Özet

Amaç: Dünya genelinde geriatrik nüfusta artış izlenmektedir. Geriatrik hastaların, uzamış yaşam süresi ve artan yaşam standartları ile daha mobil olmaları, travmaya daha açık hale gelmelerine sebep olmaktadır. Bu şekilde acil servislerde de geriatrik başvurular artış göstermektedir. Geriatrik hastalarda mortalite ve morbiditeyi ön görmek, hastane yatış/taburculuk durumlarına karar vermek acil servis şartları için önem arz etmektedir. Biz bu çalışmada, travma ile başvuran geriatrik popülasyonda skorlama sistemlerinin; prognoz ve hastane yatış/yatış süresini belirleme üzerinde anlamlı bir etkisinin olup olmadığını belirlemeyi amaçladık. Gereç ve Yöntem: Çalışmamız SBÜ Ankara Eğitim ve Araştırma Hastanesi Acil Tıp Kliniği'nde etik kurul onayı alındığı 22/06/2023 tarihi itibariyle 6 ay süre ile yapılmış olup hedeflenen hasta sayısına ulaşıldığında tamamlanmıştır. Çalışmamız prospektif, gözlemsel bir çalışmadır. Çalışmamıza acil servise travma sebebiyle başvuran 65 yaş ve üstü geriatrik popülasyon dahil edilmiştir. Çalışmamızda hastaların belirlediğimiz skorlama sistemlerine (Emergency Severity Index, Trauma Injury Severity Score, Charlson Comorbidity, Clinical Frailty Index, Trauma Spesific Frailty Index) göre aldığı puanlar hesaplanmıştır. Hastalar exitus ve yaşayanlar olarak iki gruba ayrılmıştır. Yaşayan hastalar, yatışı yapılan ve taburcu edilen hastalar olarak gruplandırılmıştır. Bulgular: Çalışmamıza 499 hasta dahil edilmiştir. Hastaların %99.6'sının yaşadığı (n=497) tespit edilmiştir. Yaşayan hastaların %84.6'sının taburcu olduğu(n=422), %15.4'ünün yatışının yapıldığı tespit edilmiştir. Hastaların %45.1'inde son bir ayda tekrar hastane başvurusu olduğu saptanmıştır. Hastaların %38.1'ine konsültasyon istemi yapılmıştır. En sık konsültasyon istenen branş Ortopedi ve Travmatoloji (%72.1) olarak belirlenmiştir. Hastaların sonlanım durumlarını belirlemek üzere skorlama sistemleri değerlendirildiğinde, yatışı öngörmek için belirlenen 5 skorlama sistemi de anlamlı bulunmuştur (p<0.001). Yatış sonlanımını belirlemede Emergency Severity Index ve Trauma Injury Severity Score'un AUC değerleri diğerlerine kıyasla daha yüksek olduğu belirlenmiştir (sırasıyla 0.843 ve 0.835). Son 1 ay içinde tekrar hastane başvurusunu öngörmede Trauma Injury Severity Score anlamlı olarak bulunmuştur (p<0.001). Sonuç: Acil servise travma ile başvuran geriatrik hastaların değerlendirmesinde triyajın doğru yapılması önem arz etmektedir. Hastanın mevcut klinik durumu ile birlikte kırılganlık durumunun da dahil edilmesi ve skorlama sistemlerinin kullanılması daha erken ve daha doğru müdahele imkânı sağlayacaktır.

Objective: The geriatric population is increasing worldwide. Geriatric patients are more mobile, with prolonged life expectancy and increased living standards, making them more vulnerable to trauma. In this way, geriatric admissions to emergency departments are also increasing. Predicting mortality and morbidity in geriatric patients and deciding on hospitalization/discharge situations are important for emergency department conditions. In this study, we aimed to determine whether scoring systems have a significant effect on prognosis and length of hospitalization/stay in geriatric populations admitted with trauma. Materials and Methods: Our study was conducted in the Emergency Medicine Clinic of Ankara Training and Research Hospital, University of Health Sciences, for six months as of 22/06/2023, when the ethics committee approval was obtained and was completed when the targeted number of patients was reached. Our study is a prospective, observational study. The geriatric population aged 65 years and older who presented to the emergency department due to trauma were included in our study. In our study, the scores of the patients were calculated according to the scoring systems (Emergency Severity Index, Trauma Injury Severity Score, Charlson Comorbidity, Clinical Frailty Index and Trauma Specific Frailty Index). Patients were divided into two groups: exitus and survivors. Surviving patients were grouped as hospitalized and discharged patients. Results: 499 patients were included in our study. It was determined that 99.6% of the patients were alive (n=497). Of the living patients, 84.6% were discharged (n=422) and 15.4% were hospitalized. It was found that 45.1% of the patients were readmitted to the hospital in the last month. Consultation was requested for 38.1% of the patients. Orthopedics and Traumatology were the most common specialties for which consultation was requested (72.1%). When scoring systems were evaluated to determine the outcome of the patients, all five scoring systems determined to predict hospitalization were found to be significant (p<0.001). The AUC values of Emergency Severity Index and Trauma Injury Severity Score were higher than the others in determining the outcome of hospitalization (0.843 and 0.835, respectively). Trauma Injury Severity Score was found to be significant in predicting readmission in the last month (p<0.001). Conclusion: Correct triage is essential in the evaluation of geriatric patients presenting to the emergency department with trauma. The inclusion of the patient's current clinical status and frailty status and the use of scoring systems will provide earlier and more accurate intervention.
