Özet
Amaç: Fasiyal paralizi, gözün tam olarak kapatılamaması, alnın kasılamaması ve yüz asimetrisiyle seyreden önemli bir cerrahi komplikasyondur. Nöroşirurjideki teknik ilerlemelerle beraber, tarihsel gelişim sürecinde farklı yaklaşımlar ve tekniklerin tarif edilmesiyle bu komplikasyon oranı belirgin olarak azalmıştır ancak bu komplikasyon hala görülebilmektedir. Bu çalışmada, özellikle frontotemporal bölgede, fasiyal sinirin ilgili anatomik katmanlar arasındaki yerleşimi ve seyri göz önüne alınarak, temel anatomik işaret noktalarına uzaklıkları hesaba katılarak, cerrahi komplikasyonla karşılaşma ihtimalini artıran tehlikeli bölgelerin tespit edilmesi amaçlanmıştır. Gereç ve Yöntem: Bu çalışmada 10 adet formalin fikse cephalus kadavra kullanılmıştır ve her kadavranın iki yüz yarısı da kullanılarak toplam 20 farklı fasiyal sinir ekstrakraniyal segmentinin diseksiyonu yapılmıştır. Pteriyonal ve supraorbital yaklaşıma uygun insizyon ve temel cerrahi teknikler kullanılarak frontotemporal bölgede scalp tabakaları kat kat ortaya kondu ve fasiyal sinirin frontotemporal dalının bu tabakalar arasındaki seyri ve inerve ettiği kaslara giriş noktaları ortaya konuldu. Belirlenen anatomik işaretler kullanılarak fasiyal sinir dallarının bu noktalara uzaklıkları ölçüldü. Bulgular: Tüm örneklerde fasiyal sinir dallarının diseksiyonu başarıyla gerçekleştirildi. Arcus zigoma seviyesinde tragusun 2 cm anterioruna kadar fasiyal sinirin herhangi bir frontotemporal dalına rastlanmadı. Bu seviyede frontotemporal dalın en ön ve en arka dalı arasındaki mesafe ortalama 16 mm olarak ölçüldü. Lateral kantus seviyesinde, en ön ve en arka dal arasındaki mesafe 30 mm olarak ölçüldü. Supraoptik yaklaşımda ise, kaş içerisine yerleştirilen bir insizyonda supraoptik çentiğin yaklaşık 47 mm lateraline kadar fasiyal sinirin frontotemporal dalının geçmediği görüldü. Kesinin frontozigomatik sutur üzerinde ilerletilmesi halinde, frontozigomatik bileşkenin ortalama 20 mm lateraline kadar fasiyal sinir dalıyla karşılaşılmadı. Sonuç: Pterional yaklaşımda fasiyal sinirin frontotemporal dalının arcus zigoma seviyesinden geçerken genel olarak 3 ana dala ayrıldığı, bu dalların antero-superior yönde ilerleyerek orbicularis oculi ve frontooksipital kası inerve ettiği görüldü. Lateral epikantus seviyesinde, bu dalların gittikçe yüzeyelleştiği; en ön ve en arka sinir arasındaki mesafenin giderek arttığı görüldü. Supraorbital yaklaşımda ise supraoptik çentiğin 47 mm lateraline kadar insizyonun güvenli olduğu, bu insizyonun frontozigomatik sutura doğru uzatılması halinde, frontozigomatik bileşkenin ortalama 20 mm lateralinin güvenli olduğu görüldü.

Purpose: Facial paralysis is an important surgical complication that results in inability to close the eye completely, inability to contract the forehead, and facial asymmetry. With the technical advances in neurosurgery and the description of different approaches and techniques in the historical development process, the rate of this complication has decreased significantly, but this complication can still be seen. This study aimed to identify dangerous areas that increase the likelihood of encountering surgical complications, especially in the frontotemporal region, by taking into account the location and course of the facial nerve between the relevant anatomical layers and their distance to basic anatomical landmarks. Materials and Methods: In this study, 10 formalin-fixed cephalus cadavers were used, and a total of 20 different facial nerve extracranial segments were dissected using both facial halves of each cadaver. Using incisions and basic surgical techniques suitable for the pterional and supraorbital approach, the scalp layers in the frontotemporal region were revealed layer by layer, and the course of the frontotemporal branch of the facial nerve between these layers and the entry points to the muscles it innervated were revealed. Using the determined anatomical landmarks, the distances of the facial nerve branches to these points were measured. Results: Dissection of facial nerve branches was successfully performed in all cadavers. No frontotemporal branch of the facial nerve was found until 2 cm anterior to the tragus at the level of the arcus zygoma. At this level, the distance between the most anterior and posterior branches of the frontotemporal branch was measured as 16 mm on average. At the level of the lateral canthus, the distance between the most anterior and posterior branches was measured as 30 mm. In the supraoptic approach, it was observed that the frontotemporal branch of the facial nerve did not pass until approximately 47 mm lateral to the supraoptic notch in an incision placed inside the eyebrow. If the incision was advanced on the frontozygomatic suture, no facial nerve branches were encountered until an average of 20 mm lateral to the frontozygomatic junction. Conclusion: In the pterional approach, it was observed that the frontotemporal branch of the facial nerve generally divided into 3 main branches as it passed through the arcus zygoma level, and these branches proceeded in the antero-superior direction and innervated the orbicularis oculi and frontoccipital muscle. At the level of the lateral epicanthus, these branches become increasingly superficial; It was observed that the distance between the most anterior and posterior nerves gradually increased. In the supraorbital approach, an incision up to 47 mm lateral to the supraoptic notch was found to be safe. If this incision was extended towards the frontozygomatic suture, an average of 20 mm lateral to the frontozygomatic junction was found to be safe.
