Özet

Bu çalışma, serebral palsili (SP)'li çocuklarda fonksiyonel seviye ve klinik tipin, ebeveynlerinin kas- iskelet sistemi problemlerine ve yaşam kalitelerine etkisini incelemek ve anne- baba üzerindeki etkileri karşılaştırmak amacıyla yapıldı. Çalışmaya 3-18 yaş arası 48 erkek 48 kız olmak üzere SP tanılı 96 çocuk ve ebeveynlerinin her ikisi dahil edilmiştir. Çocuklar ve ebeveynlerinin; yaş, boy uzunluğu, kilo, vücut kitle indeksi gibi demografik bilgileri kaydedilmiştir. Çocukların fonksiyonel seviyelerini değerlendirmek için; Kaba Motor Fonksiyon Sınıflandırma Sistemi (GMFCS) ve El Becerileri Sınıflandırma Sistemi (MACS) ölçekleri kullanılmıştır. Ebeveynlerin kas-iskelet sistemi problemlerini belirlemek Cornell Kas İskelet Rahatsızlığı Ölçeği ve Kol, Omuz ve El Sorunları Anketi (DASH); yaşam kalitesi incelemek için Aileye Etki Ölçeği (IPFAM) ve Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeği-Kısa Form (WHOQOL-BREF-27) ölçekleri kullanılmıştır. SP'li çocuğun GMFCS ve MACS ölçeklerine göre fonksiyonel seviyeleri düştükçe; ebeveynlerin her ikisinde de DASH ve Cornell skorları anlamlı derecede artmakta (p<0,05), annelerin IPFAM total, genel etki, sosyal ilişkilerde bozulma, baş etmede zorluk skorları ve babaların IPFAM total, genel etki, baş etmede zorluk skorları anlamlı derecede artmakta (p<0,05); ebeveynlerin her ikisinde de, WHOQOL tüm skorları anlamlı derecede düşmektedir (p<0,05). Ebeveynler, kas-iskelet sistemi problemleri açısından karşılaştırıldığında annelerin etkilenimlerinin babalardan daha yüksek olduğu gösterilmiştir (p<0,05); yaşam kaliteleri açısından karşılaştırıldığında yine annelerin babalara göre anlamlı derecede daha fazla etkilendiği belirlenmiştir (p<0,05). SP klinik tiplerine göre incelendiğinde, quadriplejik tip SP tanılı çocukların ebeveynlerinin Cornell skorları, diplejik ve hemiplejik tip SP tanılı çocukların ebeveynlerine göre anlamlı derecede daha yüksek olduğu bulunmuştur (p<0,05). Quadriplejik tip SP tanılı çocukların annelerinin WHOQOL tüm skorlarının ve babaların WHOQOL fiziksel sağlık ve çevre skorlarının diplejik ve hemiplejik tip SP tanılı çocukların anne ve babaların skorlarına göre anlamlı derecede daha düşük olduğu gösterilmiş (p<0,05); annelerde IPFAM total, genel etki, sosyal ilişkilerde bozulma ve baş etmede zorlanma skorlarının ve babalarda IPFAM total ve sosyal ilişkilerde bozulma skorlarının diplejik ve hemiplejik tip SP tanılı çocukların anne ve babaların skorlarına göre anlamlı derecede daha yüksek olduğu bulunmuştur (p <0,05).

This study was conducted to examine the effect of the functional level and clinical type of a child with cerebral palsy on the musculoskeletal system problems and quality of life of parents and to compare the effects on parents. 96 children diagnosed with CP, including 48 boys and 48 girls between the ages of 3-18, and both of their parents were included in the study. Demographic information such as age, height, weight, body mass index, family income level, educational status of the parents of the children and their parents were recorded. For the functional levels of children; Gross Motor Assessment System (GMFCS) and MACS scales, which are Hand Skills Classification System, were used; the clinical type is determined by the physician. Cornell and DASH scales to identify musculoskeletal problems of parents and their localization; IPFAM and WHOQOL scales were used to examine their effects on quality of life. As the functional levels of the child with CP decrease according to the GMFCS and MACS scales; DASH and Cornell scores increased significantly in both parents (p<0.05). As the functional levels of the child with CP decrease according to the GMFCS and MACS scales; IPFAM scores were significantly increased in both parents (p<0.05). As the functional levels of the child with CP decrease according to the GMFCS and MACS scales; WHOQOL scores decreased significantly in both parents (p<0.05). When parents were compared in terms of musculoskeletal problems, mothers were shown to be more affected than fathers; When compared in terms of quality of life, it was determined that mothers were affected more negatively than fathers (p<0.05). When CP was analyzed according to clinical types, Cornell and IPFAM scores of parents of children with Quadriplegic type CP were found to be significantly higher than parents of children with diplegic and hemiplegic type CP (p<0.05). It has been shown that the WHOQOL scores of parents of children with quadriplegic type CP are significantly lower than those of parents of children with diplegic and hemiplegic type CP (p<0.05).
