Özet

Omuz eklemi vücudumuzun en hareketli eklemidir ve günlük yaşamsal aktivitelerin yerine getirilmesinde önemli bir rol üstlenmektedir. Eklemin işlevinin büyük bölümü sağlam bir rotator manşet ile mümkündür. Çalışmamızda onarılamaz masif rotator manşet yırtıklarında açık cerrahi ile süperiyor kapsüler rekonstrüksiyon tekniğinin etkinliğini araştırmak, tekniğin avantajları, zorlukları ve komplikasyonları konusunda literatüre katkıda bulunmak ve ameliyat sonrası sonuçlar üzerinde etkili olabilecek belirleyici faktörlerin değerlendirilmesi amaçlandı. ''Masif rotator manşet yırtıklarında açık süperiyor kapsüler rekonstrüksiyon yapılan hastaların klinik sonuçları'' isimli çalışmamız 2017–2022 tarihleri arasında omuz ağrısı ve eklem hareket kısıtlılığı şikayetleri ile SDÜ Hastanesi Ortopedi ve Travmatoloji Kliniği'ne başvurmuş, fizik muayenesi, radyolojik tetkikleri (direk grafi ve MRG) yapılan ve inceleme sonrasında tam kat rotator manşet yırtığı tanısı alıp açık SKR tekniği ile opere edilen 30 hasta katıldı. Klinik değerlendirme omuz eklem hareket açıklıkları ile yapılırken fonksiyonel değerlendirmede Constant ve Murley omuz skoru, VAS skoru kullanıldı. Ameliyat sonrasında da 6. ayda kontrollerindeki sonuçlarla karşılaştırılarak analiz edildi. Yapılan değerlendirme sonucunda hastaların pre-operatif modifiye Constant- Murley omuz skorları ortalaması 27,57±5,87 puan iken, post-operatif modifiye Constant-Murley omuz skorları ortalaması 61,63±12,158 puan olarak tespit edildi. Hastaların pre-operatif ve post-operatif modifiye Constant-Murley omuz skorları arasında istatistiksel olarak çok yüksek anlamlı artış gözlendi (p<0,001). Pre-operatif toplam VAS ortalama değeri 7,26 ±0,84 bulunurken, post-operatif VAS ortalama değeri 1,82±0,96 tespit edildi. Pre-operatif ortalama VAS toplam skorunun, post-operatif VAS değerlerine göre istatistiksel olarak anlamlı derecede yüksek olduğu gözlendi (p<0,001). Pre-operatif omuz fleksiyon açısı derecesi 60,50 ±14,818 derece iken post-operatif omuz fleksiyon açısı derecesi 111,67±22,450 derecedir. Pre-operatif omuz abduksiyon açısı derecesi 44,83 ±10,945 derece iken post-operatif omuz abduksiyon açısı derecesi 105,17±21,111 derecedir. Post-operatif fleksiyon derecesi, abduksiyon derecesinin pre-operatif değerlerine göre istatistiksel olarak anlamlı derecede fazla olduğu elde edildi (p<0,05). Omuz AP grafilerinden Humeroakromiyal mesafeler ölçüldüğünde pre-operatif ortalama 4,46±1,62, post-operatif ortalama 7,46±2,03 bulunmuştur. Pre-operatif ve post-operatif HAM değerleri arasında istatistiksel olarak anlamlı farklılık saptanmıştır (0,001; p<0,05). Sonuç olarak, açık cerrahi ile SKR yönteminin, küçük bir insizyonla yapılması, cerrahi süreyi belirgin azaltması ve post-operatif anesteziye bağlı komplikasyonları azaltması, ucuz maliyet ve az deneyim gerektirmesi, sonuçlarının da artroskopik yöntemlerle benzer bulunması sebebiyle tercih edilebilecek bir cerrahi prosedür olduğu sonucuna varılmıştır.

The shoulder joint is the most mobile joint in our body and plays a significant role in performing daily activities. The majority of its function relies on a healthy rotator cuff. Our study aims to investigate the effectiveness of open surgical supero-spinal capsular reconstruction technique in irreparable massive rotator cuff tears, contribute to the literature regarding the advantages, challenges, and complications of the technique, and evaluate the determining factors that may affect postoperative outcomes. Our study titled“Clinical Outcomes of Patients Undergoing Open Suprascapular Capsular Reconstruction in Massive Rotator Cuff Tears”included 30 patients who presented to the SDU Hospital Department of Orthopedics and Traumatology between 2017 and 2022 with complaints of shoulder pain and limited joint movement. They underwent physical examination, radiological tests (X-ray and MRI), and were diagnosed with full-thickness rotator cuff tears. These patients were then operated on using the open supero-spinal capsular reconstruction technique. Clinical evaluation was performed by assessing the range of motion of the shoulder joint, and functional assessment included the Constant-Murley Shoulder Score and VAS score. The results were analyzed by comparing the postoperative outcomes at the 6-month follow-up with the results of their controls. The evaluation revealed that the mean preoperative modified Constant-Murley Shoulder Score of the patients was 27.57±5.87 points, while the mean postoperative modified Constant-Murley Shoulder Score was determined as 61.63±12.158 points. There was a statistically significant increase observed between the preoperative and postoperative modified Constant-Murley Shoulder Scores of the patients (p<0.001). The mean preoperative VAS score was 7.26±0.84, and the mean postoperative VAS score was 1.82±0.96, indicating a statistically significant decrease in the postoperative VAS score compared to the preoperative score (p<0.001). The mean preoperative shoulder flexion angle was 60.50±14.818 degrees, while the mean postoperative shoulder flexion angle was 111.67±22.450 degrees. The mean preoperative shoulder abduction angle was 44.83±10.945 degrees, whereas the mean postoperative shoulder abduction angle was 105.17±21.111 degrees. The postoperative flexion and abduction angles were significantly higher than the preoperative values (p<0.05). When measuring the humeroacromial distance from shoulder AP X-rays, the mean preoperative distance was 4.46±1.62, and the mean postoperative distance was 7.46±2.03. There was a statistically significant difference between the preoperative and postoperative humeroacromial distances (p<0.05). In conclusion, it has been concluded that the SKR method performed with open surgery, which involves a small incision, significantly reduces surgical time and decreases complications associated with post-operative anesthesia. It is also a cost-effective procedure that requires less experience, and its results are found to be comparable to arthroscopic methods. Therefore, it can be considered as a preferred surgical procedure.
