Özet
Tek ayak üzerinde gözler kapatılmadan, bakışlar belirli bir noktaya sabitlenerek, rotasyonel hareketlerle gerçekleştirilen sema, görsel, somatosensoriyel ve vestibular sistemlerin çok iyi bir şekilde kullanımını gerektirmektedir. Semazenlerin uzun süreli kendi etrafında dönme sonucunda denge kaybı ve otonom semptom göstermemeleri her zaman için izleyicilerin ilgisini çekmiştir. Bu nedenle çalışma semazenlerde statik ve dinamik dengenin, dengeye etki eden duyusal alt sistemler ile beraber değerlendirilmesi amacıyla planlanmıştır. Çalışmaya Konya Türk Tasavvuf Müziği ve Sema Topluluğu'na bağlı 19 semazen ile 20 sema yapmayan sağlıklı gönüllü dahil edilmiştir. Statik denge değerlendirmesi için Balance Error Scoring System (BESS), dinamik denge değerlendirmesi için Star Excursion Balance Test (SEBT), vestibular sistemin değerlendirilmesi için Post Rotatory Nistagmus (PRN) testi kullanılmıştır. Vestibular, proprioseptif ve görsel sistemlerdeki etkilerin duyu bütünlüğü açısından davranışsal çıktılarının incelenmesi için Dunn Adolesan/ Yetiskin Duyu Anketi kullanılmıştır. Statik ve dinamik denge değerlendirmesinde semazen ve kontrol grubu arasında anlamlı bir fark bulunmamıştır (p<0.05). Semazenlerin sol tarafa PRN değerleri sağ tarafa ve kontrol grubuna göre anlamlı derecede düşük bulunmuştur (p<0.05). Duyu profili değerlendirmesinde kontrol grubunda daha yüksek duyusal arayış ve duyusal hassasiyet kaydedilmiştir (p<0.05). Semazenlerde denge fonksiyonu sema yapmayan sağlıklı bireylerden farklı değildir. Ancak tekrarlı dönme aktivitesine bağlı olarak sol tarafa dönüşte vestibular sistemde duyarsızlaşma olduğu bulunmuştur. Vestibular sistemin tek başına, tek yöndeki duyarsızlaşması statik ve dinamik dengeyi etkilememektedir. Duyu profilleri karşılaştırıldığında kontrol grubunda daha fazla duyusal hassasiyet ve duyu arayışı davranışı görülmüştür. Semazenlerde bu davranışların daha düşük olması sema aktivitesinin regüle edici özelliğinden kaynaklanmaktadır.

Sema is an activity characterized with the whirling of the Sufi dervishes around themselves with eyes open and focused on a spot while turning on a single leg. Integration of visual, somatosensorial, vestibular systems are required for the activity. Sufi's ability of staying in balance, and not showing any autonomic symptoms after whirling for such a long time, has always been interesting and fascinating for the audience. The study is designed to investigate the Sufi's static & dynamic balance characteristics and their sensory components. The study in conducted on the volunteered members of the Ministry of Culture's Konya Turkish Sufi Music and Sema Assembly. 19 whirling dervishes and 20 healthy members of the assembly who do not participate in sema activity were volunteered for the assessment. Balance Error Scoring System (BESS) is used for static balance evaluation and Star Excursion Balance Test (SEBT) is used for the dynamic balance evaluation. Post Rotatory Nystagmus (PRN) test is used to measure nystagmus time as an indicator of vestibular function. Dunn Adolescent & Adult Sensory Profile is used to evaluate the behavioral outcomes of the changes in the vestibular, proprioceptive and visual systems. For static and dynamic balance scores no significant difference was found between the Sufi group and the controls (p<0.05). Sufi groups left PRN measures were found to be significantly less compared to the right and the controls on both sides (p<0.05). Dunn Adolescent & Adult Sensory Profile showed more sensory seeking and sensory defense for the control group (p<0.05). Balance of the whirling dervishes are not different then the controls additionally there is high amount of desensitization on the left vestibular response due to the whirling activity to the left. Unilateral desensitization of the vestibular system does not seem to effect the balance. When sensory profiles compered it is found that there is less sensory seeking and defense in the Sufi group to the controls therefore sema activity of the whirling dervishes might be a regulatory activity for those individuals.
