Özet

Amaç: Böbrek nakli, son dönem böbrek hastalığının en seçkin tedavi yöntemidir. Böbrek nakli yapılması planlanan hastaların pre-operatif değerlendirilmesi, operasyon sonrası takipleri, gelişebilecek komplikasyonlar ve komplikasyon gelişmesi halinde re-operasyon yapılması gereken hastalarda cerrahi tedavi yöntemlerinin değerlendirilmesi amaçlanmıştır. Yöntem: Son dönem böbrek hastalığı tanısı ile böbrek nakli yapılan hastalar retrospektif olarak değerlendirildi. 116 hastanın dosya taraması sonucu komplikasyon gelişimine bağlı re-operasyon uygulanan 13 hasta ve komplikasyon gelişmeyen 20 hasta çalışmaya alındı. 18 yaşından küçük hastalar ile kronik rejeksiyon sonrası tekrar böbrek nakli yapılan hastalar çalışma dışı bırakıldı. Böbrek nakli operasyonu, komplikasyon gelişimine bağlı operasyon kararı, aynı, deneyimli nakil uzmanı tarafından yapıldı. Hastaların pre-operatif ve post-operatif değerlendirmesi yine aynı uzman tarafından yapıldı. Bulgular: Çalışmada, 24 (%72,7)'ü erkek, 9 (%27,3)'u kadın hasta retrospektif olarak değerlendirildi. Hastalar komplikasyon gelişimine bağlı re-operasyon durumlarına göre 2 ayrı grupta değerlendirildi. Bu gruplar arasında HbA1c, VKİ, sigara kullanımı, donör ve hasta yaşı, kan transfüzyon durumu, donör böbreğin tarafı, hastanede yatış süresi ve sigorta maliyeti değerlendirildi. Komplikasyon gelişimine bağlı re-operasyon uygulanan hastalarda ve komplikasyon gelişmeyen hastalarda HbA1c %5,66±1,22 ve %6,16±1,57; VKİ 23,12±4,98 kg/m² ve 24,03±2,97 kg/m²; sigara kullanımı 8,08±10,52 paket/yıl ve 19,80±24,76 paket/yıl; donör yaşı 45,38±13,35 ve 47,90±12,54; hasta yaşı 39,92±16,79 ve 37,70±9,66 olarak bulundu. 8 (%24,2) hastada sağ böbrek, 25 (%75,8) hastada sol böbrek nefrektomide tercih edildi. Pre-operatif dönemde kan transfüzyonu 6 (%18,2) hastada uygulanmıştır. Bu hastaların 3 (%50)'ünde komplikasyon gelişimine bağlı re-operasyon uygulanmıştır. Hastanede yatış süreleri 21,77±12,13 gün ve 9,40±2,84 gün; sigorta maliyetleri 11590,08±1462,73 Amerikan Doları ve 7279,00±879,17 Amerikan Doları olarak bulundu. İstatiksel değerlendirme sonucuna göre komplikasyon gelişimine bağlı re- operasyon oranı sağ donör böbrek nefrektomide, kadın cinsiyette ve pre-operatif 11 dönemde kan transfüzyonu yapılanlarda anlamlı olarak yüksek bulundu. Bu hasta grubunda hastanede yatış süresi ve sigorta maliyetleri anlamlı oranda yüksekti. Sonuç: Böbrek nakli, son dönem böbrek yetmezliği olan hastalarda önemli bir tedavi şeklidir. Böbrek nakli sayıları her geçen gün artış göstermektedir, bununla birlikte komplikasyonlar da daha sık görülmektedir. Literatürde, böbrek nakli sonrası gelişen komplikasyonun erken ve doğru tedavisi ile greft ve hasta sağ kalımının önemli oranda arttığı bildirilmiştir. Komplikasyon gelişmesi durumunda uygun tetkik ile komplikasyon değerlendirmesinin yeterli yapılması tedavisi için yeterli plan oluşturulmasını sağlar. Uygulanacak tedavi için her hasta ayrı değerlendirilmeli, komplikasyon gelişiminin artmasına neden olan faktörler varlığı durumunda cerrah çok daha dikkatli olmalıdır. Komplikasyon gelişmesi halinde yapılan değerlendirmeler sonucu, komplikasyon tedavisi için operasyon kararı alınması mümkündür. Hastalarda görülebilecek olan vasküler, ürolojik ve genel cerrahi komplikasyonları ile ilgili yapılabilecek cerrahi müdahaleler hasta ve greft sağ kalımını artırır.

Aim: Kidney transplantation is the most selective treatment for end-stage renal disease. It is aimed to evaluate the pre-operative evaluation of patients who are planned for kidney transplantation, postoperative follow-ups, possible complications and surgical treatment methods in patients who need re-operation in case of complications. Methods: Patients who underwent kidney transplantation with the diagnosis of end- stage renal disease were evaluated retrospectively. As a result of the file scanning of 116 patients, 13 patients who underwent reoperation due to the development of complications and 20 patients who did not develop complications were included in the study. Patients younger than 18 years of age and patients who underwent re- kidney transplantation after chronic rejection were excluded from the study. Kidney transplant operation was performed by the same experienced transplant specialist, who decided to operate due to the development of complications. Pre-operative and post-operative evaluation of the patients was performed by the same specialist. Results: In the study, 24 (%72,7) male and 9 (%27,3) female patients were evaluated retrospectively. The patients were evaluated in 2 different groups according to their re-operation status due to the development of complications. Among these groups, HbA1c, BMI, smoking, donor and patient age, blood transfusion status, donor kidney side, length of hospital stay and insurance cost were evaluated. HbA1c was %5,66±1,22 and %6,16±1,57 in patients who underwent re-operation due to complication development and in patients who did not develop complications; BMI 23,12±4,98 kg/m² and 24,03±2,97 kg/m²; smoking 8,08±10,52 package/year and 19,80±24,76 package/year; donor age 45,38±13,35 and 47,90±12,54; patient age was 39,92±16,79 and 37,70±9,66. Right kidney in 8 (24,2%) patients and left kidney in 25 (75,8%) patients were preferred for nephrectomy. In the pre-operative period, blood transfusion was administered in 6 (%18,2) patients. Re-operation was performed in 3 (%50) of these patients due to the development of complications. Duration of hospitalization was 21,77±12,13 day and 9,40±2,84 day; insurance costs 13 were 11590,08±1462,73 USD and 7279,00±879,17 USD. According to the results of the statistical evaluation, the rate of re-operation due to the development of complications was found to be significantly higher in right donor kidney nephrectomy, in female gender, and in those who received blood transfusion in the pre-operative period. The length of hospital stay and insurance costs were significantly higher in this patient group. Conclusion: Kidney transplantation is an important treatment modality in patients with end-stage renal disease. The number of kidney transplants is increasing day by day, however complications are more common. In the literature, it has been reported that early and correct treatment of complications after kidney transplantation significantly increases graft and patient survival. In case of complication development, adequate evaluation of complications with appropriate examination ensures an adequate plan for treatment. Each patient should be evaluated separately for the treatment to be applied, and the surgeon should be more careful in the presence of factors that increase the development of complications. If the event of a complication, it is possibble to make an operation decision fort he treatment of complications as a result of the evaluations made. Surgical interventions for vascular, urological and general surgery complications that may be seen in patients increase patient and greft survival.
