Özet
Bu araştırmada, annelere verilen WEB Destekli Emzirme Danışmanlığı'nın (WDED) ilk 6 ayda sadece anne sütü ile emzirme, emzirme motivasyonu, emzirme sorunları üzerine etkisinin belirlenmesi amaçlandı. Araştırma, tek merkezli, prospektif, iki kollu (1:1), randomize kontrollü deneysel tasarımdadır. Araştırma örneklemini 01 Ocak-01 Eylül 2023 tarihleri arasında Mersin Üniversitesi Hastanesi Kadın Doğum Servisi'nde doğum yapan 64 anne (müdahale grubu=32; kontrol grubu 32) oluşturdu. Araştırmada müdahale grubundaki annelere altı ay boyunca WDED verildi. Araştırma verileri, Anne Tanıtıcı Bilgi Formu, Doğum Sonu Emzirme Değerlendirme ve Takip Formu, Emzirme Motivasyon Ölçeği (EMÖ), Emzirme Sorunları Değerlendirme Ölçeği (ESDÖ) ile doğum sonrası ilk 24 saat, 1., 2., 4. ve 6. aylarda online olarak toplandı. Verilerin analizinde frekans, yüzde değerleri, ortalama, standart sapma, ki-kare (Pearson-χ2, Süreklilik Düzeltmesi) testi, Bağımsız örneklemler için t-testi, Tekrarlı ölçümlerde tek yönlü ANOVA ve Tekrarlı ölçümlerde çift yönlü ANOVA testleri kullanıldı. Araştırmanın deney kaydı Clinical Trials.gov adresinden prospektif olarak alındı (NCT05950048). WDED uygulaması sonrası müdahale grubundaki annelerin ESDÖ puan ortalamalarının kontrol grubundakilere göre doğum sonrası 1, 2., 4. ve 6. ay takip testlerinde anlamlı olarak azaldığı saptandı (p<0.005). WDED uygulaması sonrası müdahale grubundaki annelerin EMÖ bütünleşmiş düzenleme, içsel motivasyon ve özdeşleşmiş düzenleme alt boyut puan ortalamalarının kontrol grubundakilere göre doğum sonrası 1, 2., 4. ve 6. ay takip testlerinde anlamlı olarak arttığı saptandı (p<0.005). WDED uygulaması sonrası müdahale grubundaki annelerin EMÖ içe yansıtılmış düzenleme-toplumsal onaylama, içe yansıtılmış düzenleme-toplumsal baskı alt boyut puan ortalamalarının kontrol grubundakilere göre doğum sonrası 1, 2., 4. ve 6. ay takip testlerinde anlamlı bulunmazken (p>0.005), EMÖ dışsal düzenleme-ek yarar alt boyut puan ortalamalarının kontrol grubundakilere göre doğum sonrası 2., 4. ve 6. ay takip testlerinde anlamlı olarak azaldığı saptandı (p<0.005). Müdahale grubundaki annelerin çoğunluğunun WDED uygulamasını öğretici ve faydalı bulduğu saptandı. Araştırma sonuçları WDED uygulamasının doğum sonu dönemde annelerin emzirme sorunlarını azalttığı, emzirme motivasyonunu arttırdığı ve ilk altı ay sadece anne sütü ile beslenme oranını artırdığına dair kanıt sunmaktadır.

In this study, the first 6 months of Web-Assisted Breastfeeding Counseling (WBBC) given to mothers the effect of exclusive breastfeeding on breastfeeding, breastfeeding motivation, breastfeeding problems was aimed to determine. The study was a single-center, prospective, two-arm (1:1), randomized controlled experimental design. The sample of the study was conducted between January 01-September 01, 2023 in Mersin 64 mothers (experimental group=32; control group 32). In the study, the mothers in the experimental group received WDED for six months. given. The research data were collected using the Maternal Introductory Information Form, Postpartum Breastfeeding Assessment and Follow-up Form, Breastfeeding Motivation Scale (BMS), Breastfeeding Problems Assessment Scale (BPAS) was collected online in the first 24 hours, 1st, 2nd, 4th and 6th months postpartum. Data were collected In the analysis, frequency, percentage values, mean, standard deviation, chi-square (Pearson-χ 2, Continuity Correction) test, Independent samples t-test, One-way ANOVA with repeated measures and Two-way ANOVA tests were used for repeated measures. Experimental enrollment of the study Clinical Prospectively obtained from Trials.gov (NCT 05950048). After WDED application the mean scores of the mothers in the experimental group compared to those in the control group significantly increased in the 1st, 2nd, 4th and 6th month follow-up tests (p<0.005). WDED After the application, the mothers in the experimental group were asked to complete the ELS integrated regulation, intrinsic motivation and identified regulation subscale mean scores compared to those in the control group. significantly increased in the 1st, 2nd, 4th and 6th month follow-up tests (p<0.005). WDED After the intervention, the mothers in the experimental group were asked to complete the ELS internal reflected regulation-social approval, introjected regulation-social pressure subscale mean scores of control group was not found to be significant in the postnatal 1st, 2nd, 4th and 6th month follow-up tests, whereas (p<0.005), the mean scores of the extrinsic regulation-additional benefit sub-dimension of the significantly decreased in the postnatal 2nd, 4th and 6th month follow-up tests compared to those in the group (p<0.005). The majority of the mothers in the experimental group found the WED application instructive and found it useful. The results of the study show that WDED application in the postpartum period reduced mothers' breastfeeding problems, increased breastfeeding motivation, and reduced exclusive breastfeeding for the first six months. provides evidence that breastfeeding increases the rate of breastfeeding.
