Özet
Giriş ve Amaç: Mezenkimal kök hücreler (MKH) rejenerasyonda önemli bir rol oynar. MKH'lerden türetilen eksozomların yara iyileşmesi üzerindeki etkisi farklı çalışmalarda gösterilmiştir. Literatürde eksozomların yara iyileşmesi aşamaları üzerindeki etkisi ve bu etkinin oluştuğu yolaklarla ilgili pek çok çalışma bulunmaktadır. Literatür taramasında yapılan çalışmalarda farklı kök hücre kaynakları kullanılmış olsa da bunların karşılaştırılmadığı fark edilmiştir. Çalışmamızın amacı yağ dokusu kaynaklı, kemik iliği kaynaklı ve göbek kordonu kaynaklı eksozomların yara iyileşmesi üzerindeki etkilerini karşılaştırmaktır. Gereç ve Yöntemler: 40 adet erkek Wistar Albino sıçanı tüm gruplarda 10'ar adet olmak üzere 4 gruba ayrıldı. Tüm gruplara streptozotosin verilerek diyabet oluşturuldu. Her ratın sırtında iki adet dairesel cilt defekti oluşturuldu. Her iki cilt defektine de kontrol grubunda 2ml salin solüsyonu, ikinci grupta 2ml 200 µg/mL adipoz kaynaklı kök hücre (AD-MKH) eksozomu, üçüncü grupta 2ml 200 µg/mL kemik iliği kaynaklı kök hücre (Kİ-MKH) eksozomu ve dördüncü grupta ise 2ml 200 µg/mL umblikal kord kaynaklı kök hücre (UK-MKH) eksozomu subkutan olarak enjekte edildi. Bulgular: Eksozom uygulanan gruplarda yara boyutları ölçüm günlerinde kontrol grubuna göre istatistiksel olarak anlamlı azalma görüldü (p<0,05), ancak eksozom uygulanan gruplar arasında fark gözlenmedi. UK-MKH eksozomu ile kutanöz yara iyileşmesinde reepitelizasyon açısından kontrol grubuna göre anlamlı bir ilerleme olduğu gözlenirken (p<0,05); eksozom uygulanan diğer gruplarda kontrol grubuna göre anlamlı bir farklılık tespit edilmedi. Eksozom uygulanan gruplar arasında epitelizasyon açısından anlamlı bir fark olmamasına rağmen UK-MKH uygulanan grupta epidermal rejenerasyonun daha iyi olduğu gözlendi. Eksozom uygulanan gruplarda anjiyogenezde anlamlı bir artış elde edildi (p<0,05). Farklı kök hücrelerden elde edilen eksozomların birbirlerinden istatistiksel olarak belirlenmiş üstünlükleri olmasa da UK-MKH eksozomları uygulanan grupta ortalama damar sayısının diğer gruplara göre daha yüksek olduğu ve yeni oluşan kılcal dokunun daha iyi yapılandırıldığı gözlendi. Fibrozis skorlaması açısından UK-MKH eksozom uygulanan grupta diğer gruplara kıyasla kollajen yoğunluğunun arttığı ve kollajen dizilişinin daha düzenli olduğu gözlendi. Ancak kontrol grubu ve diğer eksozom verilen gruplardan istatistiksel olarak anlamlı farklılık tespit edilemedi. Sonuçlar: İnsan UK-MKH'lerin eksozomlarının diyabetik ratlarda yara iyileşmesindeki anjiogenezis ve reepitelizasyon basamaklarında diğer kök hücre kaynaklı eksozomlara göre daha etkin olduğu gösterilmiştir

Introduction and Objective: Mesenchymal stem cells (MSCs) play an important role in regeneration. The effect of exosomes derived from MSCs on wound healing has been shown in different studies. In the literature, there are many studies on the effect of exosomes on wound healing stages and the pathways through which this effect occurs. Although different stem cell sources were used in the studies in the literature review, it was noticed that they were not compared. The aim of our study was to compare the effects of adipose tissue-derived, bone marrow-derived and umbilical cord-derived exosomes on wound healing. Materials and Methods: 40 male Wistar Albino rats were divided into 4 groups of 10 rats in each group. Diabetes was induced in all groups with streptozotocin. Two circular skin defects were created on the back of each rat. In both skin defects, 2 ml saline solution was injected subcutaneously in the control group, 2 ml 200 µg/mL adipose-derived stem cell (AD-MSC) exosomes in the second group, 2 ml 200 µg/mL bone marrow-derived stem cell (BM-MSC) exosomes in the third group and 2 ml 200 µg/mL umbilical cord-derived stem cell (UCB-MSC) exosomes in the fourth group. Results: There was a statistically significant decrease in wound dimensions in the exosome-treated groups compared to the control group on the measurement days (p<0.05), but no difference was observed between the exosome-treated groups. While a significant improvement was observed in cutaneous wound healing with UK-MKH exosome in terms of reepithelialization compared to the control group (p<0.05); no significant difference was detected in the other exosome-treated groups compared to the control group. Although there was no significant difference in terms of epithelialization between the exosome treated groups, it was observed that epidermal regeneration was better in the UK-MKH treated group. A significant increase in angiogenesis was obtained in the exosome treated groups (p<0.05). Although the exosomes obtained from different stem cells were not statistically superior to each other, it was observed that the average number of vessels was higher and the newly formed capillary tissue was better structured in the UK-MKH exosome treated group compared to the other groups. In terms of fibrosis scoring, it was observed that collagen density increased and collagen arrangement was more regular in the UK-MKH exosome treated group compared to the other groups. However, there was no statistically significant difference between the control group and other exosome treated groups. Conclusions: Exosomes of human UK-MSCs were shown to be more effective than other stem cell-derived exosomes in angiogenesis and reepithelialization steps in wound healing in diabetic rats
