Özet

Bu çalışmanın amacı farklı temporomandibular disfonksiyon (TMD) tiplerinde ilerleyici boyun motor kontrol egzersizlerinin kranioservikal ağrı, postür, fonksiyon ve kinezyofobi üzerindeki etkisini incelemekti. Çalışmaya, miyofasyal TMD, redüksiyonlu disk deplasmanı ve redüksiyonsuz disk deplasmanı tanısı olan 60 kadın katılımcı dahil edildi ve her TMD tipi için bir çalışma (ÇG1–ÇG3) ve bir kontrol grubu (KG1–KG3) olmak üzere toplam altı grup oluşturuldu. Çalışma gruplarına 6 hafta egzersiz tedavisi ile hasta eğitimi, kontrol gruplarına ise yalnızca hasta eğitimi uygulandı. TMD şiddeti Fonseca Anamnestik İndeks ile, ağrı şiddeti Numerik Ağrı Skalası (NRS) ile, basınç ağrı eşikleri Algometre ile, temporomandibular eklem hareket açıklıkları (EHA) kaliper ile, servikal bölge EHA'ları ve propriosepsiyonu Cervical Range of Motion (CROM) cihazıyla değerlendirildi. Derin boyun fleksör enduransı Stabilizer Biofeedback cihazıyla, postür değerlendirmeleri fotoğraf analiziyle yapıldı. Mandibular fonksiyonlar Mandibular Fonksiyon Bozukluk Anketi, kinezyofobi Tampa Kinezyofobi Ölçeği–TMD ile değerlendirildi. Tedavi sonrasında tüm ÇG'lerde TMD istirahat ağrısında (p=0,007, p=0,020, p=0,007) ve aktivite ağrısında (p=0,005, p=0,007, p=0,005) azalma gözlendi. Kontrol gruplarında ağrı şiddetlerindeki iyileşme sınırlıydı ve özellikle KG2'de anlamlı değişiklik saptanmadı (p>0,05). Tedavi sonrasında ÇG'lerde basınç ağrı eşiklerinde artış görüldü (p<0,05), KG'lerde ise bu artışlar sınırlıydı. TME ve servikal EHA'ları ÇG'lerde genel olarak artış gösterirken (p<0,05), KG'lerde bu artışlar daha sınırlıydı. Aktif ağız açıklığındaki artış en fazla ÇG3'te, servikal fleksiyon ve rotasyonlardaki artış ise daha çok ÇG2'de gözlendi (p<0,017). Propriosepsiyon sonuçlarında iyileşme sağlanamadı (p>0,05). Tedavi sonrasında ÇG'lerin enduranslarında artış vardı (p=0,008, p=0,026, p=0,021). Kontrol gruplarında ise sadece KG2'de artış gözlendi (p<0,007). Postür sonuçlarında ÇG1 ve Ç3'te iyileşmeler gözlenirken ÇG2'de sadece bir parametrede anlamlı iyileşme vardı, KG'lerde ise bu iyileşmeler sınırlıydı. Özellikle ÇG3'te daha anlamlı iyileşmeler saptandı (p<0,017). ÇG'lerde mandibular fonksiyonlarında ve kinezyofobi düzeylerinde azalma görüldü (p<0,05), KG'lerde ise sadece KG3'de toplam mandibular fonksiyon ve kinezyofobi düzeylerinde iyileşme vardı (p=0,028, p=0,005). Bu çalışmanın sonuçları, farklı TMD tiplerinde uygulanan ilerleyici boyun motor kontrol egzersizlerinin, hasta eğitimine kıyasla kranioservikal ağrı, fonksiyon, endurans, postür ve kinezyofobi üzerinde daha fazla iyileşme sağladığını; bu iyileşmelerin bir ay sonraki takiplerde de büyük çoğunlukla devam ettiğini göstermektedir. İlerleyici boyun motor kontrol egzersizlerinin, özellikle TME redüksiyonsuz disk deplasmanı olan bireylerde daha etkili olduğu görülmekte olup, bu durum söz konusu hastaların ilk basamakta fizyoterapistlere yönlendirilmesinin önemini ortaya koymaktadır.

The aim of this study was to investigate the effects of progressive neck motor control exercises on craniocervical pain, posture, function, and kinesiophobia in different types of temporomandibular joint dysfunction (TMD). The study included 60 female participants diagnosed with myofascial TMD, disc displacement with reduction, and disc displacement without reduction, and a total of six groups were formed, consisting of one study group (SG1–SG3) and one control group (CG1–CG3) for each type. The SGs received 6 weeks of exercise therapy and patient education, while the CGs received only patient education. TMD severity was assessed using the Fonseca Anamnestic Index, pain intensity using the Numerical Pain Scale, pressure pain thresholds using an algometer, temporomandibular joint (TMJ) range of motion (ROM) using a caliper, and cervical ROM and proprioception using the Cervical Range of Motion (CROM) device. Deep neck flexor endurance was assessed using the Stabilizer Biofeedback, and postural evaluations were performed via photographic analysis. Mandibular functions were assessed using the Mandibular Function Impairment Questionnaire, and kinesiophobia was evaluated using the Tampa Kinesiophobia Scale for TMD. Following treatment, a reduction in TMD resting pain (p<0.05) and activity-related pain (p<0.05) was observed in CGs. Improvement in pain was limited in the CGs, and no significant changes were detected, particularly in CG2 (p>0.05). There were increases in pressure pain thresholds in most variables in the SGs (p<0.05), while these increases were limited in the CGs. TMJ and cervical ROM generally increased in the SGs (p<0.05), whereas these increases were more limited in the CGs. The greatest increase in active mouth opening was observed in the SG3 (p<0.017). No improvement was observed in proprioception (p>0.05). There was an increase in endurance in SGs (p<0.05). In the CGs, an increase was observed only in CG2 (p<0.007). Improvements in postural outcomes were observed in SG1 and SG3, while in SG2, a significant improvement was noted in only one parameter; in the CGs, these improvements were limited. Notably, more significant improvements were observed in SG3 (p<0.017). In SGs, there was a decrease in mandibular function, and kinesiophobia (p<0.05); in the CGs, however, improvements in mandibular function and kinesiophobia were observed only in CG3 (p<0.05). Findings indicate that progressive neck motor control exercises, when applied to different types of TMD, result in greater improvements in craniocervical pain, function, endurance, posture, and kinesiophobia compared to patient education; these improvements largely persisted at the one-month follow-up. Progressive neck motor control exercises have been shown to be more effective, particularly in individuals with TMJ disc displacement without reduction, highlighting the importance of referring these patients to physiotherapists as initial care.
