Özet

AMAÇ: Vokal kordlarda gelişen ödem sesin frekansı, yoğunluğu, kalitesi, rezonansı ve aerodinamik ölçülerini etkilemektedir. Dekompanse karaciğer sirozu tanılı hastalarda meydana gelen vücuttaki yaygın ödemin vokal kordları da etkilemesine bağlı olarak sesin akustik parametrelerinde değişiklik beklenmektedir. Çalışmamız, dekompanse karaciğer siroz tanılı hastalarda sesin akustik parametrelerini kompanse karaciğer siroz tanılı hasta ve kontrol gruplarıyla karşılaştırarak bu parametrelerde değişiklik meydana gelip gelmediğini göstermeyi amaçlamaktadır. GEREÇ ve YÖNTEM: Çalışmamız tek merkezli, prospektif bir vaka kontrol çalışmasıdır. Çalışmamıza tek merkezden polikliniğe başvuran 32 dekompanse karaciğer sirozu tanılı, 32 kompanse karaciğer sirozu tanılı ve 32 kontrol grubu olmak üzere toplam 96 hasta dahil edilmiştir. Toplamda 96 hastanın yer aldığı çalışmada sesin akustik parametreleri karşılaştırıldı. BULGULAR: Çalışmamız 01/03/2023-01/12/2023 tarihlerinde Sultan 2. Abdülhamid Han Eğitim ve Araştırma Hastanesinde yapılmış olup %54.2'si kadın (n:52), %45.8'si erkek (n:44) hastadan oluşmaktadır. Çalışmamızda 32 dekompanse karaciğer sirozu tanılı, 32 kompanse karaciğer sirozu tanılı ve 32 kontrol grubu olmak üzere toplam 96 hastada sesin akustik parametreleri incelenmiştir. Dekompanse siroz tanılı hasta grubunda yüzde jitter, yüzde shimmer, harmonik gürültü oranı (HNR) ve maksimum fonasyon zamanı (MFZ) değerlerinde kompanse siroz tanılı hasta grubu ve kontrol grubuna göre anlamlı farklılıklar izlendi (P0.05). SONUÇ: Çalışmamız dekompanse karaciğer siroz tanılı hasta grubunda kompanse karaciğer sirozu tanılı hasta ve kontrol grubuna göre sesin akustik parametrelerinde anlamlı farklılık olduğunu göstermiştir. Bu konuda daha fazla çalışmaya ihtiyaç duyulsa da çalışmamız kompanse karaciğer sirozun, dekompanse karaciğer sirozuna ilerleyişini öngörmede noninvaziv bir yöntem olarak kullanılabileceğini göstermektedir.

OBJECTIVES: Edema in the vocal cords affects the frequency, intensity, quality, resonance and aerodynamic dimensions of the voice. In patients with decompensated liver cirrhosis, changes in the acoustic parameters of the voice are expected due to the widespread edema in the body affecting the vocal cords. Our study aims to compare the acoustic parameters of the voice in patients with decompensated liver cirrhosis with patients with compensated liver cirrhosis and control groups and to show whether changes occur in these parameters. METHODS: Our study is a single-center, prospective case-control study. A total of 96 patients, including 32 patients with decompensated liver cirrhosis, 32 patients with compensated liver cirrhosis, and 32 controls, who applied to the outpatient clinic from a single center were included in our study. Acoustic parameters of the voice were compared in a total of 96 patients. FINDINGS: Our study was conducted between 01/03/2023-01/12/2023 at Sultan 2nd Abdulhamid Khan Training and Research Hospital and consisted of 54.2% female (n:52) and 45.8% male (n:44) patients. In our study, acoustic parameters of the voice were analyzed in a total of 96 patients including 32 patients with decompensated liver cirrhosis, 32 patients with compensated liver cirrhosis and 32 control group. Significant differences were observed in percent jitter, percent shimmer, harmonic noise ratio (HNR) and maximum phonation time (MFR) values in the decompensated cirrhosis patient group compared to the compensated cirrhosis patient group and the control group (P0.05). CONCLUSIONS: Our study showed that there was a significant difference in acoustic parameters of the voice in decompensated liver cirrhosis patients compared to compensated liver cirrhosis patients and control group. Although more studies are needed on this subject, our study shows that it can be used as a noninvasive method to predict the progression of compensated liver cirrhosis to decompensated liver cirrhosis.
