Özet
Giriş ve Amaç: Çocuklarda acil servis başvurularının en sık nedeni olan solunum yolu enfeksiyonları (SYE), çoğunlukla aile sağlığı merkezlerinde (ASM) yönetilebilir. Bunun için SYE semptomlarına (SYES) ilişkin doğru tutum gerekir. Bu araştırmada annelerin çocukları için SYES ile gereksiz acil başvurularının eğitim ile azaltılması ve SYES hakkındaki bilgi, tutum ve davranışlarının (BTD) geliştirilmesi amaçlanmıştır. Yöntem: Kontrollü eğitim müdahalesi olarak planlanan araştırmada, Eğitim ASM'ye kayıtlı olup Şubat-Nisan 2019 tarihleri arasında başvuran ve katılmayı kabul eden, 6 ay–6 yaş çocuğu olan kadınlardan, araştırıcıya rastgele atanan birime kayıtlı olanlar müdahale grubu, diğer iki birime kayıtlı olanlar kontrol grubu olarak belirlenmiştir. Örneklem hesabı ile en az 58+58 kişiye ulaşılması gerektiği hesaplanmış, ulaşılabilir örneklem yöntemiyle minimum katılımcı sayısına ulaşılmıştır. Müdahale grubuna birebir, yüz yüze eğitim ve araştırıcılar tarafından hazırlanan broşür verilmiştir. Eğitim ve broşür içeriğinde çocuklarda SYES yönetimi ve alarm bulguları üzerinde durulmuştur. Annelere, SYES hakkında BTD sorgulayan likert tipi sorular ile, ateşe yönelik uygulamalar, çocukları için aldığı ASM, poliklinik ve acil servis hizmetleri, başvuru sayısı ve nedenleri sorulmuş, eğitim öncesi (Ö) ve sonrası (S) müdahale (M) ve kontrol (K) grubu karşılaştırılarak bulgulara erişilmiştir. Ölçeğin iç tutarlılığını gösteren cronbach-α katsayısı 0,68-0,72'dir. Bulgular: Araştırmaya kriterleri sağlayan 223 anne katılmıştır. Ancak araştırmadan çekilen, müdahale sonrası kriterleri karşılamayan veya ulaşılamayanlar çıkarıldığında, (K=118, M=60) 178 kişiyle çalışma tamamlanmıştır. Her iki grubun müdahale öncesi sosyodemografik özellikleri, çocukları için sağlık hizmeti alımları, ateşe ilişkin uygulamaları ve BTD puanları arasında anlamlı fark yoktur. Müdahale sonrası her iki grubun da BTD puanları artmıştır (K=Ö:76,9-S:82,2; M=Ö:76,9-S:83,6; p<0,001) ancak iki grup arasındaki fark anlamlı değildir (p=0,193). Her iki grupta da SYES ile acile başvuru azalmıştır. Ancak K-grubu SYES ile acile daha az sıklıkta başvururken (p=0,014), M-grubunun SYES ile acile başvuru sıklığı değişmemiştir (p=0,492). Başvuru ortancası her iki grup için de azalmıştır(Ö:1-S:0). Ancak M-grubunda hastalığın ciddiyeti nedeniyle acile başvurduğunu ve ciddi ise acile gideceğini söyleyenler azalırken, kontrol grubunda artmıştır. Acil servise SYES ile başvuranlar, ASM ve polikliniklere de daha çok başvurmuştur (p<0,001). Tartışma: Müdahale sonrası kontrol ve müdahale grubu arasında beklenen farkın görülmemesinin çeşitli nedenleri olabilir. Araştırma popülasyonu Türkiye ortalamasına göre daha az acil servislere başvurmaktadır. Eğitim içeriği anneler üzerinde uyarıcı etki yapmış ve acil durumlara hassasiyeti arttırmış olabilir. Eğitimin tek oturumda yapılması, hatırlatma olmaması ve annelerin bilgi düzeyine uygun şekilde bireyselleştirilmemesi eğitimin etkinliğini etkilemiş olabilir. İlk değerlendirme soruları, kontrol grubunda da farkındalığı arttırarak müdahalenin etkinliğini azaltmış olabilir. Sonuç olarak, bu araştırma ülkemizde çocuklarda SYES ile gereksiz acil başvuruların azaltılması amacıyla planlanacak eğitim müdahalelerinin ilki olması açısından önem arz etmektedir. Ancak beklenen azalmanın kalıcı olarak görülebilmesi ve istatistiksel olarak gösterilebilmesi için daha geniş ölçekli ve uzun vadeli müdahale çalışmaları yapılmalıdır.

Introduction and Aim: Respiratory tract infections (RTI), the most common cause of emergency applications in children, can mostly be managed in Family Health Centers (FHC). This requires the right attitude towards RTI symptoms (RTIS). In this study, it is aimed to reduce the unnecessary emergency admissions of mothers with RTIS in their children, through education and to improve their knowledge, attitudes and behaviors (KAB) about RTIS (KABaRTIS). Methods: In the study planned as a controlled educational intervention, among the women who have 6 months–6 years old children who registered to Education FHC, applied between February-April 2019 and accepted to participate; those who were registered to the unit randomly assigned to the researcher were identified as the intervention group, and those registered in the other two units were determined as the control group. Sample size was calculated as at least 58+58 and minimum number of participants has been reached with convenient sampling. The intervention group was given one-to-one, face-to-face training and a brochure prepared by the researchers. RTIS management and alarm findings were emphasized in the content of education and brochures. Likert-type questions about KABaRTIS, fever-related practices; FHC, clinic and emergency services received for their children, number and reasons of applications were compared before (B) and after (A) intervention in control (C) and intervention (I) groups. The cronbach-α coefficient showing the internal consistency of the scale was 0,68-0,72. Results: 223 mothers who met the criteria participated in the study. However, when those who were withdrawn from the research, who couldn't meet the criteria or who were inaccessible after the intervention were removed, the study was completed with 178 people (C=118, I=60). There was no significant difference between the sociodemographic characteristics, healthcare admissions for their children, fever-related practices and KAB scores of both groups before the intervention. KAB scores of both groups increased after the intervention (C=B:76.9-A:82.2; I=B:76.9-A:83.6; p<0.001), but the difference between the two groups was not significant (p=0.193). In both groups, emergency admissions with the RTIS decreased, but the C-group admitted to the emergency with the RTIS less frequently (p=0.014), while the frequency of the Igroup to the emergency with the RTIS did not change (p=0.492). The median of the admissions decreased for both groups (B:1-A:0, p=0,180). However, in the I-group, those who stated that they applied to the emergency department because of the severity of the disease and that they would go to the emergency services if a symptom is severe, decreased and for C-group it increased. Those who applied to the emergency department with RTIS also applied to FHC and clinics more (p<0.001). Discussion: There may be a few reasons for not seeing the expected difference between intervention and control group after the intervention. The study population went to the emergency departments less than the average of Turkish population. Educational content may have had a stimulating effect on mothers and increased sensitivity to emergencies. The fact that the training was done in one session, no recall and non-individualization of the training in accordance with the level of literacy of mothers may have affected the effectiveness. Also, the questionnaire might increase awareness in the C-group and overall contamination risk might reduce the effectiveness of the intervention. In conclusion, this research is important in terms of being the first of the educational interventions to be planned in Turkey to reduce unnecessary emergency admissions in children. However, in order to see the expected decrease permanently and show it statistically, larger scale and long-term intervention studies should be conducted.
