this rate was 46.7% in patients without and the difference was statistically significant (p=0.033). It was determined that the tumor size on MRI before treatment in patients with a poor response to treatment was statistically significantly larger than those with a good response (p<0.001). In the multivariate regression analysis, it was determined that 1 cm increase in tumor size significantly increased the risk of poor response to neoadjuvant therapy by 2,3 times (OR=2.3, %95 GA=1.217-4.350, p=0.010).. Discussion and Result: In this study, serum CEA value >5ng/ml, presence of free tumor nodules and large tumor size before neoadjuvant treatment in patients with rectal cancer were revealed as factors that negatively affect the neoadjuvant treatment response.
